1. SCOPE AND PURPOSE

The STARRT is an analysis of work tasks, each day, conducted by team leaders and involving the
employees in the identification and correction of hazards. The process also encourages feedback from
employees to provide continuous improvement to CONTRACTOR, its Subcontractors and project safety
processes. The STARRT will also consider and include the impact that planned work tasks may have on
other Subcontractors or work groups.
To define the way that safety is considered by individual work teams prior to starting work each day. It is
an essential element in developing the behaviour necessary to achieve the vision of zero injuries.
The Safety Task Analysis, Risk Reduction Talk (STARRT) provides a method for employees to have
input to the safety process through suggestions and voicing concerns.
This procedure must be followed by CONTRACTOR and Subcontractor employees in CDB Melchorita
Marine Facilities Project.

NOTE: CDB policy strictly forbids retaliation or reprisal against an employee for reporting a health, safety
or environmental concern.

3. DEFINITIONS

PROJECT: PERU LNG Export Project Marine Facilities EPC Contract


OWNER: PERU LNG S.R.L.
CONTRACTOR: CDB Melchorita, Consortium between the Join Venture Saipem s.a. /
Odebrecht and Jan de Nul
Any third party company (personnel), supervised by CONTRACTOR to
perform any specific job in the site, such as: construction personnel,
maintenance, etc.
SUBCONTRACTOR Any person who requests for getting inside of the project, in order to
perform activities such as: inspections or visits, vendors, personnel of
the other contractor companies who came for meetings, material
deliveries, etc.

4. ACTIVITIES DESCRIPTION

4.1 RESPONSABILITIES
When an activity is defined for a certain position in this procedure, the responsibility remains with
that person although a designee may perform the implementation of that activity. The appointed
designee must report directly to the assigned individual.

4.1.1 Construction Manager (CM)


The CM has the overall responsibility for this procedure and is responsible for supporting
it and for ensuring that all entities at the jobsite actively participate.
The CM is responsible for providing the personnel, facilities, and other resources
necessary to effectively implement this procedure.
The CM and the management team will lead by example, modeling the behavior
expected from all personnel performing work at the jobsite.

4.1.2 HSES Manager


HSES Manager, in conjunction with the CM, is responsible for implementing and
administering this procedure.
The HSE manager will:
• Function as an adjudicator on any issues arising from the implementation of this
procedure.
• Audit CONTRACTOR and Subcontractors to ensure the requirements of this
procedure are implemented and will provide feedback on performance to
CONTRACTOR and Subcontractor Management.
• Assist to the Subcontractor to plan and coordinate the work effectively implement
the requirements of the procedure.
• Ensure the required HSES records are generated and available for review by
management and authorities as appropriate.
• Ensure the existence of a system used to retain STARRT cards on files for
duration of the project.

4.1.3 Field Supervision


The CONTRACTOR’s managers, supervisors, foremen and the HSES personnel are not
expected to attend every STARRT in their area. They should aim to attend regular
STARRT for each work team in their area. Their role is to observe and provide
encouragement. Where necessary, they should coach/mentor the person leading the
STARRT to improve and develop the process. They should become involved as
necessary to answer questions and address concerns.

4.1.4 Subcontractor Construction Manager


• Has the responsibility for compliance with this procedure and shall provide the
personnel, facilities, and other resources necessary to effectively implement,
administer and enforce this procedure.
• Is responsible for the effective dissemination and education of the requirements
throughout the subcontractor and its lower level organizations.

• Will ensure that all subcontractor employees and all lower tier subcontractors
comply and actively participate with its requirements.
• Will carry out audits, to ensure that STARRT meetings are carried out by each
work team at the start of each shift;
• Enforce supervisor’s involvement in conducting the STARRT Meeting;
• Ensures Subcontractor’s HSES personnel and field engineering personnel attend
regularly;
• Ensures that issues raised by employees are followed up and that feedback on
corrective action taken is provided at the next STARRT meeting.

4.1.5 Subcontractor HSE Manager


• Subcontractor HSE Manager shall be responsible for the continuous monitoring
of the implementation of this procedure.
• Subcontractor HSE Manager will provide feedback on performance and assist
Subcontractor Supervision to plan and coordinate the work to effectively
implement the requirements of the procedure.

