Introduction
Une hospitalisation en psychiatrie relève de difficultés où se mêlent sentiments de honte, de peur ou de colère. Les angoisses qui circulent émanent des patients, mais aussi des soignants. Ainsi, avons-nous tenté de réaliser un travail touchant aux représentations de l’insécurité qui règne à l’hôpital psychiatrique, ceci dans une perspective clinique.
Méthode
Au total, 456 sujets dont 283 soignants et 173 non soignants ont été recrutés par mail. Ils répondaient à un auto-questionnaire disponible sur Internet pendant 3 semaines.
Résultats
Pour 67 % des sujets, il existe de l’insécurité à l’hôpital psychiatrique et pour 55 % de ces derniers, elle serait plus importante dans ces services qu’à l’extérieur de l’hôpital. La cause principale avancée est le « manque de personnel » (24 %), laquelle est suivie par la « pathologie psychiatrique » (21–23 %). Les soignants semblent ensuite mettre en cause le « manque de formation » (11 %) ; tandis que pour les non soignants ce sont les items « violence des patients » (9 %) et « services regroupant des patients avec tous types de pathologies » (6 %) qui sont évoqués juste avant la « formation des soignants » (6 %).
Discussion
Les items « manque de personnel » et « formation des soignants » apparaissent clairement autour des items « pathologies psychiatriques » et « violence des patients ». Nous y voyons une attente de rapports humains plus ajustés, plus contenants, plus à même de recevoir l’expression d’une souffrance. Être en mesure psychiquement de métaboliser ce vécu ne va pas de soi et, à l’instar de toute profession, cela s’acquiert quelles que soient les qualités individuelles de chacun.

Introduction
A psychiatric hospitalization involves difficulties that combines feelings of shame, fear or anger. The circulating of anxieties emanate from patients but also caregivers. Thus, we tried to make a work on the representations of insecurity in the psychiatric hospital.
Method
A total of 456 subjects with 283 caregivers and 173 non-caregivers, recruited by email. They completed a self-questionnaire available on the Internet during three weeks.
Results
For 67% of subjects, there is insecurity in the psychiatric hospital and for 55% of them, it would be more important in these services, that outside. The advanced main cause is the “lack of staff” (24%), and the latter is followed by the “psychiatric pathology” (21–23%). Caregivers then seem to think that the second cause is the “lack of training” (11%), while for non-caregivers, “lack of training” (6%) occurs an equivalent manner with items “violence of patients” (9%) and services having “patients with all types of diseases” (6%).
Discussion
Items lack of staff training and caregivers appear clearly around the items “psychiatric pathologies” and “violence of patients”. We see a wait of more adjusted human relationships, more containing, more likely to receive the speech and behavior of people suffering. Be able mentally to metabolize these experiences does not happen by itself, and like any profession, this have to be learned whatever the individual qualities of each.

De l’éprouvé du sentiment d’insécurité du nourrisson à celui d’une population, il semble exister, à première vue, un écart considérable. Néanmoins, il semblerait que le sentiment d’insécurité partagé par les individus soit lié à l’acquisition singulière d’un sentiment de sécurité construit au fil du temps, du berceau à l’âge adulte. Didier Robin, dans son ouvrage « Violence de l’Insécurité » met au goût du jour les aspects psychopathologiques de la construction du sentiment de sécurité dans son interdépendance avec le tissu familial et social. Ces aspects révèlent l’importance du lien humain dans l’appréhension des dangers extérieurs. En effet, que ce soit par le regard, la voix ou la psyché, l’individu est porté avant tout par sa figure d’attachement qui permet l’élaboration de sa subjectivité. L’intégration de ce socle de sécurité servira de tremplin aux innombrables intériorisations, de qualités variables, qui donnent à chacun un rapport au monde différent. Néanmoins, afin de soulager un sentiment d’insécurité, nos sociétés multiplient les dispositifs techniques de sûreté. Ceux-ci révèlent un paradoxe manifeste : si ces dispositifs ne permettent pas toujours de soulager un sentiment d’insécurité, il arrive parfois qu’ils contribuent même à le favoriser. Ces réflexions nous paraissent intéressantes en ce qui concerne le sentiment d’insécurité, inévitable, présent dans les services de soin et ce du côté des malades comme du côté soignant. En effet, il nous semble pertinent de remettre au devant de la scène l’importance de la relation soignante, parfois délaissée faute de moyen et noyée face aux exigences administratives et techniques.
Summary
The link between the sense of insecurity experienced by the infant and the one of a population seems at first sight not obvious. However, it seems that the sense insecurity shared by individuals is linked to the singular acquisition of sense of security built over time, from the cradle to adulthood. Didier Robin, in his book “Violence de l’insécurité” brings up to date the psychopathological aspects of the construction of the sense of security in its interdependence with family and social fabric. These aspects show the importance of human connection in the apprehension of external dangers. Indeed, the individual is supported primarily by its attachment figure, through its eyes, its voice or its psyche, and that allows the development of its subjectivity. The integration of this safety fundament will serve the springboard for countless internalizations, of varying quality, giving each a different link with the world. However, to relieve the feeling of insecurity, our societies respond often by installation of technical safety devices. These show a clear paradox: if they do not always relieve a feeling of insecurity, sometimes they even contribute to amplify it. These ideas seem interesting regarding the insecurity inevitable in the care services, from the side of the sick and also from the caregivers’ side. Indeed, it seems appropriate to put back in forward the patients-caregivers relationship, when it is sometimes forsaken for lack of means and drowned face by administrative and technical requirements.

