This is to declare that the Lease Account(s) above do not have any facilitating Leasing Agent / Broker / Representative / Attorney-in-Fact. I/we
personally handle/manage the property as lessor(s). I/we also declare that no Lessor-Agency Agreement nor Lease Faciliation Agremeent
with a third-party agent/broker has been executed for this declared account.

Should there’ll be changes / updates to this, I/we will notify in writing the Property Management Office (PMO). Non-notification will treat this
document as valid, binding, and in effect.

Note: This shall be accomplished and submitted to PMO on a per account basis, as applicable.

Corporation Data Privacy Policy. By submitting this form, you agreed to the following: DMCI Project Developers Inc., (DMCI Homes) and the Condo Corporation Data Privacy Policy and its terms.
Unit Owner Requirements:
• Copy of Valid ID
• Updated Statement of Account from PMO

Note: Provide the lease documentation to the PMO three (3) days before the intended move-in of the Tenant. For Guests, accomplish
requirements/forms ahead during working hours.

Tenant Move-in Requirements


• Accomplished and Approved Tenancy Pass Form
• Notarized Contract of Lease with required Annexes/Addendum
• At least 1 primary ID or 2 secondary IDs for the principal registered lessee
If lessee is a foreigner, submit:
➢ Alien Certificate of Registration Identity Card (ACR I-Card)
➢ Foreign Passport
• Registered lessee must submit any one (1) of the following:
➢ NBI Clearance
➢ Police Clearance
➢ Barangay Clearance (from lessee’s former residence)
• Fill out the Community Residency Application online form
• Orientation of House Rules and Regulation
• Tenant Information Sheet

Tenant Move-out Requirements


• Move-out Notification (at least 5 days before the actual moveout whether pretermed, backout or finished contract)
• Move-out Clearance
• Gate Pass (Pullout)

Note: Provide the lease documentation to the PMO three (3) days before the intended move-out of the Tenant. For Guests, accomplish
requirements/forms ahead during working hours.

This guideline may be subject to changes/updates as deemed necessary to effect improvements.
