Read the case notes and complete the writing task which follows.

Notes:

Assume that today's date is 8 January 2020. You are a nurse in the Stroke Unit of Central Hospital,
responsible for the care of Lena Popovich who was admitted 3 weeks ago after a stroke. She is now
ready to be discharged and you will refer her to Ms Linda Walter, Nurse Manager at Community
Rehabilitation Centre, for post-stroke care.

You have had a brief phone conversation today with Ms Walter..

Discharge summary

Name: Mrs Lena Popovich

DOB: 14 February 1958 (59 y.o.)

Address: 55 Treeview Street, Milestone

Admission: 25 December 2019

Diagnosis: Left-sided stroke

Social background: Retired teacher, married. Likes gardening & painting. Married son lives nearby, visits

regularly. Pt expecting first grandchild in 6 weeks.

Medication:

 Perindopril arginine/indapamide (Coversyl plus) 4mg/1.25mg daily


 Aspirin 100mg daily
 Temazepam (Temazepam-Apo) 10mg nocte
 Escitalopram oxalate (Loxalate) 5mg daily
 Metformin hydrochloride (Sandoz) 500mg 2x/day

Reason for admission:

Patient delivered to hospital by husband: 25 Dec 2021: R upper and lower limbs

sudden weakness, slurred speech, confusion.


Previous history:

14 January 2017 BP 160/90-hypertension diagnosed by local GP; prescribed Coversyl plus

5 August 2018 Pt reports insomnia, fatigue, feeling low, stressed since retiring in 2019

Mild depression; prescribed Temazepam-Apo 10mg nocte, escitalopram oxalate (Loxalate) 5mg daily

Current history:

25 December 2019

Presentation: Pt delivered to hospital by husband

 Signs of expressive aphasia (non-fluent speech)


 R upper and lower limb weakness, numbness
 Blood glucose level (BGL):13mmol/L (high), BP: 180/120 (high), temp: 37.3°C (normal)
 Dizziness

Treatment:

 Thrombolysis-heparin infusion, continued with antihypertensive Coversyl plus


4mg/1.25mg daily and anti-depressants: Temazepam-Apo 10mg nocte, Loxalate 5mg
daily
 Initial BGL management with sub-cutaneous insulin, commenced on metformin
(Sandoz) 500mg 2x/day
 Aspirin 100mg/day commenced
 Motorskills exercise, walking with mobility aid

8 January 2020 Persisting signs of mild expressive aphasia, insomnia, headache, mild R upper and lower

limb weakness, anxiety.

HbA1c: 5.5% (normal), BP: 100/75, temp: 36.9°C (normal)

Concerns: Unstable BGL, expressive aphasia, ↑ stress

Discharge plan: Refer to Nurse Manager of Community Rehab Centre, Linda Walter. As previously

discussed on the phone:

 Suggest physio, occupational & speech therapy


 Mobility training-encourage to use walker
 Emphasise risk of depression. Consider psychological counselling
 Ensure BGL monitored and investigate if needed
Writing Task:

Using the information given in the case notes, write a letter of referral to Ms Walter, Nurse Manager of

Community Rehab Centre. In your letter, briefly outline Ms Popovich's history and your concerns.
Address the letter to Ms Linda Walter, Nurse Manager, Community Rehab Centre, Greenfield.
