Wie in den Vorjahren verunglückten im Jahr 2012 in Deutschland knapp 4.000 Menschen im Autoverkehr tödlich, rund 10.000 hingegen nahmen sich selbst das Leben. Doch der Umgang damit ist paradox: Während seit Jahrzehnten alle er-denklichen Anstrengungen (Einführung des Sicherheitsgurtes, Bau von Fahrradstraßen etc.) zur Reduktion der Zahl der Verkehrstoten unternommen werden, gibt es keine vergleich-baren Anstrengungen, die hohe Rate der Selbsttötungen zu reduzieren. Dabei sprechen die nackten Zahlen eine deutliche Sprache: Auf jede Selbsttötung kommen 10 bis 15 Suizidversu-che, rund 30 Prozent aller Sterbefälle durch äußere Ursachen gehen auf das Konto der "vor-sätzlichen Selbstbeschädigung" (so der Terminus des Statistischen Bundesamtes) zurück, und in der Gruppe der jungen Erwachsenen ist Selbsttötung die häufigste Todesursache überhaupt. Selbstmord oder Suizid ist dennoch kein Thema, das die Gesellschaft beunruhigt. 1 Das war nicht immer so. Vor rund 100 Jahren stand die Selbsttötung im Mittelpunkt der öffent-lichen Aufmerksamkeit, und zwar in der breiten Öffentlichkeit der Tagespresse ebenso wie in der Fachöffentlichkeit der Seelsorger, Pädagogen, Ärzte und Gesundheitspolitiker. 1927 lagen mehr als 5.000 eigenständige Schriften zu diesem Thema vor, die Zahl der Beiträge in Zeitungen und Zeitschriften ist Legion. 2 Heute hingegen sind die vom Statistischen Bundes-amt veröffentlichten Suizidzahlen kaum eine Notiz wert. Warum beunruhigt der Freitod oder Selbstmord unsere Gesellschaft nicht mehr? Die Antwort scheint einfach: Suizid ist ein Problem der Medizin geworden. This article introduces Turing’s idea of a “paper machine” to identify and understand one importa... more This article introduces Turing’s idea of a “paper machine” to identify and understand one important mode of clinical research in the modern hospital, how that research worked, and how office technology and industrialized labor shaped and helped drive it. The unusually rich archives of Berlin psychiatry allow detailed reconstruction of the making of the new diagnostic category “hyperkinetic syndrome” in the 1920s. From the generating of data to the processing of information to the visualizing of the nature and course of the new syndrome in the lives of more than sixty patients, this case study shows how clinical research could be based on the apparatus of the clerks’ room (folders, registers, inventories, and the dispatch of documents), office technologies (new filing systems, preprinted forms, and duplicating machines), and the principles and paper practices of the division and rationalization of labor (charts organizing worktime in complex organizations). The result is an important example of clinical research embedded in the broader history of office technology, industrial labor, and the modern hospital.

However, there is one of the oldest literary genres hardly discussed in historiography or medical practice: The aphorism which has experienced a remarkable conjunction in the early 18th century in the form of tabular observations. This article shows how the knowledge of aphorism, condensed into short sentences, relates to other forms of medical reasoning and action. What does it mean to practice socially responsible science on controversial issues? In a fresh tu... more What does it mean to practice socially responsible science on controversial issues? In a fresh turn focussing on the neuroscientists’ responsibility in producing knowledge about politically charged subjects, Chalfin et al. (Am J Bioethics 8(1):1–2, 2008) caution neuroscientists to be careful about how they present their findings lest their results be used to support unfounded biases, social stereotypes and prejudices. Weisberg et al. (J Cogn Neurosci 20(3):470–477, 2008) discuss the allure of neuroscience explanations and demonstrate how laypersons easily accept dubious claims as long as (even non-relevant) neuroscientific descriptions are provided. Fine (2010) exposes the use of scientific evidence in propagating outdated gender myths by popular writers and discusses the infiltration of these prejudices into popular belief, folk culture and lifestyle. This paper discusses ways in which the ‘neuroscience of gender difference’ itself inadvertently contributes to normalising socially constructed theories about sex difference in cognitive performance. This unpremeditated effect has evident implications on the structuring of society because gender relations cut across social, political and economic boundaries. We present a theoretical reflection of factors that could interact with the scientists’ attempted objective evaluation of the subject, the methods and some principle problems, and we engage a science studies approach as our methodological tool. Our object of critique is drawn from the research on spatial abilities that articulate the dissention pertaining to sex differences in intellectual capacity.

