Yesim Isil Ulman
YESIM ISIL ULMAN: Professor of medical history and ethics at Acibadem University School of Medicine (Istanbul Turkey). She has studied humanities, political science, history of medicine and ethics. The subjects of her MA, MSc and PhD on the history of modernization in science and medicine (1986-1999). She became Associate Professor in 2005 and has been Full Professor since 2013.
She chaired and contributed to the studies and works of the Turkish Bioethics Association (2006-2012); coordinated in the Training Programme of Health Law Certificate for Acibadem Univ. Continued Education (ASEGEM) (2010- 2014) in collaborative spirit. She served as the vice-chair of the University Research Ethics Committee (2009-2014), member of Hospital Ethics Committee (2009-present), Animal Experiments Ethics Committee (2012-present). She is at the editorial board of Acibadem Univ. Journal of Health Sciences. She was invited to establish the Turkey Working Group of the Cambridge Consortium of Bioethics Education (2013) in collaboration with multidisciplinary team. Dr. Ulman is an internationally certified trainer of Moral Case Deliberation, an innovative and dialogical approach to teaching ethics and ethics consultation developed by the Amsterdam UMC Medical Humanities. She has contributed to Horizon VIRT2UE Research Integrity Project as a stakeholder and co-trainer. She has initiated Acibadem University Bioethics Master of Science Programme, and the acting director of the Acibadem University Institute of Social Sciences. She serves as referee for EU Cost Projects and reporter to the Council of Europe Bioethics Committee (DH-BIO). She is the delegate of Turkey at the International Society for the History of Medicine.
The main interests of her studies are the history of modernization in medicine; teaching ethics and medical humanities; main issues of bioethics; ethical aspects of emerging technologies, research and publication ethics, animal experiments; gender issues, health law. She has collaborated numerous international and local research projects on vulnerable groups, gender-based violence; main issues of bioethics, emerging technologies and teaching bioethics. Her latest research projects are on the ethical aspects of Artificial Intelligence in medicine and healthcare and End of Life Decision making and Living Wills.

İstanbul, dünya uygarlık tarihinin en önemli kentlerinden birisidir. Yakın zamanda yapılan kazılarda ortaya çıkan bulgulara göre, İstanbul’daki ilk yerleşimler 8.000 yıl öncesine dayanıyor. İlk sakinlerinden günümüze kadar İstanbul eşsiz nitelikleriyle bir çekim merkezi olmaya devam etmiş ve bu kadim kent 1.600 yıl boyunca kesintisiz biçimde başkentlik yapmıştır. Hem Bizans Döneminde hem Osmanlı Döneminde siyasi, ekonomik ve kültürel bir başkent olarak İstanbul’da çok farklı kültürler, diller, dinler ve topluluklar birlikte ve birbirleriyle etkileşerek yaşamıştır.
İstanbul’un tıp tarihi açısından da mirası oldukça zengindir: Orta Asya ve Şaman halk geleneklerinden, Helenistik Tıp’a, Galen Tıbbından İslam Tıbbına, Bizans Tıbbından Osmanlı Tıbbına kadar geniş bir yelpaze. İstanbul tıp pratikleri ve teorileri açısından çok farklı unsurların birleştiği; yorumlanıp harmanlandığı kozmopolit bir kenttir. Farklı uygarlıkların tıp anlayışlarının sentezlendiği İstanbul, Osmanlı Dönemi’nde de tıp açısından “kültürlerüstü” bir niteliğe sahip olmaya devam etmiştir.
Batı dünyasındaki hastane geleneğinin kökeni Bizans İmparatorluğu’na dayanır. İstanbul, ilk Hristiyan hastanelerinin kurulduğu kentlerden birisidir. Tarihi 4. yüzyıla dayanan Bizans hastaneleri zaman içinde gelişerek St. Sampson ve Pantakrator Ksenonlarıyla modern hastane kavramının habercileri olmuştur. Kaynaklar, 6. yüzyılda İstanbul’da 35 hastanenin varlığına işaret ediyor. İstanbul’da bugünkü Zeyrek Camii’nin bulunduğu bölgede yer alan Pantokrator, modern hastane fikrinin erken ve yetkin örneklerinden biridir. Burada her gün vizit ve klinik muayeneler yapılır ve gerektiği zaman hasta muayenesi için evlere gidilirdi. İstanbul’un Bizans geçmişinde hekimler, eczacılar ve diğer şifacılar önemli bir yere sahipti ve kentin farklı yerlerinde hastalıkları tedavi etmek, bedeni ve ruhu kötülüklerden korumak için çeşitli mekanlar inşa edilmişti.
İstanbul’u ele alan Osmanlılar, bir yandan Bizans tıp kurumlarını, şifa yerlerini devam ettirmeye çalışırken, diğer yandan Selçuklu geleneğinin devamı şeklinde düşünülebilecek darüşşifalar inşa etmeye başlamıştır. Fatih, Süleymaniye, Sultanahmet, Haseki ve Valide Atik Darüşşifaları şehrin önemli hastanelerindendir. İstanbul’un kozmopolit bir kent olduğunu anlamak için Osmanlı döneminde inşa edilen ve açılan hastanelere bakmak yeterlidir: askeri hastaneler, farklı etnik ve dini topluluklara ait hastaneler, belediye hastaneleri, yabancı ülkelerin misyon hastaneleri vb.
18. yüzyıl sonu ve 19. yüzyıl başlarında tıp alanındaki gelişmeler, Osmanlı Devleti’nde ve onun gözbebeği olan payitahtı İstanbul’da da bir yenileşme ve yeni çağın ihtiyaçlarına uygun yeni kurumlar oluşturma gereğini ortaya çıkarmıştır. 14 Mart 1827 günü, ilk modern tıp okulu olan Tıphane-i Amire açılmış, 1839 yılında ise Tıphane ile Cerrahhane birleştirilerek bugünkü Galatasaray Lisesi’nin bulunduğu Enderun Mektebi, yeni tıp okuluna, tam adıyla Mekteb-i Tıbbiye-i Adliye-i Şâhâne’ye mekân olmuştur . Yirminci yüzyıl başında ise Tıbbiye, Haydarpaşa’da inşa edilen modern binaya taşınmıştır. Ardından yeniden Avrupa yakasına ve Çapa’ya yerleşen kurumdan Cerrahpaşa Tıp Fakültesi doğmuş, günümüz İstanbul’unda ise tıp fakültelerinin sayısı 10’u geçmiştir.
İstanbul, Cumhuriyet döneminde açılan yeni hastaneler, tıp fakülteleri ve diğer sağlık kurumları ile her açıdan tıptaki gelişmelerin, yeni yöntem ve uygulamaların başkenti olmaya devam etmiştir. Günümüzde de İstanbul, yalnızca bu şehirde yaşayanlara değil; bütün bir ülkeye ve giderek farklı ülkelerden, coğrafyalardan gelenlere şifa dağıtmaktadır.
İstanbul Tıp Tarihi Sergisi başlıklı bu kitap, Acıbadem Üniversitesi Tıp Fakültesi I. sınıf öğrencilerinin, 2013 ve 2014 yılı 14 Mart Tıp Bayramı’nda sergiledikleri posterler ve metinlerden oluşmaktadır. Geleceğin hekimleri, bu araştırmalarını, Tıp Tarihi dersi kapsamında gerçekleştirdiler. İstanbul’un farklı yerlerinde, bugün de ayakta olan sağlık kuruluşlarını ziyaret ederek haklarında bilgi edindiler. Elbette bu kitapta yer alan posterlere konu alan sağlık kurumları, İstanbul’un tıp tarihi bakımından geniş birikiminin yalnızca bir kısmını oluşturuyor.
Tıp Tarihi açısından son derece zengin bu kentte, öğrencilerin tarihi öneme sahip sağlık kuruluşlarını ziyaret etmelerinin pek çok açıdan yarar sağladığını düşünüyoruz . Öğrencilerin kendi seçtikleri mekânları ziyaret ettikleri; gözlemlerini, izlenimlerini, araştırarak edindikleri bilgileri kendilerinin raporladıkları bir saha çalışması; yani Türkiye’de bir Tıp Fakültesi öğrencilerinin aynı gün ve aynı saatlerde şehrin çok farklı yerlerindeki kurumları birebir ziyaret ederek, sonuçlarını kaleme aldıkları bir araştırma ilk defa gerçekleşiyor. Öğrencilerimiz bu raporlarını birer postere çevirerek, 14 Mart Tıp Bayramı onuruna sergilediler. Bu uygulama yalnızca biz eğitimcileri değil, öğrencilerimizi karşılayan kurumların yetkililerini de oldukça heyecanlandırdı. Bazı kurumlar, ilk defa bir tıp fakültesinden öğrenci grubu ağırlamanın heyecanını; bazı kurumlar ise bizzat öğrenciler tarafından seçilip ziyaret edilmelerinin gururunu bizimle paylaştılar. Bu ziyaretlerin bir açıdan tarihsel bir kucaklaşma olduğunu gözledik.. İstanbul’un çok farklı kültürlerine, topluluklarına ait kurumlarının, bütün zenginliğiyle yeni kuşaklara, geleceğin hekimlerine kucak açması hepimiz için son derece anlamlıdır. Haliç kıyısında bir Musevi hastanesinin, sur dışında Rum ve Ermeni Hastanelerinin, çocuklar için, yaşlılar için, lepralılar için, psikiyatrik hastalar, ortopedik hastalar ve daha pek çok branş için kurumsallaşmış tarihi hastanelerin veya kentin bir sokağındaki tarihi bir eczanenin varlığını öğrenmek, bizi yaşadığımız kent hakkında düşündürdüğü kadar; tıbbın farklı kültür, inanç, din ve milletlerden insanları birleştiren; ortak ve evrensel değerlerini de hissetmemize olanak sağlar.
Ülkemizde tıp tarihi eğitim ve öğretimine bir seçenek olarak sunulan bu çalışma, herşeyden çok Üniversitemiz eğitim felsefesinin parçasıdır. Bu nedenle öncelikle bize bu olanağı sağlayan Acıbadem Üniversitesitesi’nin değerli Yöneticilerine; saha çalışmasının ortak fikir sahipleri Prof. Dr. Nadi Bakırcı ve Doç. Dr. Pınar Topsever başta olmak üzere, Üniversitemizde tıp eğitiminin planlanmasına sonsuz emeği geçen çalışma arkadaşlarımıza, sevgili öğrencilerimize, sağlık kurumlarında bizlere destek veren kıymetli sağlık çalışanlarına ve idarecilere çok teşekkür ediyoruz.

