Methode Das Projekt „Strukturierte Transition und Integration in der Rehabilitation“ (STIR) entwickelte partizipativ mit vier Rehabilitationskliniken in Deutschland verschiedene Maßnahmen zur strukturierten Transition und beruflichen Integration migrierter Ärzt*innen. Zu den ausgewählten Maßnahmen gehören ein Kurzfilm, eine Website, Führungskräftetrainings, interkulturelle Trainings, ein Peer Support Programm und ein Gesprächsleitfaden für Startgespräche. In diesem Artikel beschreiben wir die Vorbereitung, Durchführung und Evaluierung der Prototypen der oben genannten Maßnahmen im Rahmen einer Machbarkeitsstudie bezüglich Akzeptanz, Praktikabilität und Integrierbarkeit. Zusammenfassung Hintergrund Aufgrund der älter werdenden Bevölkerung und der Zunahme von Menschen... more Zusammenfassung Hintergrund Aufgrund der älter werdenden Bevölkerung und der Zunahme von Menschen mit chronischen Erkrankungen im Alter, steigt die Zahl der pflegenden Angehörigen sowie deren Wert für die Versorgung. Dabei agieren sie meist ohne Vorerfahrungen im Bereich der Versorgung, haben begrenztes Verständnis für das Gesundheitssystem und erfüllen die Aufgabe neben der Bewältigung des eigenen Alltags. Daher gewinnen technische Unterstützungsmöglichkeiten wie die App Liv (entwickelt im Projekt münster.land.leben) an Bedeutung. Ziel der App Liv ist die Verbesserung der digitalen Gesundheitskompetenz der vulnerablen Gruppe der pflegenden Angehörigen in Deutschland. Background: This paper examines a paradox in the German healthcare system: Complementary and alte... more Background: This paper examines a paradox in the German healthcare system: Complementary and alternative medicine (CAM) practices are a major element of medical encounters in Germany. Patients seek them, physicians provide them, and public health insurances pay for them in part. Despite all this, CAM practices are not acknowledged as scientifically valid. Material and Method: I will examine this situation in detail based on 2 ethnographic studies. The first study refers to an attempt to introduce homeopathic education at a German university. The second one is a study in the context of cancer and CAM. These cases are perfect examples of the current power struggles that are impeding the expansion of CAM practices in Germany. Results: The results should be seen from the theoretical angle of the study of science. The conventional method of proving scientific validity is in contradiction to those frameworks in which the impact of CAM might be demonstrated. There are economic interests invested in preventing the integration of CAM into existing scientific structures. However, the current hybridization of CAM with conventional medicine might be a step towards an institutionalized heterogenization of medical practices in Germany. Conclusions: A broader understanding of scientific methods within the CAM community could provide a useful frame for future research. I suggest that the CAM community more actively takes part in the discourse with representatives of conventional medicine and come out of the closet.

In this article, we analyze the collective healing process that takes place on a weekly basis in ... more In this article, we analyze the collective healing process that takes place on a weekly basis in the Divine Retreat Center (DRC) in Muringoor, Kerala. We argue that disease in the DRC is understood either as a psycho-somatic or as a spirito-somatic phenomenon. In contrast to other Charismatic communities, however, the body is the locus on which the medical effects of the healing become visible. The whole process is divided into several phases: First, there is a cleansing and disengagement procedure that aims to purify and liberate the participants through confession and counseling. Thereafter comes a climatic phase of personal emptying, transition and re-orientation during which the healing itself takes place. The procedure is finally completed with the person being spiritually "refilled" by the Holy Spirit. The dominant recurring element in the whole process is the continuous statement of healing "testimonies." As an integral part of the healing procedure, these statements are used to share personal experiences among the participants in the center. They are produced in a strict format in order to be spread far beyond through various media (TV, newspaper, Internet, etc.). They thereby constitute a speech genre that follows specific rules and patterns. Through shaping one's own biography in the frame of the testimonies, so we argue, the actual transformation of the self and therefore the miracle healing takes place.

Objectives: In Germany, rehabilitative healthcare institutions increasingly rely on migrant physi... more Objectives: In Germany, rehabilitative healthcare institutions increasingly rely on migrant physicians to meet their staffing needs. Yet until now, research on the integration of migrant physicians has focussed entirely on the acute care setting. This study is the first to address the specific advantages and challenges to integration in the field of rehabilitative medicine where a high number of migrant physicians work. From the experiences of migrant physicians and their colleagues, we provide actionable suggestions to counteract potential sources of conflict and thereby improve the integration of migrant physicians in the German workforce. Study design: We conducted a qualitative interview study. Methods: We conducted 23 interviews with a total of 26 participants occupying a variety of roles in two different rehabilitation centres (maximum variation sampling). Interviews were recorded, transcribed verbatim and parsed through thematic analysis. Results: Our research revealed advantages and challenges to integration in three distinct areas: rehabilitative care institutions, competencies of migrant professionals and inter-personal relations. The first set of issues hinges on the work processes within rehabilitative hospitals, professional prospects there and the location of the institutions themselves. Second, migrant physicians may encounter difficulties because of limited linguistic skills and country-specific knowledge. And finally, aspects of their interactions with care teams and patients may constitute barriers to integration. Conclusions: Some of the factors influencing the integration of migrant physicians are the same in both rehabilitative and acute medicine, but the rehabilitative setting presents distinct advantages and challenges that are worthy of study in their own right. We outline several measures which could help overcome challenges to the integration of migrant physicians, including those associated with professional relationships. Further research is needed to develop concrete support programmes.

Dieser Beitrag zeigt, dass Konflikte zwischen migrierten Ärzten und dem Ärzte- und Pflegeteam über Aufgaben, Zuständigkeiten und Verantwortlichkeiten der einzelnen Gesundheitsberufe häufig als Mangel an Kompetenzen der migrierten Ärzte interpretiert und nicht auf die divergierende berufliche Sozialisation zurückgeführt werden.

The appropriation of biopsychiatric concepts such as depression, and their reframing in clinical ... more The appropriation of biopsychiatric concepts such as depression, and their reframing in clinical and academic discussions, are important parts of the revitalization of bhūt vidyā as Ayurvedic psychiatry. Drawing on ethnographic fieldwork conducted in Kerala from 2009 to 2011, in this article we explore the process and the controversies of translating and correlating the biopsychiatric notion of depression, as a discrete and biologic pathological entity, with Ayurvedic notions of body, mind, and mental distress. Depression, conceptualized as a neurochemical imbalance, is, we argue, relatively compatible with Ayurvedic notions of a fluent body and mind, and so is easier to correlate with Ayurvedic concepts of do ic imbalances and blockages of channels than the former psychoanalytically dominated model of depression. The appropriation of depression within Ayurvedic discourse challenges the dichotomy of universal and culture-specific disorders, and this has a significant impact on mental health programs in Kerala.
