This article explores the way mental health services and social services are orientated on assisting people with a psychiatric disability to participate in different areas of community life. A large research project about community participation in three different countries (Estonia, Hungary and the Netherlands) confirms the findings of other studies that – regardless of geographical location – people with a psychiatric disability have difficulty fulfilling social roles in the community. They are often faced with fewer opportunities than other citizens, due to their health problems, stigma, discrimination and poverty [World Health Organisation. (2011). World report on disability. Geneva: WHO]. An important objective of social work is to help people find their way in society. Surprisingly enough, in all three countries, most social workers are mainly focused on solving individual problems, and not on community participation. Shifting the focus starts by having a clear awareness of the notion of community participation. Social workers have to work both on supporting individuals to find their way in the community (individual support), and on supporting social networks in the community to accommodate persons with a disability on the basis of equal and valued citizenship (community support).

The proportion of elderly people in the society is constantly growing. The elderly who live alone form one of the most vulnerable groups in the society that is also at considerable risk for poverty and social isolation. According to the last population census in Estonia (2011), 39.3% of the elderly (65+) lived alone. The ageing of the population is accompanied by novel challenges, among them especially the coping of the elderly living alone. In 2015, the research team at the Tallinn University School of Governance, Law and Society conducted a study to map Estonian community-based practices that are available for the elderly (65+), especially for those who live alone at home, and the way these practices support interdependent coping and prevent the need for institutional care. This study, based on qualitative and quantitative data collected in Estonian communities, suggests that cooperation between Estonian local governments and communities should be more effective and involve regular interaction. Promising practices include developing network-based community support models and broadening communication possibilities.

The Best Practice Unit (BPU) model constitutes a unique form of practice-based research. A variant of the Community of Practice model developed by Wenger, McDermott and Snyder (2002), the BPU has the specific aim of improving professional practice by combining innovation and research. The model is implemented over a period of one to two years by a group of professionals, researchers and other relevant individuals, who work towards a desired improvement. The model is characterized by interaction between individual and collective learning processes, the development of new or improved working methods, and the implementation of these methods in daily practice. Multiple knowledge resources are used, including experiential knowledge, professional knowledge and scientific knowledge. The research serves diverse purposes: articulating tacit knowledge, documenting learning and innovation processes, systematically describing the working methods that have been revealed or developed, and evaluating the efficacy of the new methods. Each BPU is supported by a facilitator, whose main task is to optimize learning processes. An analysis of ten different BPUs shows that this is a successful model. The article describes the methodology and results of an analysis of ten BPUs in different professional fields.

Ervaringsdeskundigheid is populair. Met name binnen de GGz en de Verslavingszorg worden steeds meer ervaringswerkers in dienst genomen. Dit doet een toenemend beroep op opleidingsmogelijkheden, zowel op MBO als op HBO niveau, in reguliere opleidingen of in korte trajecten voor deskundigheidsbevordering en training. In de loop van de bijeenkomsten van de Landelijke Denktank Opleidingen en Onderzoek Ervaringsdeskundigheid zijn deze ontwikkelingen besproken. Er werd gezocht naar de kenmerkende elementen van ervaringsdeskundigheid en naar de bijbehorende competenties. In de loop van deze zoektocht bleek een toenemende behoefte aan een duidelijk begrippenkader. Een helder begrippenkader vergemakkelijkt immers het ontwikkelen van beleid rond zowel ervaringsdeskundigheid zelf, als ook rond opleidingen, onderzoek en inbedding in de praktijk. Deze publicatie is een groeidocument, dat ten doel heeft de grote schat aan ervaringsdeskundigheid en ervaringskennis die (ex)cliënten van GGz, Verslavingszorg en Maatschappelijke Opvang, een herkenbare plaats te geven, en verder tot bloei te brengen. Professionals in zorg en welzijn hebben te maken met nieuwe vraagstukken rond de zorg en ondersteuning van mensen met ernstige psychische, sociale en lichamelijke beperkingen (RMO, 2001a). Aan de ene kant gaat het om basale ondersteuning in de woonsituatie, aan de andere kant betreft het de bevordering van participatie van deze burgers in de samenleving. Zorg en welzijn kunnen niet langer gescheiden van elkaar aangeboden worden. Het vereist een nieuwe geintegreerde individuele en wijkgerichte aanpak. Deze aanpak vraagt om professionals die zowel kunnen werken aan de kant van individuele ondersteuning als aan de kant van sociale samenhang en maatschappelijke participatie.