Le but de cet article pourrait être de revisiter les concepts théoriques qui portent sur les aléas du travail de séparation psychique chez les adolescents gravement suicidaires en s’appuyant sur une situation clinique particulièrement éprouvante. La position « intraitable » de ce jeune patient semble en effet témoigner de la validité de l’hypothèse soutenant que la recherche active de la mort peut constituer un moyen de renouer avec un état d’indifférenciation. Tout se passe en effet comme si la disparition du sujet constituait la condition requise pour échapper aux tourments d’une conscience séparée, et récupérer ainsi, sous couvert d’un fantasme de fusion, l’amour inconditionnel des objets primaires. L’enjeu thérapeutique pourrait être d’accompagner les adolescents pris dans ce piège narcissique dans une élaboration de la honte liée à leur corps sexué et aux sentiments ambivalents éprouvés à l’égard de leurs parents, afin qu’ils soient moins en nécessité défensive de soutenir cette mortelle quête d’union symbiotique avec « l’objet primaire ». Clinicians who work with hospitalized teenagers often face some young patients’ relentless suicidal determination: these patients seem to have sentenced themselves to death. For several months, sometimes years, no one manages to revoke this incomprehensible capital punishment. This painful conviction that they are unworthy often comes with depressive affects that resist on a long-term basis, despite the combination of antidepressants and a psychotherapeutic approach. Thinking is too stressful: they long to suspend any psychic life, to stop the emergence of any desire and to end any relational impulse. They keep trying to stay in a factitious world, far from real life requirements, as a temporary alternative to death. Through the anaesthesia it produces, the use of drugs offers them a strong protection from their pain. The regressive search for a feeling of lacking psychic differentiation seems to confirm a defect of development within the first phase of the separation individuation process. Narcissistic vulnerability is often underlined in most psychopathological studies concerning suicide attempts during teenage years. Combined with the incapacity to endure any loss, this last element seems to imply that a “relationship of encroachment” exists between the subject and its environment, as well as a persistence of a broad area of lacking differentiation within the maternal imago. From this theoretical point of view, suicidal gestures from these patients are often understood as desperate and paradoxical attempts to reach their boundaries, to maintain their identity and to “withdraw themselves from the object of alienation”. These relevant interpretations hide the “search of a narcissistic state of undifferentiated union” that F. Ladame highlighted during the psychoanalytical counselling of numerous young and seriously suicidal patients. Through this article, we propose to try and re-examine the theoretical concepts related to the ups and downs of the psychic separation work needed by teenagers, particularly for those who went through serious suicide attempts. The presentation of a particularly difficult clinical situation will help us bring context to our subject. The uncompromising posture of this young patient seems to give credence to the hypothesis claiming that the active pursuit of death may be a way to reconnect with an undifferentiated state. In the case of our teenager, everything is going as if his disappearance was the only way to escape from the torments of a separate consciousness, and therefore recover the unconditional love from primary objects through the guise of a fusion fantasy. How can we guide adolescents caught in this narcissistic trap? The therapeutic challenge could be to guide them through the elaboration of the shame attached to their sexual body and of the ambivalent feelings they experience towards their parents. The aim would be to allow them be less dependent on their defensive way to support this deadly quest of symbiotic union with the “primary object”.

Méthode L’analyse du cas de Paul s’appuie en particulier sur la phénoménologie psychiatrique, la psychopathologie d’inspiration psychanalytique, et sur ce que signifie habiter dans une dimension clinique concrète. Résultats Paul souffre d’une clinique de la perte. La menace que nous éprouvons au contact de la dissociation de Paul nous indique le fait que nous sommes aussi habités par lui-même. Discussion La métabolisation de ce vécu déroutant est indispensable, d’une part, pour accompagner Paul dans les soins sans être happé par la dynamique de sa perte, et d’autre part, de manière à ne pas alimenter l’excitation délirante qui est la sienne. La manière d’habiter de Paul s’éclaire à mesure que nous saisissons ce qu’implique d’être habité par Paul. Conclusion La proximité clinique avec les patients schizophrènes, l’analyse des ressentis contre-transférentiels d’habitation par le patient dans notre propre psychisme comme dans son logement (pratique des visites à domicile), ainsi que l’implication subjective et concrète des soignants permettent une meilleure compréhension des processus psychopathologiques et un meilleur accompagnement thérapeutique. Mais cela comporte un « coût » pour les thérapeutes en termes d’engagement et de fatigue.
Abstract Aims By way of a reflection on what it means to “inhabit the world”, we attempt here to capture what it is that prevents Paul, a schizophrenic patient, from sharing a common horizon with others. Method The analysis of the case of Paul is in particular based on psychiatric phenomenology, on psychopathology of the psychoanalytic type, and on what it means to “inhabit” a concrete clinical dimension. Results Paul has a clinical profile characterised by loss. The threat we experience when in contact with Paul's dissociation becomes easier to apprehend as we realise that we are also inhabited by him. Discussion It is essential to metabolise this unsettling experience, first so as to accompany Paul in the care process without being caught up in the dynamic of his loss, and secondly to avoid fuelling his delirious excitement. Paul's mode of inhabiting becomes clearer as we realise what it means to be inhabited by him. Conclusion Clinical proximity with schizophrenic patients, the analysis of our counter-transference experiences of the patient's inhabiting of our own psyche and of his home (we conduct home visits) and the concrete, subjective implication of caregivers all enable better understanding of the psychopathological processes and better therapeutic accompaniment. This does however entail a cost for the therapists in terms of implication and fatigue.

