biihead – Underlined page headings

Defines a \pagestyle{biiheadings} in which the heading is underlined, and contains chapter and section numbers and titles. (Thus, of course, it only works in report and book classes.

The package was written for LaTeX 2.09, presumably before the fancyheadings family had become available.