Introducing the Council of Europe’s “Guide on the decision-making process regarding medical treatment in end-of-life situations”

AIMS AND SCOPE OF THE MEETING
The aims of this symposium are to introduce the printed Turkish version of the Guide to the allied academic, scientific, public circles, and to create an internationally collaborated scientific platform for discussing end-of-life situations from medical, clinical, bioethical, judicial, legal perspectives on one hand and for having a deeper insight into the issue through recent studies carried out in Turkey mainly based on the Council of Europe’s afore-mentioned Guide.
The Guide on the decision-making process regarding medical treatment in end-of-life situations was drawn up by the Committee on Bioethics (DH‑BIO) of the Council of Europe in the course of its work on patients rights and with the intention of facilitating the implementation of the principles enshrined in the Convention on Human Rights and Biomedicine (Oviedo Convention, ETS No. 164, 1997).
The purpose of this guide is to serve as a useful tool for informing the public and training professionals. It is aimed at health professionals, patients, their families and all those who face problematic decisions with regard to medical treatment in end-of-life situations, and provides help for the development of practices. The guide is also a source of material for any discussion held within our societies on the decision-making process regarding medical treatment in end-of-life situations in that it proposes benchmarks relating both to the practices and the principles that can be applied in this context (Council of Europe).

Alongside the need for an understanding of what to do to combat the pandemic, there should co-exist enquiry into how such action should be undertaken: to put it another way, what attitudes and behaviours should come into play. There is a need to investigate the ethical dimension of the problem. In combatting the pandemic, an evidence-based scientific and medical approach should continue, whilst from the bioethical perspective, there should be openness, transparency and realism in the way that developments are shared with the public. Everyone should be treated equally. In situations calling for quarantine and isolation, it is necessary that any sacrifice of personal autonomy be proportionate to the requirements of public health and call for a reasonable level of self-sacrifice; that individuals, hospitals, organisations and countries will act in solidarity and support each other co-operatively; that governments be accountable for the response they give to the requirements of their people and that economic life be supported with reciprocal collaboration and solidarity. The response should preserve human rights and dignity. It is recommended that the pandemic be managed in a way that is humane and pluralist, whilst maintaining attitudes and behaviours that are respectful and responsible towards the environment, the biosphere, biodiversity and future generations.

COVID-19 Enfeksiyon Hastalığı Salgınına Biyoetik Açıdan Bakış

GİRİŞ
COVID-19 pandemisiyle, kanıta dayalı bilimsel yöntemlere dayalı, acil önlemler alınarak, salgını kontrol etmek üzere hızla karşılık vermek gerekir.
Hastalıkla mücadelede bilimsel, tıbbi, hukuki, ekonomik, sosyolojik açılardan ne yapmak gerektiğini anlamanın yanısıra bu mücadelenin “nasıl” yapılacağının, hangi tutum ve davranışlar gözetilerek verileceğinin, bir başka deyişle meselenin etik boyutunun sorgulanmasına da ihtiyaç vardır.
Salgının yarattığı sorunlarla mücadelede, etik olarak kabul edilebilir çözümler ararken, insan hakları ve insan onuru temeline dayanan (2) etik ilkeler yol göstericidir.
HALKIN SAĞLIĞI, TOPLUMUN YARARI, ŞEFFAFLIK, HESAP VERİLEBİLİRLİK, AKILCILIK, GÜVEN:
Salgın gibi olağandışı dönemlerde, tek tek bireylerin sağlığından çok, tüm toplumun sağlığını; nüfusun sağlığını koruma, geliştirme, hastalıkları, sakatlığı önlenmeyi vurgulayan halk sağlığı yaklaşımı öne çıkar.
Epidemiyolojik bulgular, nüfus verileri, nicel hesaplamalar ve amprik risk değerlendirmeleriyle, sağlığın belirleyenlerini dikkate alarak salgının gidişatı üzerine projeksiyon modelleri geliştiren halk sağlığı yaklaşımından yararlanarak; bilimsel bir veritabanına dayanan stratejiler (3) ile salgının gidişatı yönetilmelidir.
Salgın yönetiminde, insan ve toplum sağlığı yararına tasarlanmış, çok disiplinli,tüm tarafların eşitlikle temsil edildiği karar süreçleriyle çalışan, çoğulcu, şeffaf, hesap verilebilir, adil kamu politikaları ve stratejileri üreten; akılcı muhakeme ile gecikmeden etkili yanıt verebilen, kamuoyunda güven yaratan işleyişe ihtiyaç vardır.
AYRIMCILIK YAPMAMA:
Dezavantajlı grupların korunmasında, sözel ya da fiziksel hiç bir şekilde ayrımcılık ve damgalamaya uğramamaları için ortak sorumlulukla, adaletle davranılmalıdır (4).
ADALET, EŞİTLİK, HAKKANİYET
Sağlık sisteminde ağır talep yoğunluğu yaşanacağı, sınırlı kaynakların adil biçimde dağıtımında zorluklar çıkabileceği hallerde, herkesin, insan hakları hukuku ve meslek etiği ilkeleri doğrultusunda, eşitlikle ve hakkaniyetle, bireysel ve toplumsal bir hak olarak sağlık hizmetine erişiminin sağlanması önemlidir. Etiğin adalet ve eşitlik ilkeleri ışığında, halkın sağlığını ve güvenliğini koruma sorumluluğunu yerine getirmek, bunu güvence altına almak üzere tedbirler geliştirmek, sağlık sistemine destek sağlayan politikalar uygulamak hükümetlerin görevidir.
Bireyler de, karşılıklılık prensibi gereği (5), bulaşıcı hastalığının yayılmasını önlemek üzere, el hijyenini sağlayarak, toplumsal hareketliliklerini kısıtlayarak, sokağa çıkmayarak, diğer insanlarla aralarında sosyal mesafeyi koruyarak, sosyal izolasyon, karantina kurallarını uygulayarak üzerlerine düşen görevi sorumlulukla yerine getirmelidirler (6). Bu kuralların herkese eşitlikle, şeffaf, açık, denetlenebilir biçimde uygulanmasının hukuki alt yapısını ve işleyişini sağlamak; yurttaşların istihdam, gelir, sosyal güvenlik kaybı gibi zararlarını telafi eden sosyal destek stratejileri geliştirmek yöneticilerin görevlerindendir (7).
SORUMLULUK, DAYANIŞMA, ÇOĞULCULUK, İNSAN HAKLARI VE ONURU
Sonuç olarak, içinde bulunduğumuz küresel sağlık krizi ile mücadelede, kanıta dayalı bilimsel-tıbbi müdahale sürerken; gelişmelerin açıklıkla ve gerçekçilikle kamuoyuyla paylaşıldığı; herkese insan onuruna ve haklarına uygun eşitlikle muamele edildiği, karantina ve izolasyon durumlarında kişi özerkliğinden, toplum sağlığı adına orantılı fedakarlık beklendiği (8); insanlar, hastaneler ve ülkeler arasında dayanışma ve işbirliğinin desteklendiği; hükümetlerin hesap verilebilirlikle halkın ihtiyaçlarına cevap verebildiği, iktisadi hayatın karşılıklı işbirliği ve dayanışma ile desteklendiği, insan haklarına ve onuruna uygun, insani, çoğulcu, etik değerleri kılavuz alan tutum ve davranışlar önerilir.