4.1.6 Subcontractor supervisor


• Subcontractor supervisor shall be thoroughly familiar with this procedure and with
their individual responsibilities regarding its implementation and enforcement.
• Subcontractor supervisor shall ensure that only competent persons are assigned
work tasks. This includes ensuring the worker has the skills, physique and
knowledge to safely execute the work task. Conducting the STARRT
• Subcontractor supervisor must conduct the STARRT ensuring that it remains
focused on safety and not employee Relation / Industrial Relation issues

4.1.7 Employees
• Employees must receive appropriate training in order to know, understand and
comply with the health, safety and environmental requirements of this procedure
as applicable to the work they perform. Attend the STARRT in their area each
day Participate in the STARRT by raising questions or concerns relating to safety
• Attend the STARRT each day in their assigned area, raising questions or
concerns relating to safety
• Employee must report to his Supervisor any equipment malfunction that may
effect the safe operation of the equipment.
• Employee must advise their immediate Supervisor whenever unsure of the
instructions for a task or where concerned about the safety status of any task.

4.2 INSTRUCTIONS

4.2.1 Guidelines for STARRT


Supervisors will ensure employee involvement in the STARRT process is optimized by:
• Ensuring that all employees under their charge are trained in the STARRT
process
• Establishing a system to follow-up with all of their crew members to verify the
STARRT process is being correctly implemented
• Requiring their employees to participate in an individual STARRT review prior to
the start of each shift for each new task

Supervisors will discuss the hazards of the planned work activity with the employees to
compile information necessary to complete the STARRT card:
• Upon completion of the STARRT card, supervisors will review the requirements
of the card with the employees.
• The employees performing the work and supervisor will sign the card.
• The card will be displayed and available for review in the immediate work area.
STARRT cards are to be collected by the supervisor and forwarded to the Subcontractor
HSE Manager at the end of each shift or completion of the work task.

The supervisor is responsible for ensuring that cards are properly completed and
returned.
STARRT cards are to be reviewed for HSE follow-up requirement and for proper
usage/completion by the Subcontractor HSE Manager. STARRT cards to be maintained
on file in the Subcontractor HSE Office.

4.2.2 STARRT Meeting Preparation


Prior to the STARRT meeting, the Subcontractor’s Supervisor will complete the STARRT
meeting -start checklist (attached), identifying anticipated hazards for the tasks of their
employees. These hazards, together with any others raised during the talk, will be
discussed with the employees to ensure the mutual understanding of both the hazard and
the precautions and preparations necessary to complete the specific work tasks, safely.

4.3 STARRT FORMAT


The Subcontractor’s Supervisor will use the following broad format to conduct the
STARRT meeting:
• Review previous day’s safety effort and outcome by asking questions e.g.
• “How did we do yesterday?”
• “Were there any incidents?”
• “Was the correct equipment available to allow the work to proceed safely?”
• Did the equipment operate correctly?”
• “Did the work progress as planned?”
• “What brought about any changes to the plan?”
• Briefly overview today’s tasks, focusing on what safety hazards may be faced.
• Discuss in detail, the foreseen hazards and the planned control measures for
these hazards – refer to STARRT checklist and get employee input and
involvement.
NOTE: On the STARRT Card any additional hazards identified as a result of the
discussion and the control measures agreed to with the employees.
• Finish the meeting with a positive and encouraging comment.
Employees are encouraged to make comment and suggestions regarding the tasks and
hazards as outlined by their supervisor. In closing the meeting each day, employees will
be asked for improvement suggestions and any concerns they may have regarding the
tasks for the day.

4.4 STARRT FOLLOW-UP


Supervisors shall ensure that issues raised in the STARRT are followed up during the
day.
Feedback will be provided to the employee who raised the issue during the day and
reviewed at the next day’s STARRT meeting.
Immediate feedback to the employee is an absolute requirement of this safety system. It
is not optional.
Supervisor to ensure that STARRT card is handed to the Area manager for his review,
action and signature.
Area Manager is to ensure that all outstanding HSE action items identified are resolved
as soon as practical.
Subcontractor HSE Manager will review, comment and conduct check for the timely
implementation of identified actions.
Subcontractor HSE Manager will retain STARRT cards on file for duration of the project
or as required by PRC HSE regulations.