À travers la présentation d’un cas clinique d’une adolescente née sous X, nous proposons un développement presque « en négatif » du thème des migrants. Nous verrons à travers les concepts de narcissisme, de fonctionnement « limite », les traces laissées par l’abandon, entravant le travail nécessaire de séparation à l’adolescence. Nous présenterons alors l’intérêt d’un soutien de la narrativité. Il s’agira d’intégrer l’abandon et l’adoption dans une construction filiale singulière, notamment à travers l’élaboration d’un roman familial, aussi fantasmatique soit-il.
Migration can be thought of as a distancing, separation from its origins, its roots, but what about those individuals whose personal history has been erased, born without a name, from a woman, or parents without an identity? Through the presentation of a clinical case of a teenage girl born under X, we propose to present the development which is almost “negative”, of the theme of migrants. We will see through the concepts of narcissism and a functioning “limit”, the traces left by abandonment, which implies the work that is nescessary in adolescent separation. We then present the interest of supporting the narrative. It will integrate abandonment and adoption in a sole entity, particularly through the development of a family, as fantastical as it may be.

L’objectif est d’analyser la chronicisation à l’hôpital au sein de cette population. Méthode : à partir de quatre cas cliniques issus de notre pratique clinique et d’entretiens semi-structurés avec treize patients schizophrènes, nous avons effectué une analyse clinique et psychopathologique centrée sur la compréhension phénoménologique de l’expérience de la première hospitalisation. Ce matériel verbal a fait l’objet d’une analyse à la fois qualitative et quantitative. Résultats : plusieurs facteurs tels que l’atteinte du soi minimal, de l’expérience de la temporalité, du lien aux proches, mais aussi l’expérience de la violence, participent à l’installation d’une chronicité chez ces patients. En revanche, l’hôpital semble rarement être investi comme un chez-soi. Associée à la passivité induite par la dynamique institutionnelle, l’altération majeure du soi minimal entrave l’appropriation de l’expérience chez ces patients. Discussion : la chronicité dans l’expérience schizophrénique est une réalité clinique irréductible. Cela suppose de considérer le temps existentiel et d’interroger sans cesse le cadre de soins. Le sujet étant limité dans son processus de territorialisation au moment même de l’évènement de son hospitalisation, nous relevons dans son discours un défaut de transpassibilité de celui-ci, signant sa difficulté d’appropriation et conduisant à l’incompréhension au sens évènemential de l’hospitalisation et de son issue. Conclusion : dans les schizophrénies les enjeux existentiels sont inhérents à l’expérience de l’hospitalisation. L’institution doit elle-même être soignée afin de préserver son potentiel réflexif et de maintenir une dynamique ajustée à la prise en charge des patients souffrant de schizophrénie.

En 2001, la vallée de la Somme a connu d'importantes inondations. Cette étude cherchait à évaluer le lien entre les manifestations du vécu traumatique et l'attachement à l'habitat des sinistrés. Dix sujets (âge ≈ 64 ± 8 ans ; sexe ratio = 1 ; propriétaires) ont consenti à répondre à un entretien semi-directif portant à la fois sur le vécu de l'inondation et sur l'attachement à l'habitat, puis aux auto-questionnaires suivants : Questionnaire de Styles d'Attachement (QSA) de Feeney, Noller, et Hanrahan (1994) ; Impact of Event Scale - Revised (IES-R) de Weiss et Marmar (1997) et un questionnaire de recueil de données socio-démographiques. Les passations se déroulaient au domicile des participants. Les résultats ont indiqué que pour un trauma non négligeable (n=5), on retrouvait un attachement à l'habitat considérable (n=4) ou non négligeable (n=1). Si l'attachement à l'habitat n’apparaissait pas exclusivement déterminant pour la survenue d'un trauma, en revanche il colorait nettement les manifestations du vécu traumatique. Le QSA n'a pas permis d'évaluer les liens entre le style d'attachement des individus et celui porté à leur habitat. Ces résultats ont alimenté nos connaissances sur les liens existant entre un individu et son habitat sur l'incidence de ce dernier dans le vécu traumatique lié à une inondation. Ces éléments pourraient être explorés également à travers d'autres événements traumatiques touchant l'habitat ; susceptibles d'améliorer, d'une part, notre compréhension du trauma dans des situations spécifiques, et d'autre part de mettre en évidence des données complémentaires concernant l'attachement à l'habitat.

Dès lors, une réflexion sur le soin psychique peut s'amorcer. Paul souffre de schizophrénie et nous confronte à certaines difficultés émanant de son expression dissociative, mais également de l'incompréhension avec laquelle nous sommes traversés par cette dissociation. Il s'agit alors de soumettre une réflexion sur l'engagement clinique nécessaire et suffisant pour accompagner Paul dans la réappropriation d'un monde qui lui échappe. Méthode : dans un premier temps, l'analyse psychopathologique permet de cerner les contours d'une problématique complexe qui concerne l'être-au-monde de Paul, dans la perte de sa capacité à habiter le monde. À travers une réflexion sur ce qu'implique habiter le monde du point de vue de la phénoménologie psychiatrique, sur la clinique schizophrénique et enfin sur ce que signifie habiter dans une dimension clinique concrète, nous tentons ici de saisir ce qui empêche Paul de se tenir dans un horizon commun avec ses semblables. Résultats : Paul souffre d'une clinique de la perte l'ayant conduit à la dilution de son être, lâché par les amarres qui le tiennent à la réalité. La menace que nous éprouvons au contact de la dissociation de Paul nous indique le fait d'être habité par lui-même. La métabolisation de ce vécu déroutant est indispensable, d'une part, pour accompagner Paul dans les soins sans être happé par la dynamique de sa perte, et d'autre part, de manière à ne pas alimenter l'excitation délirante qui est la sienne. Discussion : le cheminement de Paul se réalise en parallèle au nôtre. La manière d'habiter de Paul s'éclaire à mesure que nous saisissons ce qu'implique d'être habité par Paul. Cette proximité clinique permet d'établir un lien suffisamment solide pour descendre avec Paul dans l'enfer de son intimité, par exemple au moment des visites à domicile. Conclusion : ensemble, nous percevons l'incapacité pour Paul de continuer d'habiter sa maison. L'expérience commune de ce constat permet alors à Paul de travailler ses possibilités de réinvestissement des perspectives d'habiter, fondées sur le sens commun. La phénoménologie de l'habiter s'avère un outil intéressant pour éclairer la clinique psychiatrique et la psychopathologie.