Ethics teaching is globally considered an essential part of medical education fostering professionalism. It does not only provide knowledge for good clinical conduct, but also trains medical students as virtuous practitioners. Although Turkey has had a considerable experience in ethics education of healthcare professionals, the general state of ethics curricula at medical schools in Turkey is unknown.
Methods:
The purpose of this study was to collect comprehensive data about the ethics education programs at medical schools in Turkey. To this aim, we designed a cross-sectional descriptive questionnaire survey which focuses on the content, teaching years, teaching, assessment and evaluation methodologies, workforce and infrastructure. We delivered the questionnaire to all medical schools in Turkey. Seventy-nine medical schools participated in this study (response rate: 78%).
Results:
Although most institutions had an undergraduate ethics curriculum (91.1%), the findings suggest deficiency of teaching personnel (34.2% had no instructors). Furthermore, the distribution and composition of the workforce was imbalanced. The content varies largely among institutions. Medical schools with an ethics department were more likely to diversify teaching topics. However, ethics education was largely based on the four-principle approach. The content was usually conveyed to students theoretically. Around 90% of schools had classroom lectures. It is the only method used at one-third of them. Clinical ethics education was mostly lacking. Multiple-choice tests were widely used to assess and evaluate student attainments (86.1%).
Conclusions:
Staff qualified to teach ethics and ethics education integrated into the six-year medical curriculum given by a multidisciplinary team are urgent necessities. Considering teaching, assessment and evaluation methodologies used, most medical schools seem to fall short of fostering students to develop ethical attitudes. Endeavors aiming for modern topics should be encouraged. As the organization of ethics education change continuously, we think that a platform for monitoring ethics education at medical schools in Turkey should be established. Such a body would help ethics instructors to network and find solutions to current problems and build shared wisdom.

İstanbul’un tıp tarihi açısından da mirası oldukça zengindir: Orta Asya ve Şaman halk geleneklerinden, Helenistik Tıp’a, Galen Tıbbından İslam Tıbbına, Bizans Tıbbından Osmanlı Tıbbına kadar geniş bir yelpaze. İstanbul tıp pratikleri ve teorileri açısından çok farklı unsurların birleştiği; yorumlanıp harmanlandığı kozmopolit bir kenttir. Farklı uygarlıkların tıp anlayışlarının sentezlendiği İstanbul, Osmanlı Dönemi’nde de tıp açısından “kültürlerüstü” bir niteliğe sahip olmaya devam etmiştir.
Batı dünyasındaki hastane geleneğinin kökeni Bizans İmparatorluğu’na dayanır. İstanbul, ilk Hristiyan hastanelerinin kurulduğu kentlerden birisidir. Tarihi 4. yüzyıla dayanan Bizans hastaneleri zaman içinde gelişerek St. Sampson ve Pantakrator Ksenonlarıyla modern hastane kavramının habercileri olmuştur. Kaynaklar, 6. yüzyılda İstanbul’da 35 hastanenin varlığına işaret ediyor. İstanbul’da bugünkü Zeyrek Camii’nin bulunduğu bölgede yer alan Pantokrator, modern hastane fikrinin erken ve yetkin örneklerinden biridir. Burada her gün vizit ve klinik muayeneler yapılır ve gerektiği zaman hasta muayenesi için evlere gidilirdi. İstanbul’un Bizans geçmişinde hekimler, eczacılar ve diğer şifacılar önemli bir yere sahipti ve kentin farklı yerlerinde hastalıkları tedavi etmek, bedeni ve ruhu kötülüklerden korumak için çeşitli mekanlar inşa edilmişti.
İstanbul’u ele alan Osmanlılar, bir yandan Bizans tıp kurumlarını, şifa yerlerini devam ettirmeye çalışırken, diğer yandan Selçuklu geleneğinin devamı şeklinde düşünülebilecek darüşşifalar inşa etmeye başlamıştır. Fatih, Süleymaniye, Sultanahmet, Haseki ve Valide Atik Darüşşifaları şehrin önemli hastanelerindendir. İstanbul’un kozmopolit bir kent olduğunu anlamak için Osmanlı döneminde inşa edilen ve açılan hastanelere bakmak yeterlidir: askeri hastaneler, farklı etnik ve dini topluluklara ait hastaneler, belediye hastaneleri, yabancı ülkelerin misyon hastaneleri vb.
18. yüzyıl sonu ve 19. yüzyıl başlarında tıp alanındaki gelişmeler, Osmanlı Devleti’nde ve onun gözbebeği olan payitahtı İstanbul’da da bir yenileşme ve yeni çağın ihtiyaçlarına uygun yeni kurumlar oluşturma gereğini ortaya çıkarmıştır. 14 Mart 1827 günü, ilk modern tıp okulu olan Tıphane-i Amire açılmış, 1839 yılında ise Tıphane ile Cerrahhane birleştirilerek bugünkü Galatasaray Lisesi’nin bulunduğu Enderun Mektebi, yeni tıp okuluna, tam adıyla Mekteb-i Tıbbiye-i Adliye-i Şâhâne’ye mekân olmuştur . Yirminci yüzyıl başında ise Tıbbiye, Haydarpaşa’da inşa edilen modern binaya taşınmıştır. Ardından yeniden Avrupa yakasına ve Çapa’ya yerleşen kurumdan Cerrahpaşa Tıp Fakültesi doğmuş, günümüz İstanbul’unda ise tıp fakültelerinin sayısı 10’u geçmiştir.
İstanbul, Cumhuriyet döneminde açılan yeni hastaneler, tıp fakülteleri ve diğer sağlık kurumları ile her açıdan tıptaki gelişmelerin, yeni yöntem ve uygulamaların başkenti olmaya devam etmiştir. Günümüzde de İstanbul, yalnızca bu şehirde yaşayanlara değil; bütün bir ülkeye ve giderek farklı ülkelerden, coğrafyalardan gelenlere şifa dağıtmaktadır.
İstanbul Tıp Tarihi Sergisi başlıklı bu kitap, Acıbadem Üniversitesi Tıp Fakültesi I. sınıf öğrencilerinin, 2013 ve 2014 yılı 14 Mart Tıp Bayramı’nda sergiledikleri posterler ve metinlerden oluşmaktadır. Geleceğin hekimleri, bu araştırmalarını, Tıp Tarihi dersi kapsamında gerçekleştirdiler. İstanbul’un farklı yerlerinde, bugün de ayakta olan sağlık kuruluşlarını ziyaret ederek haklarında bilgi edindiler. Elbette bu kitapta yer alan posterlere konu alan sağlık kurumları, İstanbul’un tıp tarihi bakımından geniş birikiminin yalnızca bir kısmını oluşturuyor.
Tıp Tarihi açısından son derece zengin bu kentte, öğrencilerin tarihi öneme sahip sağlık kuruluşlarını ziyaret etmelerinin pek çok açıdan yarar sağladığını düşünüyoruz . Öğrencilerin kendi seçtikleri mekânları ziyaret ettikleri; gözlemlerini, izlenimlerini, araştırarak edindikleri bilgileri kendilerinin raporladıkları bir saha çalışması; yani Türkiye’de bir Tıp Fakültesi öğrencilerinin aynı gün ve aynı saatlerde şehrin çok farklı yerlerindeki kurumları birebir ziyaret ederek, sonuçlarını kaleme aldıkları bir araştırma ilk defa gerçekleşiyor. Öğrencilerimiz bu raporlarını birer postere çevirerek, 14 Mart Tıp Bayramı onuruna sergilediler. Bu uygulama yalnızca biz eğitimcileri değil, öğrencilerimizi karşılayan kurumların yetkililerini de oldukça heyecanlandırdı. Bazı kurumlar, ilk defa bir tıp fakültesinden öğrenci grubu ağırlamanın heyecanını; bazı kurumlar ise bizzat öğrenciler tarafından seçilip ziyaret edilmelerinin gururunu bizimle paylaştılar. Bu ziyaretlerin bir açıdan tarihsel bir kucaklaşma olduğunu gözledik.. İstanbul’un çok farklı kültürlerine, topluluklarına ait kurumlarının, bütün zenginliğiyle yeni kuşaklara, geleceğin hekimlerine kucak açması hepimiz için son derece anlamlıdır. Haliç kıyısında bir Musevi hastanesinin, sur dışında Rum ve Ermeni Hastanelerinin, çocuklar için, yaşlılar için, lepralılar için, psikiyatrik hastalar, ortopedik hastalar ve daha pek çok branş için kurumsallaşmış tarihi hastanelerin veya kentin bir sokağındaki tarihi bir eczanenin varlığını öğrenmek, bizi yaşadığımız kent hakkında düşündürdüğü kadar; tıbbın farklı kültür, inanç, din ve milletlerden insanları birleştiren; ortak ve evrensel değerlerini de hissetmemize olanak sağlar.
Ülkemizde tıp tarihi eğitim ve öğretimine bir seçenek olarak sunulan bu çalışma, herşeyden çok Üniversitemiz eğitim felsefesinin parçasıdır. Bu nedenle öncelikle bize bu olanağı sağlayan Acıbadem Üniversitesitesi’nin değerli Yöneticilerine; saha çalışmasının ortak fikir sahipleri Prof. Dr. Nadi Bakırcı ve Doç. Dr. Pınar Topsever başta olmak üzere, Üniversitemizde tıp eğitiminin planlanmasına sonsuz emeği geçen çalışma arkadaşlarımıza, sevgili öğrencilerimize, sağlık kurumlarında bizlere destek veren kıymetli sağlık çalışanlarına ve idarecilere çok teşekkür ediyoruz.