In der Neuzeit gehört die Begutachtung zum Repertoire medizinischer Tätigkeiten, ob vor Gericht, in Sachgutachten oder bei Aufgaben des Gemeinwesens. Der Band widmet sich erstmals systematisch dieser verbreiteten, in der historischen Forschung aber unterbelichteten Praxis, die der Vorbereitung informierter Entscheidungen in Justiz, Verwaltung und Politik dient. Die Beiträge des Konzeptbandes verfolgen das Gutachten als eine Form des wissenden Handelns vom 16. bis zum 20. Jahrhundert. Am Beispiel der Medizin arbeiten sie heraus, dass im Zuge einer Begutachtung nicht einfach Fachwissen in einen fachfremden Entscheidungszusammenhang übertragen wird. Vielmehr prägt die Logik des Verfahrens selbst die Mobilisierung und Generierung von Wissen. Als im Mai 2013 der Spiegel seine Story über die „Günstige Teststrecke“ lancierte, machten Worte ... more Als im Mai 2013 der Spiegel seine Story über die „Günstige Teststrecke“ lancierte, machten Worte von „schnellem Profit“ und „Menschenversuchen“ die Runde. Es war die Rede vom „Versuchslabor Ost“, in dem „unerprobte Arzneien“ eingesetzt wurden. Nur wenige Stimmen rieten zur Besonnen-heit und Genauigkeit im Umgang mit diesem Kapitel der west-östlicher Vergangenheit, das kei-neswegs so unbekannt war, die der Spiegel damals glauben machen wollte. Der vielfach geäußerte Verdacht ethischer und rechtlicher Grenzverletzungen gab den Anlass für ein breit angelegtes Forschungsvorhaben. Es wurde wesentlich von dem bzw. der Bundesbeauftragten der Bundesregierung für die Neuen Bundesländer und der Bundesstiftung zur Aufarbeitung der SED-Diktatur gefördert und finanziert. Auch die Bundesärztekammer, die Landesärztekammern Baden-Württemberg, Bayern, Berlin, Hamburg, Niedersachsen, Saarland und Sachsen sowie der Verband forschender Arzneimittelhersteller (VfA) und der Bundesverband der Pharmazeuti-schen Industrie (BPI) haben sich an der Finanzierung beteiligt. Die Studie wurde von einem unabhängigen Forschungsteam unter Leitung von Prof. Dr. Volker Hess am Institut für Geschichte und Ethik der Medizin am Universitätsklinikum Charité in Berlin durchgeführt. Ein internationaler wissenschaftlicher Beirat von unabhängigen Experten und Exper-tinnen aus Medizin und Geschichtswissenschaft unter dem Vorsitz von Prof. Dr. Carola Sachse (Universität Wien) hat die Forschungen begleitet. Die vorliegende Monografie zieht nach zweieinhalb Jahren intensiver historischer Aufarbeitung Bilanz: • Für den Zeitraum von 1961 bis 1990 fanden sich Hinweise auf bis zu 900 klinische Studien, die im Auftrag von Westfirmen in der DDR durchgeführt wurden. Davon sind 321 Studien für eine genauere Analyse archivalisch hinreichend dokumentiert. • Auftraggeber waren überwiegend branchenführende bundesdeutsche, aber auch schweizeri-sche, französische, britische und US-amerikanische Firmen. Insgesamt wurden Aufträge von 75 Firmen aus 16 westlichen Ländern nachgewiesen. • Als Prüfzentren fungierten 120 Universitätskliniken, Bezirkskrankenhäuser und andere Einrich-tungen des Gesundheitswesens, die sich über das gesamte Gebiet der DDR verteilten. • Die westlichen Auftragsstudien in der DDR waren überwiegend Teil von größeren multinational und multizentrisch angelegten Arzneimittelprüfungen. Die Teilstudien wurden in allen ange-schlossenen Prüfzentren, ob in der DDR oder in anderen Ländern, nach den gleichen Standards durchgeführt. • Diese Standards entsprachen – auf beiden Seiten des Eisernen Vorhangs – nicht den heutigen Regeln, sondern wurden im Zuge der Weiterentwicklung internationaler ethischer Regeln und nationaler Gesetze nach und nach konkretisiert. Systematische Verstöße gegen die historisch jeweils geltenden Regeln konnten für die in der DDR durchgeführten Studien nicht nachgewie-sen werden. Verstöße in Einzelfällen konnten wir für die DDR nicht in einem vermehrten Ausmaß feststellen. • Der Befund hinsichtlich der Aufklärung und Einwilligung von Patientinnen und Patienten bleibt unvollständig: In allen von uns geprüften archivierten Studienberichten sind Einverständniser-klärungen anonymisiert und listenförmig dokumentiert. Über die tatsächliche Praxis der Aufklä-rungsgespräche liegen