Indeed, the individual is supported primarily by its attachment figure, through its eyes, its voice or its psyche, and that allows the development of its subjectivity. The integration of this safety fundament will serve the springboard for countless internalizations, of varying quality, giving each a different link with the world. However, to relieve the feeling of insecurity, our societies respond often by installation of technical safety devices. These show a clear paradox: if they do not always relieve a feeling of insecurity, sometimes they even contribute to amplify it. These ideas seem interesting regarding the insecurity inevitable in the care services, from the side of the sick and also from the caregivers’ side. Indeed, it seems appropriate to put back in forward the patients-caregivers relationship, when it is sometimes forsaken for lack of means and drowned face by administrative and technical requirements.

Le but de cet article pourrait être de revisiter les concepts théoriques qui portent sur les aléas du travail de séparation psychique chez les adolescents gravement suicidaires en s’appuyant sur une situation clinique particulièrement éprouvante. La position « intraitable » de ce jeune patient semble en effet témoigner de la validité de l’hypothèse soutenant que la recherche active de la mort peut constituer un moyen de renouer avec un état d’indifférenciation. Tout se passe en effet comme si la disparition du sujet constituait la condition requise pour échapper aux tourments d’une conscience séparée, et récupérer ainsi, sous couvert d’un fantasme de fusion, l’amour inconditionnel des objets primaires. L’enjeu thérapeutique pourrait être d’accompagner les adolescents pris dans ce piège narcissique dans une élaboration de la honte liée à leur corps sexué et aux sentiments ambivalents éprouvés à l’égard de leurs parents, afin qu’ils soient moins en nécessité défensive de soutenir cette mortelle quête d’union symbiotique avec « l’objet primaire ». Clinicians who work with hospitalized teenagers often face some young patients’ relentless suicidal determination: these patients seem to have sentenced themselves to death. For several months, sometimes years, no one manages to revoke this incomprehensible capital punishment. This painful conviction that they are unworthy often comes with depressive affects that resist on a long-term basis, despite the combination of antidepressants and a psychotherapeutic approach. Thinking is too stressful: they long to suspend any psychic life, to stop the emergence of any desire and to end any relational impulse. They keep trying to stay in a factitious world, far from real life requirements, as a temporary alternative to death. Through the anaesthesia it produces, the use of drugs offers them a strong protection from their pain. The regressive search for a feeling of lacking psychic differentiation seems to confirm a defect of development within the first phase of the separation individuation process. Narcissistic vulnerability is often underlined in most psychopathological studies concerning suicide attempts during teenage years. Combined with the incapacity to endure any loss, this last element seems to imply that a “relationship of encroachment” exists between the subject and its environment, as well as a persistence of a broad area of lacking differentiation within the maternal imago. From this theoretical point of view, suicidal gestures from these patients are often understood as desperate and paradoxical attempts to reach their boundaries, to maintain their identity and to “withdraw themselves from the object of alienation”. These relevant interpretations hide the “search of a narcissistic state of undifferentiated union” that F. Ladame highlighted during the psychoanalytical counselling of numerous young and seriously suicidal patients. Through this article, we propose to try and re-examine the theoretical concepts related to the ups and downs of the psychic separation work needed by teenagers, particularly for those who went through serious suicide attempts. The presentation of a particularly difficult clinical situation will help us bring context to our subject. The uncompromising posture of this young patient seems to give credence to the hypothesis claiming that the active pursuit of death may be a way to reconnect with an undifferentiated state. In the case of our teenager, everything is going as if his disappearance was the only way to escape from the torments of a separate consciousness, and therefore recover the unconditional love from primary objects through the guise of a fusion fantasy. How can we guide adolescents caught in this narcissistic trap? The therapeutic challenge could be to guide them through the elaboration of the shame attached to their sexual body and of the ambivalent feelings they experience towards their parents. The aim would be to allow them be less dependent on their defensive way to support this deadly quest of symbiotic union with the “primary object”.

Abstract Aims By way of a reflection on what it means to “inhabit the world”, we attempt here to capture what it is that prevents Paul, a schizophrenic patient, from sharing a common horizon with others. Method The analysis of the case of Paul is in particular based on psychiatric phenomenology, on psychopathology of the psychoanalytic type, and on what it means to “inhabit” a concrete clinical dimension. Results Paul has a clinical profile characterised by loss. The threat we experience when in contact with Paul's dissociation becomes easier to apprehend as we realise that we are also inhabited by him. Discussion It is essential to metabolise this unsettling experience, first so as to accompany Paul in the care process without being caught up in the dynamic of his loss, and secondly to avoid fuelling his delirious excitement. Paul's mode of inhabiting becomes clearer as we realise what it means to be inhabited by him. Conclusion Clinical proximity with schizophrenic patients, the analysis of our counter-transference experiences of the patient's inhabiting of our own psyche and of his home (we conduct home visits) and the concrete, subjective implication of caregivers all enable better understanding of the psychopathological processes and better therapeutic accompaniment. This does however entail a cost for the therapists in terms of implication and fatigue.