Introducing the Council of Europe’s “Guide on the decision-making process regarding medical treatment in end-of-life situations”

AIMS AND SCOPE OF THE MEETING
The aims of this symposium are to introduce the printed Turkish version of the Guide to the allied academic, scientific, public circles, and to create an internationally collaborated scientific platform for discussing end-of-life situations from medical, clinical, bioethical, judicial, legal perspectives on one hand and for having a deeper insight into the issue through recent studies carried out in Turkey mainly based on the Council of Europe’s afore-mentioned Guide.
The Guide on the decision-making process regarding medical treatment in end-of-life situations was drawn up by the Committee on Bioethics (DH‑BIO) of the Council of Europe in the course of its work on patients rights and with the intention of facilitating the implementation of the principles enshrined in the Convention on Human Rights and Biomedicine (Oviedo Convention, ETS No. 164, 1997).
The purpose of this guide is to serve as a useful tool for informing the public and training professionals. It is aimed at health professionals, patients, their families and all those who face problematic decisions with regard to medical treatment in end-of-life situations, and provides help for the development of practices. The guide is also a source of material for any discussion held within our societies on the decision-making process regarding medical treatment in end-of-life situations in that it proposes benchmarks relating both to the practices and the principles that can be applied in this context (Council of Europe).

Alongside the need for an understanding of what to do to combat the pandemic, there should co-exist enquiry into how such action should be undertaken: to put it another way, what attitudes and behaviours should come into play. There is a need to investigate the ethical dimension of the problem. In combatting the pandemic, an evidence-based scientific and medical approach should continue, whilst from the bioethical perspective, there should be openness, transparency and realism in the way that developments are shared with the public. Everyone should be treated equally. In situations calling for quarantine and isolation, it is necessary that any sacrifice of personal autonomy be proportionate to the requirements of public health and call for a reasonable level of self-sacrifice; that individuals, hospitals, organisations and countries will act in solidarity and support each other co-operatively; that governments be accountable for the response they give to the requirements of their people and that economic life be supported with reciprocal collaboration and solidarity. The response should preserve human rights and dignity. It is recommended that the pandemic be managed in a way that is humane and pluralist, whilst maintaining attitudes and behaviours that are respectful and responsible towards the environment, the biosphere, biodiversity and future generations.

COVID-19 Enfeksiyon Hastalığı Salgınına Biyoetik Açıdan Bakış

GİRİŞ
COVID-19 pandemisiyle, kanıta dayalı bilimsel yöntemlere dayalı, acil önlemler alınarak, salgını kontrol etmek üzere hızla karşılık vermek gerekir.
Hastalıkla mücadelede bilimsel, tıbbi, hukuki, ekonomik, sosyolojik açılardan ne yapmak gerektiğini anlamanın yanısıra bu mücadelenin “nasıl” yapılacağının, hangi tutum ve davranışlar gözetilerek verileceğinin, bir başka deyişle meselenin etik boyutunun sorgulanmasına da ihtiyaç vardır.
Salgının yarattığı sorunlarla mücadelede, etik olarak kabul edilebilir çözümler ararken, insan hakları ve insan onuru temeline dayanan (2) etik ilkeler yol göstericidir.
HALKIN SAĞLIĞI, TOPLUMUN YARARI, ŞEFFAFLIK, HESAP VERİLEBİLİRLİK, AKILCILIK, GÜVEN:
Salgın gibi olağandışı dönemlerde, tek tek bireylerin sağlığından çok, tüm toplumun sağlığını; nüfusun sağlığını koruma, geliştirme, hastalıkları, sakatlığı önlenmeyi vurgulayan halk sağlığı yaklaşımı öne çıkar.
Epidemiyolojik bulgular, nüfus verileri, nicel hesaplamalar ve amprik risk değerlendirmeleriyle, sağlığın belirleyenlerini dikkate alarak salgının gidişatı üzerine projeksiyon modelleri geliştiren halk sağlığı yaklaşımından yararlanarak; bilimsel bir veritabanına dayanan stratejiler (3) ile salgının gidişatı yönetilmelidir.
Salgın yönetiminde, insan ve toplum sağlığı yararına tasarlanmış, çok disiplinli,tüm tarafların eşitlikle temsil edildiği karar süreçleriyle çalışan, çoğulcu, şeffaf, hesap verilebilir, adil kamu politikaları ve stratejileri üreten; akılcı muhakeme ile gecikmeden etkili yanıt verebilen, kamuoyunda güven yaratan işleyişe ihtiyaç vardır.
AYRIMCILIK YAPMAMA:
Dezavantajlı grupların korunmasında, sözel ya da fiziksel hiç bir şekilde ayrımcılık ve damgalamaya uğramamaları için ortak sorumlulukla, adaletle davranılmalıdır (4).
ADALET, EŞİTLİK, HAKKANİYET
Sağlık sisteminde ağır talep yoğunluğu yaşanacağı, sınırlı kaynakların adil biçimde dağıtımında zorluklar çıkabileceği hallerde, herkesin, insan hakları hukuku ve meslek etiği ilkeleri doğrultusunda, eşitlikle ve hakkaniyetle, bireysel ve toplumsal bir hak olarak sağlık hizmetine erişiminin sağlanması önemlidir. Etiğin adalet ve eşitlik ilkeleri ışığında, halkın sağlığını ve güvenliğini koruma sorumluluğunu yerine getirmek, bunu güvence altına almak üzere tedbirler geliştirmek, sağlık sistemine destek sağlayan politikalar uygulamak hükümetlerin görevidir.
Bireyler de, karşılıklılık prensibi gereği (5), bulaşıcı hastalığının yayılmasını önlemek üzere, el hijyenini sağlayarak, toplumsal hareketliliklerini kısıtlayarak, sokağa çıkmayarak, diğer insanlarla aralarında sosyal mesafeyi koruyarak, sosyal izolasyon, karantina kurallarını uygulayarak üzerlerine düşen görevi sorumlulukla yerine getirmelidirler (6). Bu kuralların herkese eşitlikle, şeffaf, açık, denetlenebilir biçimde uygulanmasının hukuki alt yapısını ve işleyişini sağlamak; yurttaşların istihdam, gelir, sosyal güvenlik kaybı gibi zararlarını telafi eden sosyal destek stratejileri geliştirmek yöneticilerin görevlerindendir (7).
SORUMLULUK, DAYANIŞMA, ÇOĞULCULUK, İNSAN HAKLARI VE ONURU
Sonuç olarak, içinde bulunduğumuz küresel sağlık krizi ile mücadelede, kanıta dayalı bilimsel-tıbbi müdahale sürerken; gelişmelerin açıklıkla ve gerçekçilikle kamuoyuyla paylaşıldığı; herkese insan onuruna ve haklarına uygun eşitlikle muamele edildiği, karantina ve izolasyon durumlarında kişi özerkliğinden, toplum sağlığı adına orantılı fedakarlık beklendiği (8); insanlar, hastaneler ve ülkeler arasında dayanışma ve işbirliğinin desteklendiği; hükümetlerin hesap verilebilirlikle halkın ihtiyaçlarına cevap verebildiği, iktisadi hayatın karşılıklı işbirliği ve dayanışma ile desteklendiği, insan haklarına ve onuruna uygun, insani, çoğulcu, etik değerleri kılavuz alan tutum ve davranışlar önerilir.

