IPSG No.1: Identify Patient Correctly

Identification Band

Identification Band (ID Band) is a water-resistant band containing the


patient’s identification information, such as name, health record number, etc.
Policy on Patient Identification

POLICY:
• All patients shall be identified using two patient identifiers, not including the use of the patient’s
room number or location.
• Patients shall be identified before administering medications, blood, or blood products.
• Patients shall be identified before taking blood and other specimens for clinical testing.
• Patients shall be identified before providing treatments and procedures.
• Al-Khafji National Hospital shall initiate and maintain a medical record for every patient assessed or
treated, such that:
• Sufficient information to identify the patient, support the diagnosis, justify the treatment,
document the course and results of treatment, and promote continuity of care among health care
providers is included.
• The medical record of every patient receiving emergency care shall include the time of arrival,
the conclusions at termination of treatment, the patient’s condition at discharge, and follow-
up care instructions;
• As part of its performance improvement activities, Al-Khafji National Hospital regularly assesses
patient health record content and the completeness of patient clinical information.
• Identification of newborn infants born in Al-Khafji National Hospital shall be in compliance with
Ministry of Health (MOH).
Procedures

• All patients are provided with a unique medical record MR number that is automatically
generated by the hospital Information System when a patient registers.
• An ID band is affixed to all inpatients, same-day (short-stay) patients, and Emergency
Room patients.
• The following information should be included on the identification band:
• Patient’s name
• Health record number
• Date of Birth DOB
• Gender M or F
• If the patient’s medical condition prohibits the application of the ID band to the
patient’s extremity (wrist or ankle) e.g., in cases of burns, the ID band must be attached
to a visible part of the patient’s body appropriate to the patient’s condition and
allergies.
• The patient’s full name must be used. Shortened versions are not acceptable.
Procedures

 If the patient’s medical condition prohibits the application


of the ID band to the patient’s extremity (wrist or ankle)
e.g., in cases of burns, the ID band must be attached to a
visible part of the patient’s body appropriate to the
patient’s condition and allergies.
 The patient’s full name must be used. Shortened
versions are not acceptable.
 Staff must confirm the patient’s identity prior to
performing any procedure:
Same name and similar name patients:

 Being alert to the potential risk of misidentification when


patients have similar names.
 Taking responsibility for activating the alert notice through
the unit nursing staff.
 Paying close attention to the patient’s medical record
number in communication, care, and treatment, when the
patient ID Alert sign is placed on the front of a patient’s
medical record.
 Communicating to colleagues during shift change or patient
handover.
Mothers and newborns are accurately identified

• Each mother and newborn infant will be accurately identified


immediately after birth while the mother and infant are still in
the birth room. These procedures include a dual identity bracelet
system
• Al-Khafji National Hospital Newborn Identification will be used for
documentation and completed before transfer from the birth
area.
Dual Identification System

• A dual identification bracelet system will be utilized and will remain in place
until the discharge of both mother and infant.
• The mother will receive instructions on infant security and the importance of
checking her identification bracelet with her infant’s identification bracelet
at every interaction.
• Two bracelets with preprinted identical identification numbers are prepared.
• The following information is recorded by the nurse using an indelible pen to
prevent smudging or loss of text due to water immersion when bathing:
• Baby (gender) of (mother’s full name),
• Mother’s health record number,
• Date and time of birth.
• One bracelet is placed on the wrist of the mother. This should be the same
wrist that holds the mother’s inpatient identification band.
• The other bracelet is placed on the right ankle of the newborn infant.