À travers la présentation d’un cas clinique d’une adolescente née sous X, nous proposons un développement presque « en négatif » du thème des migrants. Nous verrons à travers les concepts de narcissisme, de fonctionnement « limite », les traces laissées par l’abandon, entravant le travail nécessaire de séparation à l’adolescence. Nous présenterons alors l’intérêt d’un soutien de la narrativité. Il s’agira d’intégrer l’abandon et l’adoption dans une construction filiale singulière, notamment à travers l’élaboration d’un roman familial, aussi fantasmatique soit-il.
Migration can be thought of as a distancing, separation from its origins, its roots, but what about those individuals whose personal history has been erased, born without a name, from a woman, or parents without an identity? Through the presentation of a clinical case of a teenage girl born under X, we propose to present the development which is almost “negative”, of the theme of migrants. We will see through the concepts of narcissism and a functioning “limit”, the traces left by abandonment, which implies the work that is nescessary in adolescent separation. We then present the interest of supporting the narrative. It will integrate abandonment and adoption in a sole entity, particularly through the development of a family, as fantastical as it may be.

L’objectif est d’analyser la chronicisation à l’hôpital au sein de cette population. Méthode : à partir de quatre cas cliniques issus de notre pratique clinique et d’entretiens semi-structurés avec treize patients schizophrènes, nous avons effectué une analyse clinique et psychopathologique centrée sur la compréhension phénoménologique de l’expérience de la première hospitalisation. Ce matériel verbal a fait l’objet d’une analyse à la fois qualitative et quantitative. Résultats : plusieurs facteurs tels que l’atteinte du soi minimal, de l’expérience de la temporalité, du lien aux proches, mais aussi l’expérience de la violence, participent à l’installation d’une chronicité chez ces patients. En revanche, l’hôpital semble rarement être investi comme un chez-soi. Associée à la passivité induite par la dynamique institutionnelle, l’altération majeure du soi minimal entrave l’appropriation de l’expérience chez ces patients. Discussion : la chronicité dans l’expérience schizophrénique est une réalité clinique irréductible. Cela suppose de considérer le temps existentiel et d’interroger sans cesse le cadre de soins. Le sujet étant limité dans son processus de territorialisation au moment même de l’évènement de son hospitalisation, nous relevons dans son discours un défaut de transpassibilité de celui-ci, signant sa difficulté d’appropriation et conduisant à l’incompréhension au sens évènemential de l’hospitalisation et de son issue. Conclusion : dans les schizophrénies les enjeux existentiels sont inhérents à l’expérience de l’hospitalisation. L’institution doit elle-même être soignée afin de préserver son potentiel réflexif et de maintenir une dynamique ajustée à la prise en charge des patients souffrant de schizophrénie.

En 2001, la vallée de la Somme a connu d'importantes inondations. Cette étude cherchait à évaluer le lien entre les manifestations du vécu traumatique et l'attachement à l'habitat des sinistrés. Dix sujets (âge ≈ 64 ± 8 ans ; sexe ratio = 1 ; propriétaires) ont consenti à répondre à un entretien semi-directif portant à la fois sur le vécu de l'inondation et sur l'attachement à l'habitat, puis aux auto-questionnaires suivants : Questionnaire de Styles d'Attachement (QSA) de Feeney, Noller, et Hanrahan (1994) ; Impact of Event Scale - Revised (IES-R) de Weiss et Marmar (1997) et un questionnaire de recueil de données socio-démographiques. Les passations se déroulaient au domicile des participants. Les résultats ont indiqué que pour un trauma non négligeable (n=5), on retrouvait un attachement à l'habitat considérable (n=4) ou non négligeable (n=1). Si l'attachement à l'habitat n’apparaissait pas exclusivement déterminant pour la survenue d'un trauma, en revanche il colorait nettement les manifestations du vécu traumatique. Le QSA n'a pas permis d'évaluer les liens entre le style d'attachement des individus et celui porté à leur habitat. Ces résultats ont alimenté nos connaissances sur les liens existant entre un individu et son habitat sur l'incidence de ce dernier dans le vécu traumatique lié à une inondation. Ces éléments pourraient être explorés également à travers d'autres événements traumatiques touchant l'habitat ; susceptibles d'améliorer, d'une part, notre compréhension du trauma dans des situations spécifiques, et d'autre part de mettre en évidence des données complémentaires concernant l'attachement à l'habitat.

Il s'agit alors de soumettre une réflexion sur l'engagement clinique nécessaire et suffisant pour accompagner Paul dans la réappropriation d'un monde qui lui échappe. Méthode : dans un premier temps, l'analyse psychopathologique permet de cerner les contours d'une problématique complexe qui concerne l'être-au-monde de Paul, dans la perte de sa capacité à habiter le monde. À travers une réflexion sur ce qu'implique habiter le monde du point de vue de la phénoménologie psychiatrique, sur la clinique schizophrénique et enfin sur ce que signifie habiter dans une dimension clinique concrète, nous tentons ici de saisir ce qui empêche Paul de se tenir dans un horizon commun avec ses semblables. Conclusion : ensemble, nous percevons l'incapacité pour Paul de continuer d'habiter sa maison. L'expérience commune de ce constat permet alors à Paul de travailler ses possibilités de réinvestissement des perspectives d'habiter, fondées sur le sens commun. La phénoménologie de l'habiter s'avère un outil intéressant pour éclairer la clinique psychiatrique et la psychopathologie.

