    AFFIDAVIT OF DENIAL

    I, ROSELA ACSE, of legal age, Filipino, and a resident, Silay City, Negros
    Occidentall, Philippines, after having been duly sworn to in accordance with law
    hereby depose and say:

    1. That I am a Master Teacher I at Doña Montserrat Lopez Memorial


    High School, Silay City, Negros Occidental.

    2. That on June 30, 2016 at 8:40 in the morning, I filed my application


    for leave to travel abroad for twenty (20) days thru the endorsement of my
    principal Mr. Wilfredo O. Hermosura.

    3. That I have already submitted all the requirements for the said
    application for leave and lately the Schools’ Division Superintendent, Dr. Neri
    Anne M. Alibuyog thru Administrative Officer V, Jowey C. Alimon, in a letter
    dated July 21, 2016, required me a final requisite which is for me to submit this
    affidavit of denial.

    4. That in compliance to the said requirement I hereby state that as of


    this writing I have not received any complaint whether criminal or administrative
    against me from Office of the Ombudsman or other investigating body.

    5. That if and when I receive such complaint I hereby undertake to


    inform the Department of Education of such complaint and I further undertake
    to personally face any and all complaints against me in the matter and the
    procedure prescribed by law and that i will come back to the country as soon as
    my leave has been fully served.

    6. That I am executing this affidavit to attest to the truth of the


    foregoing facts.