jedoch nur vereinzelte Informationen aus Interviews mit Patient/inn/en und Prüfärzten der DDR-Kliniken sowie Monitoren der auftraggebenden Firmen vor. Sie lassen vermuten, dass zumeist den jeweils geltenden Regel und dem je historischen Verständnis des Arzt-Patientenverhältnisses entsprechend gehandelt wurde. • Die DDR-Behörden stellten westlichen Unternehmen das Gesundheitssystem ihres Staates als Forschungseinrichtung zur Verfügung, um knappe Devisen für die eigene überschuldete Plan-wirtschaft zu erwirtschaften. • Die Annahme, die klinischen Studien seien in der DDR besonders „billig“ zu haben gewesen, wird der komplexen Sachlage nicht gerecht. Der entscheidende betriebswirtschaftliche Vorteil auf Seiten der westlichen Auftraggeber war nicht das geringere Honorar, sondern der beträchtliche Zeit- und Effizienzgewinn, den die aufsichtführenden Behörden des diktatorischen DDR-Regimes gewährleisteten, indem sie Einzelinteressen von Prüfzentren und Prüfärzten deckelten und öffentliche Kritik ausschalteten und so für eine zügige operative Durchführung sorgten. • Das ökonomische Ungleichgewicht bot westlichen Arzneimittelherstellern noch weitere Vorteile: Anhand einer Reihe von Einzelfällen lässt sich zeigen, wie sich Pharmafirmen gezielt das Gefäl-le der medikamentösen Versorgung zwischen Ost und West zunutze machten, um in der DDR Studiendesigns zu realisieren, die im Westen zum jeweiligen Zeitpunkt so nicht mehr durchführbar waren.

The history of psychiatric care has so far been presented as a history of hospitali- sation. A cl... more The history of psychiatric care has so far been presented as a history of hospitali- sation. A closer look to psychiatric policlinics may afford a wider and more ba- lanced perspective. The scant literature on this out-patient facility has concentrat- ed only on its ties with the academia, presenting it as an instrumental appendix of the university clinic into the city, a filter for and a reservoir of scientifically inter- esting cases. This paper proposes a comparative analysis of the diagnostic spec- trum of the clinic and of the policlinic in Strasbourg and Berlin. A comparative appraisal of two lines of tension – the clinic vs. the policlinic and the new-found- ed institution in Strasbourg vs. “first psychiatry” of the German Empire – will show the main differences in clientele, diagnostic grids, and distribution of the main diseases categories. Drafting the rough picture of the major differences in terms of complaints, troubles, and mental conditions with witch outpatients con- fronted the city-clinic the paper argues that the policlinic was more than an extension of the asylum into the urban space. While policlinics may well have been conceived and financed as a means of research and teaching, they were shaped by the novel and specifically urban disorders, and developed along an interplay with the rapidly growing industrial city. In this sense, the policlinic grew into an au- tonomous institution strongly influenced by local legislative and administrative conditions, such as admission standards, remittance and transfer routines, reimbursement or competing facilities. Consequently, we argue that, instead of “medi- calising” the town, the policlinic represented an “urbanization” of the traditional psychiatric institution.

Die Einführung der Psychopharmaka in die DDR aus der Perspektive der neueren Arzneimittelgeschichte,” Medizinhistorisches Journal 42 (2007)). The project was promoted by the Deutschen Forschungsgemeinschaft (HE 2220–2227). Der vielfach geäußerte Verdacht ethischer und rechtlicher Grenzverletzungen gab den Anlass für ein breit angelegtes Forschungsvorhaben. Es wurde wesentlich von dem bzw. der Bundesbeauftragten der Bundesregierung für die Neuen Bundesländer und der Bundesstiftung zur Aufarbeitung der SED-Diktatur gefördert und finanziert. Auch die Bundesärztekammer, die Landesärztekammern Baden-Württemberg, Bayern, Berlin, Hamburg, Niedersachsen, Saarland und Sachsen sowie der Verband forschender Arzneimittelhersteller (VfA) und der Bundesverband der Pharmazeuti-schen Industrie (BPI) haben sich an der Finanzierung beteiligt.