l’habiter en considérant la psychopathologie des schizophrénies. Les schizophrénies sont des troubles qui touchent à la façon d’habiter le monde. Ainsi pour ces personnes souffrant de schizophrénie ne va pas de soi. En conséquence, elles nécessitent un accompagnement social, qui suppose de porter une attention toute particulière à la compréhension de leur manière d’Être.
Introduction
Une hospitalisation en psychiatrie relève de difficultés où se mêlent sentiments de honte, de peur ou de colère. Les angoisses qui circulent émanent des patients, mais aussi des soignants. Ainsi, avons-nous tenté de réaliser un travail touchant aux représentations de l’insécurité qui règne à l’hôpital psychiatrique, ceci dans une perspective clinique.
Méthode
Au total, 456 sujets dont 283 soignants et 173 non soignants ont été recrutés par mail. Ils répondaient à un auto-questionnaire disponible sur Internet pendant 3 semaines.
Résultats
Pour 67 % des sujets, il existe de l’insécurité à l’hôpital psychiatrique et pour 55 % de ces derniers, elle serait plus importante dans ces services qu’à l’extérieur de l’hôpital. La cause principale avancée est le « manque de personnel » (24 %), laquelle est suivie par la « pathologie psychiatrique » (21–23 %). Les soignants semblent ensuite mettre en cause le « manque de formation » (11 %) ; tandis que pour les non soignants ce sont les items « violence des patients » (9 %) et « services regroupant des patients avec tous types de pathologies » (6 %) qui sont évoqués juste avant la « formation des soignants » (6 %).
Discussion
Les items « manque de personnel » et « formation des soignants » apparaissent clairement autour des items « pathologies psychiatriques » et « violence des patients ». Nous y voyons une attente de rapports humains plus ajustés, plus contenants, plus à même de recevoir l’expression d’une souffrance. Être en mesure psychiquement de métaboliser ce vécu ne va pas de soi et, à l’instar de toute profession, cela s’acquiert quelles que soient les qualités individuelles de chacun.

Introduction
A psychiatric hospitalization involves difficulties that combines feelings of shame, fear or anger. The circulating of anxieties emanate from patients but also caregivers. Thus, we tried to make a work on the representations of insecurity in the psychiatric hospital.
Method
A total of 456 subjects with 283 caregivers and 173 non-caregivers, recruited by email. They completed a self-questionnaire available on the Internet during three weeks.
Results
For 67% of subjects, there is insecurity in the psychiatric hospital and for 55% of them, it would be more important in these services, that outside. The advanced main cause is the “lack of staff” (24%), and the latter is followed by the “psychiatric pathology” (21–23%). Caregivers then seem to think that the second cause is the “lack of training” (11%), while for non-caregivers, “lack of training” (6%) occurs an equivalent manner with items “violence of patients” (9%) and services having “patients with all types of diseases” (6%).
Discussion
Items lack of staff training and caregivers appear clearly around the items “psychiatric pathologies” and “violence of patients”. We see a wait of more adjusted human relationships, more containing, more likely to receive the speech and behavior of people suffering. Be able mentally to metabolize these experiences does not happen by itself, and like any profession, this have to be learned whatever the individual qualities of each.
De l’éprouvé du sentiment d’insécurité du nourrisson à celui d’une population, il semble exister, à première vue, un écart considérable. Néanmoins, il semblerait que le sentiment d’insécurité partagé par les individus soit lié à l’acquisition singulière d’un sentiment de sécurité construit au fil du temps, du berceau à l’âge adulte. Didier Robin, dans son ouvrage « Violence de l’Insécurité » met au goût du jour les aspects psychopathologiques de la construction du sentiment de sécurité dans son interdépendance avec le tissu familial et social. Ces aspects révèlent l’importance du lien humain dans l’appréhension des dangers extérieurs. En effet, que ce soit par le regard, la voix ou la psyché, l’individu est porté avant tout par sa figure d’attachement qui permet l’élaboration de sa subjectivité. L’intégration de ce socle de sécurité servira de tremplin aux innombrables intériorisations, de qualités variables, qui donnent à chacun un rapport au monde différent. Néanmoins, afin de soulager un sentiment d’insécurité, nos sociétés multiplient les dispositifs techniques de sûreté. Ceux-ci révèlent un paradoxe manifeste : si ces dispositifs ne permettent pas toujours de soulager un sentiment d’insécurité, il arrive parfois qu’ils contribuent même à le favoriser. Ces réflexions nous paraissent intéressantes en ce qui concerne le sentiment d’insécurité, inévitable, présent dans les services de soin et ce du côté des malades comme du côté soignant. En effet, il nous semble pertinent de remettre au devant de la scène l’importance de la relation soignante, parfois délaissée faute de moyen et noyée face aux exigences administratives et techniques.

The link between the sense of insecurity experienced by the infant and the one of a population seems at first sight not obvious. However, it seems that the sense insecurity shared by individuals is linked to the singular acquisition of sense of security built over time, from the cradle to adulthood. Didier Robin, in his book “Violence de l’insécurité” brings up to date the psychopathological aspects of the construction of the sense of security in its interdependence with family and social fabric. These aspects show the importance of human connection in the apprehension of external dangers. Clinical proximity with schizophrenic patients, the analysis of our counter-transference experiences of the patient's inhabiting of our own psyche and of his home (we conduct home visits) and the concrete, subjective implication of caregivers all enable better understanding of the psychopathological processes and better therapeutic accompaniment. This does however entail a cost for the therapists in terms of implication and fatigue.
Migration can be thought of as a distancing, separation from its origins, its roots, but what about those individuals whose personal history has been erased, born without a name, from a woman, or parents without an identity? Through the presentation of a clinical case of a teenage girl born under X, we propose to present the development which is almost “negative”, of the theme of migrants. We will see through the concepts of narcissism and a functioning “limit”, the traces left by abandonment, which implies the work that is nescessary in adolescent separation. We then present the interest of supporting the narrative. It will integrate abandonment and adoption in a sole entity, particularly through the development of a family, as fantastical as it may be.