Ethics teaching is globally considered an essential part of medical education fostering professionalism. It does not only provide knowledge for good clinical conduct, but also trains medical students as virtuous practitioners. Although Turkey has had a considerable experience in ethics education of healthcare professionals, the general state of ethics curricula at medical schools in Turkey is unknown.
Methods:
The purpose of this study was to collect comprehensive data about the ethics education programs at medical schools in Turkey. To this aim, we designed a cross-sectional descriptive questionnaire survey which focuses on the content, teaching years, teaching, assessment and evaluation methodologies, workforce and infrastructure. We delivered the questionnaire to all medical schools in Turkey. Seventy-nine medical schools participated in this study (response rate: 78%).
Results:
Although most institutions had an undergraduate ethics curriculum (91.1%), the findings suggest deficiency of teaching personnel (34.2% had no instructors). Furthermore, the distribution and composition of the workforce was imbalanced. The content varies largely among institutions. Medical schools with an ethics department were more likely to diversify teaching topics. However, ethics education was largely based on the four-principle approach. The content was usually conveyed to students theoretically. Around 90% of schools had classroom lectures. It is the only method used at one-third of them. Clinical ethics education was mostly lacking. Multiple-choice tests were widely used to assess and evaluate student attainments (86.1%).
Conclusions:
Staff qualified to teach ethics and ethics education integrated into the six-year medical curriculum given by a multidisciplinary team are urgent necessities. Considering teaching, assessment and evaluation methodologies used, most medical schools seem to fall short of fostering students to develop ethical attitudes. Endeavors aiming for modern topics should be encouraged. As the organization of ethics education change continuously, we think that a platform for monitoring ethics education at medical schools in Turkey should be established. Such a body would help ethics instructors to network and find solutions to current problems and build shared wisdom.
Human trafficking, including forced prostitution, is a form of human rights violation regarding right to life and respect for human dignity. The Physician-patient relationship may serve as a process for victim identification on the basis of physicians' responsibilities to detect human trafficking. Method: This empirical study was based on a survey among physicians who may have been in contact with foreign sex workers. Almost eighty physicians in three health facilities were selected according to their high potentiality for coming into contact with foreign sex workers as patients. 228 physicians selected to that aim. 82 of them responded to the questionnaire which was performed in order to evaluate their perception and knowledge human trafficking and their attitudes towards sex work.
Results: All physicians think that some women among the sex workers are exploited, exposed to violence and coerced to work against their will (100%). Almost all of them know that it is compulsory for the healthcare professionals to inform the security forces of the women who are exploited (80.0%). Nearly half of them have given medical care to a sex worker (46.9%) and 39.0 percent have thought that it may be related to human trafficking. The ones who have informed the police of a human trafficking case are only 10 per cent.
Conclusions: We think that the professional should be equipped with ethical values encompassing both a patient's dignity and general welfare. This may include the professional's responsibility to make an assessment as to whether the patient is a victim or a sex worker. Furthermore human trafficking in sex work and sexual health are public health issues therefore it should be covered under standards of practice and ethical codes of conduct.
Etik kelimesi kullanıldığında, genellikle, ilk akla gelen sözcük ahlaktır. Ahlak ile etik kavramları, bazen birbiri yerine kullanılır, bu tamamen yanlış olmamakla ve bazı durumlarda anlaşılır olmakla birlikte, etik ve ahlak birebir aynı kavramlar değildir. Ahlak, belirli bir dönemde belli insan topluluklarınca benimsenmiş olan, bireylerin birbirleriyle ilişkilerini düzenleyen törel davranış kurallarının, yasalarının, ilkelerinin toplamıdır (1). Etik, insanın ahlaki yönünün düşünsel, felsefi analizidir. Etik, ahlaki davranışın yapısını, ahlaki deneyimlerin niteliğini, ahlakın hem evrensel özelliğini, hem de toplumdan topluma farklılıklar gösteren ve zaman içinde değişen özelliklerini inceler. Etik, ahlak konusunda, geçmiş, şu an ve geleceğe ilişkin karar ve eylemlerin dikkatli ve sistematik bir biçimde düşünülmesi ve çözümlenmesi ile uğraşır. Ahlak insanların karar ve eylemlerinin değersel boyutudur. Ahlakın dili ‘haklar’, ‘sorumluluklar’ ve ‘erdemler’ gibi isimler ve ‘iyi’ ve ‘kötü’, ‘doğru’ ve ‘yanlış’, ‘adil’ ve ‘adil olmayan’ gibi sıfatlar içerir. Bu tanımlamalara göre etik birincil olarak bilmekle ilgiliyken, ahlak yapmakla ilgilidir. Aradaki ilişki, etiğin, insanların belli bir yönde karar vermesi veya eylemesi için akla yatkın bir ölçüt sağlamaya çalışmasıyla kurulur (2). Tıp etiği, etiğin, tıbbi uygulamada karşılaşılan değer sorunlarını, değer çatışmalarını, ikilemleri inceleyerek, çözümlemeye çalışan uğraş alanıdır.
Biyoetik ise, etiğin yüzyıllardır gelişen birikimine yaslanmakla birlikte, son otuz yılda yaşam ve sağlık bilimlerinde hızla gelişmeleri, değer boyutuyla dengelemek üzere geliştirilen kapsayıcı bir disiplindir. Tüm canlılar, yani, insanlar, hayvanlar ve geniş anlamda çevre, biyosfer kapsamında ortaya çıkan değer sorunlarının, ikilemlerin çözümlemesiyle uğraşan, etiğin uygulamalı alanıdır(3). Klinik uygulamada değer çatışmaları ve ikilemlerin çözümünün yanı sıra, tıpta, biyomedikal araştırmalarda, yaşam bilimleri teknolojilerinde, ahlaki, yasal, siyasi ve sosyal meseleler biyoetiğin ilgi alanlarındandır. Biyoetiğin başlıca üç çalışma alanı vardır.
İlki, biliminsanlarının mesleki yükümlülükleri ve sorumlulukları gibi mesleki değerleri, kuramsal ve uygulamalı olarak inceleyen akademik biyoetiktir. Biyomedikal bağlamda değerli, iyi, doğru nedir, sorularının yanıtını arar ve bu kaygıları sistematik biçimde irdeleyen disiplindir. İkinci olarak, kamu politikaları ve hukuk ile biyoetiğin ilişkisini ele alır; klinik uygulamaların ve tıbbi araştırmaların yasal olarak düzenlenmesinde, yerel ve uluslararası düzlemde hukuki ve bağlayıcı sözleşme, bildirge, kodlar ve yapıların işleyişini inceler. Biyoetiğin üçüncü çalışma alanı, klinik etiktir; hasta bakımını geliştirmek, hastaya daha iyi hizmet vermek amacıyla, biyoetiğin klinik uygulamalarla ilişkisine odaklanır. Aslında bu üç temel alan birbiriyle ilişkili ve bağlantılıdır. Klinik biyoetik, biyoetikçilerle klinisyenlerin birlikte çalıştıkları, çok disiplinli bir uğraş dalıdır (4).
Yenileyici (Rejeneratif) Tıp
Tıpta yeni gelişmekte olan teknolojiler, biyoetiğin, anılan üç yönüyle birden, ilgilendiği konulardan biridir. Yeni gelişen teknolojilerinden biri olan rejeneratif (yenileyici) tıp, son yıllarda, gerek bilimsel uygulamaların niteliği gerek araştırma fonlarının tahsisi gerek bu bağlamdaki politik tartışmalar gerekse basında bolca rastlanan “mucize” tedavi haberleri ile hem bilim çevrelerinin hem de kamuoyunun ilgisini çeken konulardan olmuştur.
Yenileyici (Rejeneratif) tıp, konjenital anomaliler, hastalıklar, travmalar ve yaşlanmadan ötürü hasar görmüş hücre, doku veya organların, fonksiyonlarını yeniden kazanmak üzere, tıbben, onarma, yerine koyma, yenileme, yeniden oluşturma, yeniden üretmeye odaklanan, disiplinler arası bir araştırma ve klinik uygulama alanıdır. Artık gelenekselleşmiş organ nakli teknolojilerinin ötesinde; çözünebilir moleküller, gen tedavisi, kök hücre nakli, doku mühendisliği, hücre ve doku tiplerinin yeniden programlanması gibi tekniklerden yararlanır. Organ nakli tedavilerinden farklı olarak, bir başka bedenden, doku ve organ aktarılması yerine, insanın bedensel potansiyelini harekete geçirerek, in vivo (canlı bedende) dokunun yenilenerek onarılmasını sağladığı için, organ nakli tedavilerinin de ötesine geçmiştir. Genetik, moleküler biyoloji, malzeme bilimi, kök hücre biyolojisi, gelişimsel biyoloji ve doku mühendisliği yenileyici tıbba katkıda bulunurlar. Doku mühendisliğinde araştırmacılar, hücreleri, yapı iskelesi (scaffold) malzemesiyle, çözünebilir molekülleri büyütme ve geliştirmeye yönlendirerek, yeni doku ve organlar üretmeye çalışırlar(5). Bu yeni tıp teknolojilerinin, dünyada, diyabet, otoimmün hastalıkları, kardiyovasküler hastalıklar, kanser, nörodejeneratif hastalıklar gibi pek çok hastalık grubunu tedavi etme potansiyeli taşıdığı öngörülmektedir.

