I agree and understand that participation in basketball involves significant risks or


death associated with participation in community. These risks include the possibility
of very serious injuries which can occur for a variety of reasons. Participants and
parents assume full responsibility for all injuries and damages which may occur in or
at any of Basketball League events.

I/We have read this participation, assumption of risk and agreement, and have
signed it freely and voluntarily without any inducement.