En 2001, la vallée de la Somme a connu d'importantes inondations. Cette étude cherchait à évaluer le lien entre les manifestations du vécu traumatique et l'attachement à l'habitat des sinistrés. Dix sujets (âge ≈ 64 ± 8 ans ; sexe ratio = 1 ; propriétaires) ont consenti à répondre à un entretien semi-directif portant à la fois sur le vécu de l'inondation et sur l'attachement à l'habitat, puis aux auto-questionnaires suivants : Questionnaire de Styles d'Attachement (QSA) de Feeney, Noller, et Hanrahan (1994) ; Impact of Event Scale - Revised (IES-R) de Weiss et Marmar (1997) et un questionnaire de recueil de données socio-démographiques. Les résultats ont indiqué que pour un trauma non négligeable (n=5), on retrouvait un attachement à l'habitat considérable (n=4) ou non négligeable (n=1). La menace que nous éprouvons au contact de la dissociation de Paul nous indique le fait d'être habité par lui-même. La métabolisation de ce vécu déroutant est indispensable, d'une part, pour accompagner Paul dans les soins sans être happé par la dynamique de sa perte, et d'autre part, de manière à ne pas alimenter l'excitation délirante qui est la sienne. Discussion : le cheminement de Paul se réalise en parallèle au nôtre. La manière d'habiter de Paul s'éclaire à mesure que nous saisissons ce qu'implique d'être habité par Paul. Cette proximité clinique permet d'établir un lien suffisamment solide pour descendre avec Paul dans l'enfer de son intimité, par exemple au moment des visites à domicile. Conclusion : ensemble, nous percevons l'incapacité pour Paul de continuer d'habiter sa maison. L'expérience commune de ce constat permet alors à Paul de travailler ses possibilités de réinvestissement des perspectives d'habiter, fondées sur le sens commun. La phénoménologie de l'habiter s'avère un outil intéressant pour éclairer la clinique psychiatrique et la psychopathologie.

Ainsi pour ces personnes souffrant de schizophrénie ne va pas de soi. En conséquence, elles nécessitent un accompagnement social, qui suppose de porter une attention toute particulière à la compréhension de leur manière d’Être.

Une hospitalisation en psychiatrie relève de difficultés où se mêlent sentiments de honte, de peur ou de colère. Les angoisses qui circulent émanent des patients, mais aussi des soignants. Ainsi, avons-nous tenté de réaliser un travail touchant aux représentations de l’insécurité qui règne à l’hôpital psychiatrique, ceci dans une perspective clinique.
Méthode
Au total, 456 sujets dont 283 soignants et 173 non soignants ont été recrutés par mail. Ils répondaient à un auto-questionnaire disponible sur Internet pendant 3 semaines.
Résultats
Pour 67 % des sujets, il existe de l’insécurité à l’hôpital psychiatrique et pour 55 % de ces derniers, elle serait plus importante dans ces services qu’à l’extérieur de l’hôpital. La cause principale avancée est le « manque de personnel » (24 %), laquelle est suivie par la « pathologie psychiatrique » (21–23 %). Les soignants semblent ensuite mettre en cause le « manque de formation » (11 %) ; tandis que pour les non soignants ce sont les items « violence des patients » (9 %) et « services regroupant des patients avec tous types de pathologies » (6 %) qui sont évoqués juste avant la « formation des soignants » (6 %).
Discussion
Les items « manque de personnel » et « formation des soignants » apparaissent clairement autour des items « pathologies psychiatriques » et « violence des patients ». Nous y voyons une attente de rapports humains plus ajustés, plus contenants, plus à même de recevoir l’expression d’une souffrance. Être en mesure psychiquement de métaboliser ce vécu ne va pas de soi et, à l’instar de toute profession, cela s’acquiert quelles que soient les qualités individuelles de chacun.