This article reviews the definition and contextual meaning of social ethics at a broader level by giving special emphasis to the ethical theories and principles, focusing on the societal and public setting. Ethics will be deliberated with social and community aspects. Based on the principle of justice and public health ethics, the concept of social ethics has been investigated concisely through the relationship between man, as a moral person, and the society in exemplification of the issues of healthcare ethics. It is argued that the tension between individualism and communitarian needs can be reconciled with the perspective of social ethics by respecting the individual autonomy without disregarding the common good and social justice. By promoting the values of social responsibility, solidarity, social utility, social ethics has been proposed as the basis of a rational, moral, egalitarian, pluralistic, democratic society rising on the pillars of human rights and human dignity.

This study aims to introduce the Turkish Bioethics Association (TBA) by dealing with its objectives, functions, works and activities as an academic, non-governmental organization in Turkey with a special emphasis on the activities of social responsibility. It will also try to envisage future perspectives of the TBA.
Establishment of TBA
TBA was founded in Ankara in 1994. The idea of establishing a bioethics society in Turkey was inspired by Prof. Yaman Ors and Dr. Yasemin Oguz, on their way back from the 1st World Bioethics Congress at Amsterdam where many countries and members were represented by local, national societies (12). First steps to the Association were taken by the academics of medical ethics, veterinary medicine ethics, and dentistry ethics. Prof. Berna Arda, the first president of TBA worked to institutionalize the Association just from its inception. In a short while, the Association brought together not only bioethicists from the medical sciences, but also the authors of ethics, history and philosophy with young MA and PhD researchers from the allied disciplines all over Turkey (13).
The Scope of TBA
Turkish Bioethics Association is defined, according to its Bylaws, as a platform to take up and discuss problems arising in health care and medical sciences in an interdisciplinary way. This definition necessitates the explanation of the concept of bioethics which is implemented in its broadest sense by the Association. TBA considers bioethics as a discipline dealing with moral value issues in practice of healthcare professions (such as medicine, nursing, dentistry, pharmacy, veterinary medicine, etc.), and also other disciplines (biology, social sciences, philosophy, law, ...) (14). TBA is concerned not only with moral problems stemmed from medical-clinical practices, but also with other moral issues originating from the activities influencing the living beings. Biomedical researches, publication ethics, environmental ethics, animal rights, policies affecting health care system, public health issues and related topics are also main concerns of TBA.
Objectives of TBA
The objectives of the Association are:
• to contribute to the development of bioethics,
• to improve the undergraduate and postgraduate education
• to develop contacts with healthcare disciplines as well as with other relevant areas,
• to facilitate the exchange of information between researchers in bioethics
• to organize regular academic, scientific meetings in bioethics;
• to encourage the development of research and teaching in bioethics;
• to promote and make known of issues of bioethics to the public.
• to be alert and attentive on problems of bioethics offending dignity of the discipline (15).
Recent Meetings
On the basis of social responsibility TBA is quite sensible to the hot topics of the health system in Turkey and some of its recent meetings may be exemplified to this aim: Symposium on the Ethical Aspects of Organ Transplantation (30), Symposium of the New Reproductive Technologies and New Motherhood have been realized to provide for a multidisciplinary platform to discuss the issues in view of medicine, ethics, forensic medicine, law, sociology, psychiatry and history (31). The Panel of Medical Malpractice has set a good example to this approach by dealing with medical error and the mandatory medical insurance that concern all healthcare workers in Turkey (32). The meetings mentioned above have been realized in cooperation with the Istanbul Chamber of Physicians as an indispensable stakeholder of the issues at debate. The latest and sixth congress was named after New Horizons in Bioethics in line with the previous congress in 2008 by emphasizing TBA’s mission to hold meeting in a bioethical perspective. 2010 Congress has achieved this goal successfully by integrating the studies of medical ethics and related sciences together with the cooperative branches such as medical education, medical law, sociology, public health, history, biomedical branches in a multidisciplinary vision. It has also given a special emphasis on the rights of vulnerable groups by assuring them a platform of speech (33).
Turkish Bioethics Association, as a member of EACME, will be organizing the European Association of the Centres of Medical Ethics Annual Meeting for the first time in Istanbul-Turkey on September 15-17th 2011 (34). The scientific programme will cover a wide range of topics related to bioethics from a cross-cultural perspective, including bioethics and humanities, universal values and cultural diversity, European Biomedicine Convention, human rights and bioethics, and health care policy making. This international platform will hopefully provide a basis for handling the professional and moral values and bioethical issues in conflict. On the eve of the event, Globalising European Bioethics Education Summer School will be held for foreign and Turkish participants in conjunction with TBA-EACME Conference on September 11-14, 2011. The Executive Committee wishes its local members and international colleagues all to experience the international arena for exploring moral and ethical values in a cross-cultural vision leaning on a participatory democratic platform nourished from the ethical discourse.
Conclusion
The teaching of ethics owes a great deal to the medical school curriculum in Turkey and has rooted in the steps of 19th century modernization movement in medical education. The emergence of the Turkish Bioethics Association can be better evaluated in line with this historical past that has given rise to the making of a bioethics society. As an academic and non-governmental organization, Turkish Bioethics Association has substantiated itself both at local and international platforms as a promising society based on this rich tradition. It will keep on contributing to the academic literature, collegiate teaching and current debates in healthcare system in view of bioethics by its functions briefly depicted in this study.

GLEUBE Yaz Okulu (İstanbul, 11-14 Eylül 2011)
Oslo Üniversitesi liderliğinde ERASMUS programı kapsamında gerçekleştirilen Globalising European Bioethics Education Yaz Okulu Türkiye’den ve dünyanın çeşitli ülkelerinde 20 öğrenci ile başarıyla yapılmış; Türkiye Biyoetik Derneği, üyeleri ile birlikte bu organizasyona bilimsel, eğitsel katkı sunmuşlardır.
Türkiye Biyoetik Derneği VII. Kongresi (12-13 Ekim 2012)
Derneğimizin iki yılda bir gerçekleştirdiği kongresi, insan hakları, eğitim, sağlık politikaları ana temaları altında, toplumumuzun ve sağlık sistemimizin temel sorunlarını ele alan bilimsel bir tartışma platform şeklinde, uluslararası katılımlı olarak gerçekleşmiştir. Doğrudan biyoetik uzmanı olmayan disiplinlere mensup araştırmacıların ilgiyle ve merakla katkı verdikleri kongremizin Sonuç Bildirgesine Bültenimizden erişebilirsiniz.
(II) Derneğimizin Kitapları:
Biyoetik Araştırmaları: Esas olarak, Derneğimizin 2009-2011 döneminde gerçekleştirdiği uluslararası ve yurt içi toplantılarda sunulan bildirilerden geliştirilen makaleleri içeren bu kitap, alanımız literatürüne özgün bir araştırma kaynağı hizmeti sunmak üzere yayımlanmıştır. Bu yayınla, Derneğimizin başlangıcından, Ekim 2012 tarihine kadar yayımladığı Görüş’leri toplu halde bir araya getirilmektedir. Derneğimizin üyesi bulunduğu EACME tarafından yurt dışında geniş bir kitleye, alanın ilgililerine duyurulmuştur.
Değişen Dünyada Biyoetik: Temel olarak Türkiye Biyoetik Derneği VII. Kongresi'nde gerçekleştirilen konferans, panel ve bildiri metinlerinden geliştirilerek oluşan bu kitapta; toplumumuzda, ülkemizde tartışılmakta olan birçok mesele biyoetik değerler sistemi açısından ele alınmıştır. Kitapta insan hakları biyoetik bağından, kişi hak ve özgürlüklerine; akademik bağımsızlıktan, katılımcı demokrasiye; sağlık çalışanlarına, kadınlara yönelik şiddetten, kadın cinayetlerine; namusun medikalizasyonundan, namus cinayetlerine; dünyada sağlık reformlarından, ülkemizde sağlıkta dönüşüme; tıp eğitiminden yeni bilimsel teknolojilere içinde yaşadığımız toplumun ve akademinin güncel sorunları ele alınmıştır.
Klinik Araştırma Etik Kurulları Kılavuzu Çevirisi
Avrupa Konseyi Biyoetik Yürütme Komitesi (CDBI) tarafından 17 Şubat 2011 tarihinde kabul edilerek yayımlanan bu el kitabı Yeşim Ülman tarafından dilimize çevrilmiş ve EACME Kongresi’nde okurlarla buluşmuştur. Kılavuz kısa zamanda tükenince 2. Baskısı yapılmıştır.

