Research on Strategic Decision-making of Government, Medical Institutions and Patients in Dual-referral System Based on Multi-party Evolutionary Game

    Background: Dual referral is an important breakthrough for guiding patients in seeking medical treatment and efficiently allocating resources. However, the conflict of interest among the parties involved has caused the current dual-referral system to not be effectively implemented in China. Methods: According to the payment matrix of different strategies used by the government, hospitals, primary medical institutions, and patients in the dual-referral system, we constructed an evolutionary game model of four participants on the micro-level. We analyzed the dynamic path of evolution of participants’ strategic behavior and an evolutionarily stable strategy using the replicator dynamics function. Additionally, we used MATLAB for a numerical simulation. Results: The government's financial allocation determines its regulatory strategy; government supervision can promote hospitals implementing the dual-referral, and the willingness of primary medical institutions to cooperate with hospitals will also increase. The implementation of a dual-referral system influences patients’ healthcare service choices towards the primary level. Conclusions: It is necessary to accurately grasp the profit generation logic and the evolution mechanism of the strategies between participants. Additionally, the government should play a positive guiding and supervisory role and should promote doctor–patient decision-making to facilitate the implementation of a dual-referral system.