Het boek is geschreven voor professionals in de praktijk van zorg en welzijn, en voor studenten van sociale opleidingen. Het bevat bijdragen die gericht zijn op het geven van ondersteuning in de ‘frontlinie’ van de Wmo, aan individuen of groepen. Het bevat ook waardevol materiaal voor bestuurders en beleidsmedewerkers van gemeentes en uitvoeringsorganisaties, bijvoorbeeld hoe ‘de Kanteling’ vorm en inhoud kan worden gegeven, en hoe maatschappelijke steunsystemen opgezet kunnen worden. Het gaat om praktische methoden en benaderingen die zich goed lenen om interdisciplinair te gebruiken.

De transitie van het sociale stelsel brengt ingrijpende veranderingen met zich mee. De huidige stelselwijziging gaat niet alleen gepaard met veranderingen in de financiering en organisatie van de dienstverlening, maar ook met verandering in visie en benadering. Het toekomstperspectief is een samenleving waarbij mensen zo zelfredzaam mogelijk zijn, maar ook een aandeel nemen in de zorg voor elkaar. De lokale overheid en sociaal professionals hebben vooral een faciliterende functie en springen bij als het echt nodig is. Of het nu gaat om burgers, beroepskrachten, organisaties of overheden, de transformatie vereist dat alle participanten in het sociale domein leren anders te kijken en te handelen dan ze gewend waren. Telkens is de vraag of en welke professionele inzet nodig is, en hoe dit in samenspel binnen sociale netwerken tot stand kan komen. Dit is een terugkerende zoektocht.
Klassieke vormen van leren en onderwijzen volstaan niet. De nieuwe werkelijkheid moet al doende nog worden uitgevonden. Het is een kwestie van experimenteren, reflecteren en zoeken naar de beste oplossing. Dit zoekend en ontwikkelend leren wordt gekenmerkt door een grote mate van onzekerheid. Onderweg staan veel uitdagingen en dilemma’s te wachten. Hoe doe je zoveel mogelijk recht aan de vragen van de burger, zijn leefwereld en hoe kunnen systemen hieraan dienstbaar gemaakt worden? Hoe zorg je dat de visie overeind blijft als zich tegenkrachten voordoen? Wat kun je organiseren om het leren van diverse betrokkenen te ondersteunen, zonder al te veel te sturen? Dat is de kern van dit boek over transformatieleren. Het boek biedt een aantal boeiende inzichten hoe in de praktijk nieuwe vormen van leren gestalte kunnen krijgen. Leervormen die actiegericht en innovatief zijn en die teams en lokale netwerken helpen hun praktijk te ontwikkelen. Hierbij gebruik makend van alle kracht die aanwezig is: burgerkracht, beroepskracht, organisatiekracht, ondernemerskracht en leerkracht.

Tallinn University in co-operation with Tartu University has conducted a four month long research on deinstitutionalisation policies, financed by the Estonian Ministry of Social Affairs, between August and November 2015. The research programme consisted of an international review on the experiences of seven selected European countries and on focus groups and individual interviews among Estonian stakeholders related to deinstitutionalisation and community based services in the field of disability care and mental health. Based on the research the international research group suggested a number of considerations for the Estonian Case. Some of the most relevant are reported in this Research note.
De veronderstelling dat formele zorg zonder meer tegen haar informele variant kan worden uitgeruild en dat de participatiesamenleving maakbaar is, getuigt van een simplistische denkhouding. Sociale innovatie gebaseerd op dergelijke verkeerde aannames is bijzonder risicovol.