(IV) Türkiye Biyoetik Derneği Araştırma Etiği Ödülü
Türkiye Biyoetik Derneği Araştırma Etiği Ödülü, ilk defa olarak, “insan sağlığını üstün tutan bilimsel araştırmaları” nedeniyle Kocaeli Üniversitesi Halk Sağlığı uzmanı Prof. Dr. Onur Hamzaoğlu’na verilmiştir. Derneğimizin 10.10.2012 tarihli Yönetim Kurulu kararıyla, değerli bilim insanına ödülü Türkiye Biyoetik Derneği VII. Kongresi’nde yer alan “Etik Açıdan Hamzaoğlu Paneli” sonunda takdim edilmiştir. Ödül beratını bültenimizde sunuyoruz.

25–27 Kasım 2011 tarihlerinde Ankara’da gerçekleştirilen TTB-UDEK Tıpta Uzmanlık Eğitimi Kurultayı’nda ilk gün gerçekleştirilen Çalışma Grubu toplantılarına Doç. Dr. Murat Civaner (Etik), Yrd. Doç. Dr. Hafize Öztürk Türkmen (Sürekli Tıp Eğitimi Sürekli Mesleki Gelişim), Yrd. Doç. Dr. Volkan Kavas (Toplum Sağlığını Geliştirme), Dr. Önder İlgili (Asistan ve Genç Uzman Hekimler) Derneğimizi temsil etmişlerdir. Üyelerimizin raporları Bültenimizde yer almaktadır.
Bu Kurultay’da, ilk defa, uzmanlık dernekleri için etkinliklerini, çalışma amaç ve hedeflerini anlatacakları özel bir oturum ayrılmıştır. Bu bölümde Derneğimiz adına Yönetim Kurulu Başkanımız Doç. Türk Tabipleri Birliği Organ ve Kompozit Doku Nakilleri Komisyonu ve Raporu
TTB Nisan ayında derneklere yaptığı çağrı ile ülkemizde son dönemde gerçekleşen yüz ve kompozit doku nakilleri hakkında bir çalışma grubu oluşturmuştur. Derneğimizin temsil edildiği bu Komisyon, olguları tıbbi, etik ve hukuki boyutu ile inceleyerek, oluşturduğu raporunu yayımlamış ve bir basın toplantısı ile kamuoyu ve ilgili taraflarla paylaşmıştır. Bu önemli eli kitabının oluşmasında Türkiye Biyoetik Derneği, Son Dönemde gerçekleştirilen organ ve kompozit doku nakilleri raporu Görüşü önemli kaynaklardan birini oluşturmuştur. Organize Sanayi Bölgelerinde Sokak Hayvanları Sorunu Projesi
Tuzla Organize Sanayi bölgesinde faaliyet gösteren Fritem firması Derneğimize başvurarak, bilimsel destek talebinde bulunmuşlardır. Sosyal sorumluluk projesi kapsamında hazırlamış oldukları, sanayi bölgeleri içinde sokak hayvanlarını; itlaf edilmeden, yaşadıkları doğal çevreden dışlanmadan, uzaklaştırılmadan; beslenme, barınma, tedavi ihtiyaçlarının insani biçimde temin edildiği; insan ile dost, çevre ile uyumlu yaşayabilmelerinin sağlandığı projelerine Derneğimizce destek verilmiştir. Ülke çapında örnek proje olarak geliştirilen bu çalışma, VII. Biyoetik Kongresi’nde bir panelde irdelenmiş Değişen Dünyada Biyoetik kitabımızda yer almıştır.
Derneğimizin 10. Çalışma Dönemi 24 Kasım 2012 tarihinde Ankara’da yapılacak Olağan Seçimli Genel Kurulumuzla tamamlanıyor. Derneğimizin yeni yönetime, yeni projelerinde içtenlikle başarılar diliyoruz.

Türkiye Biyoetik Dernegi Yonetim Kurulu
Bu sempozyumun amacı, yeni tıp teknolojilerinin, kadınların aldığı sağlık hizmetlerini ve annelik süreçlerini nasıl etkilediğini araştırmaktır. Konuya tarihsel bir perspektiften yaklaşarak, günümüzde üreme teknolojilerinin kadınlara ve yaşamlarına nasıl etki ettiği; bu teknolojilerin kadınların yararı ve gereksinimlerine uygun gelişme göstermesi için neler yapılabileceği araştırılacaktır.
Yeni üreme teknolojilerinin getirdiği toplumsal, kültürel sorunlara bakış açışını oluştururken ve ana tartışma başlıklarını çizerken biyoetiğin dört temel ilkesinden yararlanılarak sorulara yanıt aranmaya, çözüm önerileri getirmeye çalışılacaktır.
İnsanlara yarar ve mutluluk sağlama iddiasıyla geliştirilen yeni üreme teknolojileri tam tersine, öjenik yaklaşımlara, "seçilerek doğurulan proje çocuklar" yaratılmasına yol açarak zarar verebilir mi? Bu çelişki annelik pratiğini ve kavramını nasıl etkiler?
Tıp uzmanlarının ve hizmet alan ailelerin, üreme teknolojilerine ilişkin somut gereksinimleri tanımlaması ve bunların karşılanmasını talep ederek yarar sağlaması mümkün müdür?
Üreme teknolojileri bağlamında bakıldığına, farklı toplumsal kesimlerden kadınların annelik kararlarını özerk ve özgür bir birey olarak vermesi mümkün müdür?
Tıbbi hizmete adil ve eşit ölçüde erişim kavramı, yeni üreme teknolojileri bağlamında ne denli geçerli ve gerçekçi olabilir? Toplumların kendine özgü koşulları göz önüne alındığında bu gereksinimlere yanıt verecek şekilde kaynak sağlamak ve hizmet sunmak olanaklı mıdır?
Toplantının sonunda oluşturulacak Sonuç Bildirgesiyle sorunlar ve çözüm önerileri bir bütün olarak sunulacaktır.
Yöntem
Sempozyum paneller biçiminde yapılandırılacak, toplantı sonunda oluşturulacak Sonuç Bildirgesi’yle sorunlar ve çözüm önerileri bir bütün olarak sunulacaktır. Sempozyuma poster bildiri ile katılmak olanaklıdır. Poster Bildiri özeti göndermek için son tarih 20 Şubat 2009'dur. Özetler 200-250 sözcük arası olmalıdır. Yazarların ünvanları, kurumları ve iletişim adresleri belirtilmelidir. Poster bildiriler 70 x 50 cm boyutunda olmalıdır. Değerlendirme kurulunca en iyi poster ödülü verilecektir.

"ETHICAL REASONING IN CLINICAL YEARS:
ETHICS ROUNDS, ACIBADEM UNIVERSITY EXPERIENCE"

Introduction: at Acibadem University School of Medicine, medical ethics and bioethics are integrated in the curriculum in phase one (year 1-2-3) within the professionalism program entitled "Clinical Medicine and Professional Skills (CMPS)" in form of a "Medical Ethics & Humanities" track and in phase two (years 4-5-6) within the clinical clerkships as "Ethics rounds".
Aim: The ethic rounds aim to develop ethical sensitivity and professional motivation during the clerkship period by fostering professional and ethical values in clinical and ethical decision-making in daily practice; to integrate ethical formation in preclinical years with the practice based clinical experience with actual cases.
Materials and methods: Ethic Rounds are featured within the internal medicine clinical clerkship which takes place four times within the academic year, rotating groups of students. Additionally, Forensic Medicine inserted Ethics Rounds in the Clerkship curriculum in the 5th year. Thus, five ethics rounds are organized.
The ethics rounds are organized in an interdisciplinary manner with clinicians from different branches. Presented cases may feature different themes working together with the department of medical ethics. The clinicians are asked to choose a case representing an ethical dilemma from their daily clinical practice. They are provided with a guide to prepare this case for discussion with the students during the ethics round. The format of this guide includes a short case description, formulation of the problem and identification of the related ethical values and/or principles. The clinicians are also provided with a short theoretical outline about ethical values and principles.
Results:
The students described the ethics rounds as beneficial, remindful, stimulating, and proposed repeating these exercises in each clinical year. They think this practice has stabilized what they have already learned in ethics lessons at preclinical years. They find it instructive to think over the clinical case in the perspective of ethical issues and legal practice. They liked that the cases were chosen from the real daily clinical practice. They enjoyed to express their ideas and views actively and in an open way with peers and clinicians and instructors. They commented that they glean ideas, take lessons from the clinicians’ experiences for managing difficult patient encounters and breaking bad news.
The clinicians find the ethics rounds beneficial. They think that the systematic real life experience such as ethics rounds at undergraduate level will be quite helpful for students to get ready for coping with the ethical dilemmas and conflicts in professional life.
Conclusion: Ethics Rounds is sustainable and compatible with the vertical integration in medical education in order to enhance professionalism.

