accidents Making safety manuals Preparation of risk assessment, method statements and
frequency and severity rates. Issue work permits Preparation of daily reports
Preparation of weekly, monthly and annual reports. Maintaining the safety records
Submitting proposal Arrange special trainings for employees like first aid, fire fighting,
rigger, scaffolding etc.

2-

HAZARD
A hazard is something with the potential to cause harm

RISK
Risk is the likely hood that the harm from one or more particular
hazards.

CONTROL
For avoid accidents we have to identify all hazards, asses risks
and take good control measures.

3-
ACCIDENT
An unplanned, unwanted sequence of events which result in
some kind of loss.
INCIDENT
An unplanned, unwanted sequence of events which had the
potential to result in some kind of loss

4-House keeping is an orderly arrangement of tools, equipment ,materials etc

and good clean and tidy.it is a practical method of increasing production and
reducing accidents.
5-
 Permit to work system is a formal written system used to control
certain types of work that are potentially hazardous.
 A permit to work is a document which specifies the work to be done
and the precautions to be taken.
 These may include isolation of supply systems and the fitting of
locking devices to control.
 Most permits are only designed to cover for 12hrs or 24 hrs work and
require an authorization signature for any time extension.
 Permit should be signed by an authorized person

PTW SYSTEM ARE FOR THE FOLLOWING WORKS.

 Hot works like welding, cutting grinding etc

 Works on electrical systems
 Maintenance on machinery
 Confined space entry
 Excavation works
 Work on operational pipelines

6-
 A risk assessment is a careful examination of what in your work could
cause harm to people, so that you can weigh up whether you have
taken enough precautions or should do more to prevent harm.
 The aim is to make sure that no one gets hurt or becomes ill.

STEPS TO RISK ASSESSMENT

 Identify the hazards
 Identify those at risk
 Evaluate the risk
 Examine existing work place precautions
 Decide or implement any new work place precautions
 Record assessment
 Inform
 Monitor/review
7-tool box talk is a small talk to workers about the works, hazards in that work or near,
risk from that hazard and controll measures. it is required to give every day. and need to
give some informations like fire and extinguishers, safe use of electrical, mechanical
equipments, safe works in confined spaces, use of ppe, etc. normally foremen are giving
the tool box talk. Frequently safety officer have to give ideal tool box talk.

8- All construction and other industries are very dangerous places. there should be
followed some safety disciplines. every body must to know all safety rules and
regulations before entering the site. safety personnels are giving this talk. this talk is
called site safety induction or orientation.

9-It is used to measure the safety perfoments of an industry.

Frequency rate = total no of reportable LTIx 1000000

--------------------------------------------
total no of man hours worked

severity rate = total no of man days losted due to reportable LTIx 1000000
------------------------------------------------------------------------
total no of man hours worked

10- fire is a chemical reaction in which a combustible material combines with oxygen in
the atmosphere to give out heat and flame.
If we remove any thing from fire triangle(fuel
or oxygen or heat) the fire will be put off.

11.emergency plan is an action plan for the proper response during any major emergency
situations like fire, chemical or gas release or oxygen depletion etc.it should be know all
responsible personnels.

12-it is a designated area to accommodate workers safely during emergency situations. all
emergency exits (escape routes) will leed to to the assembly point.

14-noise is defined as an unwanted sound.

15- conduction, convection and radiation.

29- Safety measures during Gas (oxy-acetylene) welding and

cutting.
1- welder should be competitive person.
2- All flammable materials should be removed before start the work.
3- Should keep the fire extinguisher near the work .
4- All floor and wall openings should be covered.
5- Before starting gas welding or cutting all hoses caring acetylene and oxygen gas
shall be checked thoroughly for any defects. Defective greasy or oil soaked hoses
shall not be used.
6- Use only appropriate tools to ignite torches do not use open flames or matches.
7- Both the oxygen and fuel gas lines must be equipped with flash back preventers
(reverse flow check valves or non return valves) at there connection to the torch.
8- Cylinder valves shall be closed when work is completed or stopped.
9- No welding job shall be carried out unless regulator and pressure gauge are
provided.
10- Cylinder valves shall be closed completely before transporting from one place to
another.
11- Pressure higher than recommended by the supplier or maker shall never be
applied for operating blow pipe.
12- Care shall be taken not to allow the pressure in the oxygen cylinder to fall to a
point that may permit the acetylene or other combustible gas to flow back in to
oxygen cylinder and vice versa.
13- Smoking by work man or welders is strictly prohibited while they are handling
gas cylinders.

Safety precautions during electric arc welding and cutting operations.

1. All floor and wall openings with non combustible material.

2. Approved only authorized persons should perform welding and cutting
operations. Before starting a job check whether a cutting and welding
permit is required.
3. Inspect all welding and cutting equipment before use.
4. Remove all flammable materials from there.
5. keep fire extinguishers there
6. Cover eye protection must be worn.
7. All cables and connections are in sound condition.
8. Frames of electric welding machines shall be effectively grounded.

16-the safety audit is the process that identifies unsafe conditions and unsafe acts in the
plant and recommends safety improvement. Safety audit is a very effective tool with
safety management for identifying weak spots and planning corrective actions.

17- a confined space space is a work place of an enclosed nature where there is a risk of
death or serious injury from hazardous substances or dangerous conditions.eg.oil and gas
separators,silos,boilers,vessels etc.
18-
PPE (PERSONAL PROTECTIVE EQUIPMENTS)

 It is a last line protection.

PPE devices are designed to interpose an effective barrier between a person

and harmful objects, substances or radiations etc.

TYPES OF PPE
 Hearing protection (ear plug, ear muff etc)
 Respiratory protection (dust mask, breathing apparatus etc)
 Eye protection (safety goggles, welding shield etc)
 Skin protection (cover all, hand gloves etc)
 Safety belts and safety harnesses.
Helmet and safety shoes

19- check list is a written format for ensure the safety measures of that work
or equipment .eg. Crane check list, excavation check list, scaffolding check
list etc.

20-After any accident a person being absent from work for more than 3 days
(excluding day of accident/injury) it is called LTI. This LTI required report
to labor ministry.

22-Danger, warning, mandatory, safe conditions and fire fighting signals

23-After a major accident took place call police and ambulance immediately.
Secure the accidental place without any disturbance up to complete legal
enquiry. Inform labor ministry through phone or fax. Do a detailed accident
investigation and prepare accident report. Submit the reports. Convey the
messages of the accident to workers for avoid reoccurrence of same
accident.
24-Financial, morale and law

25-MSDS (MATERIAL SAFETY DATA SHEAT) is a detailed catalogue of

chemicals. In MSDS clearly mention all qualities of chemicals. This is very
important during the handling of chemicals.