Die Studie wurde von einem unabhängigen Forschungsteam unter Leitung von Prof. Dr. Volker Hess am Institut für Geschichte und Ethik der Medizin am Universitätsklinikum Charité in Berlin durchgeführt. Ein internationaler wissenschaftlicher Beirat von unabhängigen Experten und Exper-tinnen aus Medizin und Geschichtswissenschaft unter dem Vorsitz von Prof. Dr. Carola Sachse (Universität Wien) hat die Forschungen begleitet.
Die vorliegende Monografie zieht nach zweieinhalb Jahren intensiver historischer Aufarbeitung Bilanz:
• Für den Zeitraum von 1961 bis 1990 fanden sich Hinweise auf bis zu 900 klinische Studien, die im Auftrag von Westfirmen in der DDR durchgeführt wurden. Davon sind 321 Studien für eine genauere Analyse archivalisch hinreichend dokumentiert.
• Auftraggeber waren überwiegend branchenführende bundesdeutsche, aber auch schweizeri-sche, französische, britische und US-amerikanische Firmen. Insgesamt wurden Aufträge von 75 Firmen aus 16 westlichen Ländern nachgewiesen.
• Als Prüfzentren fungierten 120 Universitätskliniken, Bezirkskrankenhäuser und andere Einrich-tungen des Gesundheitswesens, die sich über das gesamte Gebiet der DDR verteilten.
• Die westlichen Auftragsstudien in der DDR waren überwiegend Teil von größeren multinational und multizentrisch angelegten Arzneimittelprüfungen. Die Teilstudien wurden in allen ange-schlossenen Prüfzentren, ob in der DDR oder in anderen Ländern, nach den gleichen Standards durchgeführt.
• Diese Standards entsprachen – auf beiden Seiten des Eisernen Vorhangs – nicht den heutigen Regeln, sondern wurden im Zuge der Weiterentwicklung internationaler ethischer Regeln und nationaler Gesetze nach und nach konkretisiert. Systematische Verstöße gegen die historisch jeweils geltenden Regeln konnten für die in der DDR durchgeführten Studien nicht nachgewie-sen werden. Verstöße in Einzelfällen konnten wir für die DDR nicht in einem vermehrten Ausmaß feststellen.
• Der Befund hinsichtlich der Aufklärung und Einwilligung von Patientinnen und Patienten bleibt unvollständig: In allen von uns geprüften archivierten Studienberichten sind Einverständniser-klärungen anonymisiert und listenförmig dokumentiert. Über die tatsächliche Praxis der Aufklä-rungsgespräche liegen jedoch nur vereinzelte Informationen aus Interviews mit Patient/inn/en und Prüfärzten der DDR-Kliniken sowie Monitoren der auftraggebenden Firmen vor. Sie lassen vermuten, dass zumeist den jeweils geltenden Regel und dem je historischen Verständnis des Arzt-Patientenverhältnisses entsprechend gehandelt wurde.
• Die DDR-Behörden stellten westlichen Unternehmen das Gesundheitssystem ihres Staates als Forschungseinrichtung zur Verfügung, um knappe Devisen für die eigene überschuldete Plan-wirtschaft zu erwirtschaften.
• Die Annahme, die klinischen Studien seien in der DDR besonders „billig“ zu haben gewesen, wird der komplexen Sachlage nicht gerecht. Der entscheidende betriebswirtschaftliche Vorteil auf Seiten der westlichen Auftraggeber war nicht das geringere Honorar, sondern der beträchtliche Zeit- und Effizienzgewinn, den die aufsichtführenden Behörden des diktatorischen DDR-Regimes gewährleisteten, indem sie Einzelinteressen von Prüfzentren und Prüfärzten deckelten und öffentliche Kritik ausschalteten und so für eine zügige operative Durchführung sorgten.
• Das ökonomische Ungleichgewicht bot westlichen Arzneimittelherstellern noch weitere Vorteile: Anhand einer Reihe von Einzelfällen lässt sich zeigen, wie sich Pharmafirmen gezielt das Gefäl-le der medikamentösen Versorgung zwischen Ost und West zunutze machten, um in der DDR Studiendesigns zu realisieren, die im Westen zum jeweiligen Zeitpunkt so nicht mehr durchführbar waren.