The Turkey Working Group realized its third workshop on the theme “How can ethics be taught in health sciences?" to discuss developing an approach towards the current challenges of methodology of ethics education in the context of healthcare professionals’ moral development. By answering this question, the Workshop aimed to emancipate educators, professionals from clichés by encouraging them to seek ways for innovative educational methods, by giving emphasis on the importance of collaboration and interaction. We think the systematical review of the themes referred by the Group in this workshop, would be a useful initial structural ground for further discussions and implementations.
The learning environment of medical education has both negative and positive influences on students’ acquisition of ethics-related skills, as well as professional skills, knowledge and attitudes. Considering the recent trend in medical education towards more student-centered and professional values-oriented curricula, one should expect that today graduates are more humanistic and virtuous. On the other hand, students are generally idealistic and compassionate when they start the journey to become doctors. However, despite students’ good intentions and medical schools’ struggle for improving teaching of ethics, related skills and attitudes, cynicism emerges eventually in most of the students while they are becoming healthcare professionals. The Cambridge Bioethics Education Turkey Group proposes that a student-centered and professional values-oriented curriculum can help to raise more humanistic and virtuous students. Despite the barriers in the current structure of modern medicine and medical education, there are effective student-centered teaching methods and various strategies to overcome negative influences of the hidden curricula, which in the end help us produce better equipped healthcare professionals in terms ethical competency, moral awareness and reflective skills.

The Cambridge Consortium of Bioethics Education was established by the Cambridge University Press in 2011. The Platform mainly aimed at developing bioethics education through annual meetings with other bioethics educators around the world to share experiences and ideas on developing curricula . The Consortium initiated country-based Working Groups in June 2013, specifically in Hungary, New Zealand, Pan-Arab (Lebanon), Romania, Russia, Serbia, Spain, The Netherlands, and Turkey .
The purpose of the Working Groups is to encourage people and institutions to work together on developing bioethics education in their countries by setting up focus groups that invite others to share information and develop mutual projects; to further how bioethics is taught in universities, hospitals, and to the general public.
Turkey Working Group
Upon the invitation of the Cambridge Consortium, the Turkey Working Group was established in 2013. Focus Group Members in Turkey are composed of Murat Aksu, Fatih Artvinli, Nadi Bakirci, Tuna Cakar, Muhtar Cokar, Figen Demir, Volkan Kavas, Gulsum Onal, Isil Pakis, Melike Sahiner, Pinar Topsever, Inci User, Yesim Isil Ulman (chair), Kevser Vatansever, Vedat Yildirim. The Group is based in the History of Medicine and Ethics Department at Acibadem University School of Medicine in Istanbul. The Group dynamic is based on the volunteering zeal and dedication of the professionals forming this collective.
Participants: Kevser Vatansever, Volkan Kavas, Yesim Isil Ulman (Turkey), Yolande Voskes, Menno de Bree, Guy Widdershoven (Netherlands), Peter Kakuk (Hungary), Vojin Rakic (Serbia), Rouven Porz (Switzerland, guest).
Aim of the meeting: to exchange experiences and explore the opportunities for mutual, multi-based scientific research for the working groups in the Cambridge Consortium of Bioethics Education.
Program of the meeting: After an introductory round of the participants, a draft research proposal was presented on behalf of the Turkish working group. In the afternoon, a seminar was organized with the staff of the Department of Medical Humanities, on ethics education in international perspective. Next, possibilities for funding were discussed, especially COST. Finally, future cooperation was discussed, focusing on goals and possible contributions to European programs, and decisions were made for further actions.
The Group began to act by organizing workshops to assign its aims and strategies. The aims of the Group are to raise awareness in ethics education and moral decision making in the academy and at clinical setting; to work multiprofessionally in coordination with other healthcare professionals; to enhance advocacy of ethics teaching at university; to follow humanities perspective; to share ideas on curriculum development; to compare and learn mutually from the experiences; to choose topics for discussion on the issues of bioethics and societal issues; to structure multi-based research among countries. It is opening a specific website to facilitate the cooperation and sharing of information.
The Group specified its third workshop on the methodology of teaching bioethics at medical schools. This workshop will take place at Ankara University School of Medicine on May 23rd, 2014, hosted by the Department of Medical History and Ethics. This oral presentation will give information about the functions of the Group in detail.

Workshop Group I Report
By answering the question above, while creating new ones in search of answers to them, this Workshop aims to emancipate ourselves from clichées and stereotyped thoughts and applications; it intends to emphasize the importance of collaboration and interaction.
Discussing the feasibility of teaching bioethics, noticing the positive and negative effects of learning environment over ethical values and ethical decision-making will help to be aware, to understand, to solve the problems in this process. During medical education, the dynamics at macro level are to determine particularly the development of ethical attitudes and behaviors of the medical students, young physicians and healthcare professionals in general. This determining factor in question can have either positive or negative effects. As a result of this interaction, the student develops attitudes and behaviors different from the point he has started in the beginning of medical education process.
Toplantı, Montparnasse’ta, 18. yüzyılda bir porselen fabrikasıyken, 1964’te Columbia Üniversitesi’ne geçmiş; önceleri S. de Beauvoir, J. Derrida, R. Barthes gibi Fransız entelektüellerinin devam etmiş olduğu bir araştırma merkezi olan Reid Hall’da yapıldı.
Yöntem:
Toplantıda, önceki yıllardaki gibi bu yıl da, üç gün boyunca, biyoetik ve insan bilimleri eğitimi, biyoetik değerler, klinik etik, araştırma etiği vb. konularında uygulama örnekleri sunularak, deneyimler paylaşıldı. Her oturumda soru ve katkılarla geniş yer ayrılarak, derinlemesine tartışmalar yürütüldü. Son gün öğleden sonra ise, Haziran 2013 toplantısında kurulmuş bulunan ülke grupları faaliyetlerine ayrılan bir panel yapıldı.
DH-BIO, bu Kılavuzu hazırlarken, özellikle, 30 Kasım ve 1 Aralık 2010 tarihlerinde, Biyoetik İzleme Komitesi (CDBI) tarafından düzenlenen, yaşamın son dönemindeki tıbbi olgularda karar verme sürecini ele alan sempozyumda varılan sonuçlarına dayanarak çalışmıştır.
Bu Sempozyumun koordinasyonu, CDBI Başkanı Sayın Isabelle Erny’ye (Fransa) tevdi edilmişti. Sempozyum öncesinde yapılan hazırlık çalışmalarında, CDBI, başlıca iki rapordan yararlanmıştır: Raporlardan ilki, Prof. Lucie Hacpille (Fransa) tarafından kaleme alınan, “Yaşamım son dönemindeki olgularda alınan tıbbi kararlar ve mevcut seçeneklerin etik yönü”dür. Diğeri ise Prof. Roberto Andorno’nun (İsviçre), “Tıbbi Bakımla ilgili önceden belirtilmiş istekler, ortak İlkeler ve ulusal hukuk sistemleri arasındaki farklılıklar” başlıklı raporudur.
Sempozyum sonunda elde edilen sonuçlara dayanarak, tıbbi karar verme sürecini desteklemek üzere bir kılavuz taslağı hazırlamak amacıyla, çalışma grubu oluşturulmuştur. Isabelle Erny başkanlığındaki bu çalışma grubu, Dr. Béatrice Ioan (Romanya), Prof. Andreas Valentin (Avusturya) ve sempozyum genel raportörü Prof. Régis Aubry (Fransa)’den oluşmaktadır.
Aralık 2012’de, DH-BIO, çalışma grubunun hazırladığı kılavuz taslağını, konuyla ilgili tarafların görüşlerine açmağa karar vermiştir. Taslak, 2013 yılı Şubat ayından Nisan ayına kadar kamuoyunun görüşlerine açık tutulmuştur. Bu süre zarfında, (özellikle, hastalar, sağlık profesyonelleri, biyoetik uzmanları ve insan hakları hukukçuları gibi) konuyla ilgili taraflarının görüş ve önerileri alınmıştır.
Kılavuz Taslağı, alınan görüşleri kapsayacak biçimde yeniden gözden geçirilmiş, DH-BIO’nun 4. Genel kurulunda (26-28 Kasım 2013) metin, tartışılarak onaylanmıştır. Kabul edilen Kılavuz, İnsan Hakları İzleme Komitesi (CDDH)’ne sunulmuş; burada yapılan incelemeden sonra bilgi için Avrupa Konseyi Temsilciler Meclisi’ne gönderilmiştir.
