    Medical Act I & III(b) 1949.

    This is to inform that MoHFW Employee Id .7645 Dr.Realm Phangcho , KGMU Registration No.
    2020MS091 ,UP Medical Council Reg No. 25562/25/09/2019 is directed to renew his/her
    mentioned Practice License Bearing No. 25562 Under- Uttar Pradesh Medical Board ,India,as it
    has been detected expired on 16/10/2024.Hereby, he/she is not allowed to perform / represent
    any un authorized activities/operation/clinical duties on exemption.

    Therefore, the said employee will also be forwarded to the Director General of Health
    Services,New Delhi. The employee is thereby directed to pay the renew Challan fee of
    ₹6,423.00/-( in words Six Thousand Four Hundred Twenty Three Rupees only) in immediate to
    extend his/her permission/entitled to perform /represent his/her fellowship duties.
