PATIENTS BILL OF RIGHTS

Patient’s Rights:
1. You have the right to receive considerate, respectful and compassionate health care in a
safe setting regardless of your age, sex, gender, religion, ethnicity, political affiliation,
disability or capacity to pay free from all forms of abuse, neglect, or ill treatment.

2. You have the right to be assigned a competent physician and be informed of the names of
all health care team members involved in your care. Additionally, you have the right to
know the possible fees and receive information about financial assistance.

3. You have the right to notify a family member or person of your choice of your medical
transaction in the primary care facility.

4. You have the right to have someone remain with you during your medical transaction in
the facility unless it compromises your or others’ rights, safety or health.

5. You have the right to exercise your spiritual and cultural belief within the capacity and
rules of the facility.

6. You have the right to be informed and give consent before any non-emergency
procedures or research/experiment or to refuse such procedure or participation.

7. You have the right to privacy and confidentiality of your medical records and care
discussions, examinations, and treatment, as well as to see or obtain a copy of your
medical records except those records restricted by law.

8. You have the right to request an escort during physical examinations.

9. You have the right to be represented by someone (assignee) to decide on your behalf
when the circumstances warrant it.

10. You have the right to ask about and be informed of the complaint process and express
grievances without fear of recrimination or reprisal. You are encouraged to speak directly
to the health care provider involved in your care.
Patient’s Responsibilities:

1. You are responsible for providing accurate and complete information about your present
complaints, past medical history, and other matters pertaining to your health.

2. You are responsible for reporting any changes in your condition.

3. You are responsible for asking questions if you do not understand directions or
procedures.

4. You are responsible for following the treatment plan prescribed by your physician,
including the instructions of nurses and other health care providers.

5. You are responsible for keeping appointments and notifying the facility and physician if
you are unable to do so.

6. You are responsible for your actions if you choose to refuse treatment or not follow the
physician's orders.

7. You are responsible for following the facility’s policies and procedures.

8. You are responsible for being considerate of the rights of other patients, their family
members, and facility personnel.

9. You are responsible for respecting your own personal property and that of others in the
facility.