Introduction
A psychiatric hospitalization involves difficulties that combines feelings of shame, fear or anger. The circulating of anxieties emanate from patients but also caregivers. Thus, we tried to make a work on the representations of insecurity in the psychiatric hospital.
Method
A total of 456 subjects with 283 caregivers and 173 non-caregivers, recruited by email. They completed a self-questionnaire available on the Internet during three weeks.
Results
For 67% of subjects, there is insecurity in the psychiatric hospital and for 55% of them, it would be more important in these services, that outside. The advanced main cause is the “lack of staff” (24%), and the latter is followed by the “psychiatric pathology” (21–23%). Caregivers then seem to think that the second cause is the “lack of training” (11%), while for non-caregivers, “lack of training” (6%) occurs an equivalent manner with items “violence of patients” (9%) and services having “patients with all types of diseases” (6%).
Discussion
Items lack of staff training and caregivers appear clearly around the items “psychiatric pathologies” and “violence of patients”. We see a wait of more adjusted human relationships, more containing, more likely to receive the speech and behavior of people suffering. Be able mentally to metabolize these experiences does not happen by itself, and like any profession, this have to be learned whatever the individual qualities of each.
De l’éprouvé du sentiment d’insécurité du nourrisson à celui d’une population, il semble exister, à première vue, un écart considérable. Néanmoins, il semblerait que le sentiment d’insécurité partagé par les individus soit lié à l’acquisition singulière d’un sentiment de sécurité construit au fil du temps, du berceau à l’âge adulte. Didier Robin, dans son ouvrage « Violence de l’Insécurité » met au goût du jour les aspects psychopathologiques de la construction du sentiment de sécurité dans son interdépendance avec le tissu familial et social. Ces aspects révèlent l’importance du lien humain dans l’appréhension des dangers extérieurs. En effet, que ce soit par le regard, la voix ou la psyché, l’individu est porté avant tout par sa figure d’attachement qui permet l’élaboration de sa subjectivité. L’intégration de ce socle de sécurité servira de tremplin aux innombrables intériorisations, de qualités variables, qui donnent à chacun un rapport au monde différent. Néanmoins, afin de soulager un sentiment d’insécurité, nos sociétés multiplient les dispositifs techniques de sûreté. Ceux-ci révèlent un paradoxe manifeste : si ces dispositifs ne permettent pas toujours de soulager un sentiment d’insécurité, il arrive parfois qu’ils contribuent même à le favoriser. Ces réflexions nous paraissent intéressantes en ce qui concerne le sentiment d’insécurité, inévitable, présent dans les services de soin et ce du côté des malades comme du côté soignant. En effet, il nous semble pertinent de remettre au devant de la scène l’importance de la relation soignante, parfois délaissée faute de moyen et noyée face aux exigences administratives et techniques.
Summary
The link between the sense of insecurity experienced by the infant and the one of a population seems at first sight not obvious. However, it seems that the sense insecurity shared by individuals is linked to the singular acquisition of sense of security built over time, from the cradle to adulthood. Didier Robin, in his book “Violence de l’insécurité” brings up to date the psychopathological aspects of the construction of the sense of security in its interdependence with family and social fabric. These aspects show the importance of human connection in the apprehension of external dangers. Indeed, the individual is supported primarily by its attachment figure, through its eyes, its voice or its psyche, and that allows the development of its subjectivity. The integration of this safety fundament will serve the springboard for countless internalizations, of varying quality, giving each a different link with the world. However, to relieve the feeling of insecurity, our societies respond often by installation of technical safety devices. These show a clear paradox: if they do not always relieve a feeling of insecurity, sometimes they even contribute to amplify it. These ideas seem interesting regarding the insecurity inevitable in the care services, from the side of the sick and also from the caregivers’ side. Indeed, it seems appropriate to put back in forward the patients-caregivers relationship, when it is sometimes forsaken for lack of means and drowned face by administrative and technical requirements.
Le but de cet article pourrait être de revisiter les concepts théoriques qui portent sur les aléas du travail de séparation psychique chez les adolescents gravement suicidaires en s’appuyant sur une situation clinique particulièrement éprouvante. La position « intraitable » de ce jeune patient semble en effet témoigner de la validité de l’hypothèse soutenant que la recherche active de la mort peut constituer un moyen de renouer avec un état d’indifférenciation. Tout se passe en effet comme si la disparition du sujet constituait la condition requise pour échapper aux tourments d’une conscience séparée, et récupérer ainsi, sous couvert d’un fantasme de fusion, l’amour inconditionnel des objets primaires. L’enjeu thérapeutique pourrait être d’accompagner les adolescents pris dans ce piège narcissique dans une élaboration de la honte liée à leur corps sexué et aux sentiments ambivalents éprouvés à l’égard de leurs parents, afin qu’ils soient moins en nécessité défensive de soutenir cette mortelle quête d’union symbiotique avec « l’objet primaire ».
Abstract
Clinicians who work with hospitalized teenagers often face some young patients’ relentless suicidal determination: these patients seem to have sentenced themselves to death. For several months, sometimes years, no one manages to revoke this incomprehensible capital punishment. This painful conviction that they are unworthy often comes with depressive affects that resist on a long-term basis, despite the combination of antidepressants and a psychotherapeutic approach. Thinking is too stressful: they long to suspend any psychic life, to stop the emergence of any desire and to end any relational impulse. They keep trying to stay in a factitious world, far from real life requirements, as a temporary alternative to death. Through the anaesthesia it produces, the use of drugs offers them a strong protection from their pain. The regressive search for a feeling of lacking psychic differentiation seems to confirm a defect of development within the first phase of the separation individuation process. Narcissistic vulnerability is often underlined in most psychopathological studies concerning suicide attempts during teenage years. Combined with the incapacity to endure any loss, this last element seems to imply that a “relationship of encroachment” exists between the subject and its environment, as well as a persistence of a broad area of lacking differentiation within the maternal imago. From this theoretical point of view, suicidal gestures from these patients are often understood as desperate and paradoxical attempts to reach their boundaries, to maintain their identity and to “withdraw themselves from the object of alienation”. These relevant interpretations hide the “search of a narcissistic state of undifferentiated union” that F. Ladame highlighted during the psychoanalytical counselling of numerous young and seriously suicidal patients. Through this article, we propose to try and re-examine the theoretical concepts related to the ups and downs of the psychic separation work needed by teenagers, particularly for those who went through serious suicide attempts. The presentation of a particularly difficult clinical situation will help us bring context to our subject. The uncompromising posture of this young patient seems to give credence to the hypothesis claiming that the active pursuit of death may be a way to reconnect with an undifferentiated state. In the case of our teenager, everything is going as if his disappearance was the only way to escape from the torments of a separate consciousness, and therefore recover the unconditional love from primary objects through the guise of a fusion fantasy. How can we guide adolescents caught in this narcissistic trap? The therapeutic challenge could be to guide them through the elaboration of the shame attached to their sexual body and of the ambivalent feelings they experience towards their parents. The aim would be to allow them be less dependent on their defensive way to support this deadly quest of symbiotic union with the “primary object”.

By way of a reflection on what it means to “inhabit the world”, we attempt here to capture what it is that prevents Paul, a schizophrenic patient, from sharing a common horizon with others.
Method
The analysis of the case of Paul is in particular based on psychiatric phenomenology, on psychopathology of the psychoanalytic type, and on what it means to “inhabit” a concrete clinical dimension.
Results
Paul has a clinical profile characterised by loss. The threat we experience when in contact with Paul's dissociation becomes easier to apprehend as we realise that we are also inhabited by him.
Discussion
It is essential to metabolise this unsettling experience, first so as to accompany Paul in the care process without being caught up in the dynamic of his loss, and secondly to avoid fuelling his delirious excitement. Paul's mode of inhabiting becomes clearer as we realise what it means to be inhabited by him.
