Background: Unintentional injuries in childhood constitute a significant public health problem. O... more Background: Unintentional injuries in childhood constitute a significant public health problem. Our purpose is to estimate the incidence of identifiable unintentional childhood injuries of any type and severity, and to document risk factors of non-transient nature. Methods: We have undertaken a prospective population-based investigation in a Greek town with a population of 748 children (0–14 years old). All identifiable injuries have been monitored during a twelve-month period through information provided by the health care outlets or educational institutions as well as the police station and the regional hospital. Results: The overall incidence was 28.2 per 100 person-years (95 % confidence interval from 24.4 to 32.0), whereas the incidence of injuries with Hopkins Injury Severity Score equal to or higher than four was 6.3 with 95 % confidence interval 4.5 to 8.1. They primarily concern osteosarcoma, Ewing’s sarcoma, bone lymphoma and bone metastases. Generally, primary malignant bone tumors have osteosclerotic, osteolytic or mixed radiological imaging features, they disrupt the bone cortex and cause periosteal reaction, while they often expand to the adjacent soft tissues. They may affect local and distant lymph nodes and cause metastatic disease extending hematogenously mainly to the lungs. The prognosis depends on the histological identity, the size and location of the lesion and the presence of metastases on diagnosis. Nevertheless, the introduction of modern chemotherapy regimens and the wide excision of the lesions have recently improved survival, creating new paths to a more molecular approach and the development of therapies based on the molecular profile of these tumors. Trans-catheter aortic valve implantation (TAVI) has become an established and effective alternative therapeutical procedure for inoperable and high-risk patients with symptomatic AS. The procedural success is greatly dependent on a thorough pre-TAVI imaging screening. This requires a comprehensive and multi-modality approach, in which multi-slice computed tomography (MSCT) is the cornerstone in the selection of eligible patients, in choosing the appropriate prosthesis and size, and in mapping the safest access route for the intervention. From our experience of more than 400 TAVI procedures and many more MSCTs for screening purposes, we provide clinical and technical details on the use of MSCT pre-TAVI and brief review of the knowledge so far.
Patients with moderate to severe hidradenitis suppurativa failing adalimumab therapy, or those in... more Patients with moderate to severe hidradenitis suppurativa failing adalimumab therapy, or those ineligible to receive it, remain a population with an unmet need. Twenty patients not eligible for adalimumab were randomized to receive 12 weeks of blind treatment with placebo or MABp1, a true human antibody targeting IL-1α. Hidradenitis suppurativa clinical response score at week 12 was the primary endpoint. The primary endpoint was met in 10% and 60% of placebo- and MABp1-treated patients, respectively (odds ratio = 13.50, 95% confidence interval = 1.19-152.51). Clinical efficacy was maintained at 24 weeks in 0% and 40%. Improvement in the visual analog scale was reported by 20% and 85.7%, respectively, of patients failing previous anti-TNF treatment. Ultrasonography showed decreased neovascularization and lesion skin depth in the MABp1 group. Review of data from a large injury database. The Emergency Department Injury Surveillance System in Greece. A total of 2672 injured infants. Specially trained health visitors performed in-person interviews with the children's guardians, using a precoded questionnaire. The results of an independent survey of 777 mothers of noninjured children younger than 2 years attending the same emergency departments were used to allow quantification of the role of specific nursery equipment in the causation of infant fall injuries. Annual rate of injury by falling in infants, overall and by cause. About 4400 infant fall injuries occur annually in Greece, corresponding to an annual incidence rate of 44 injuries per 1000 infants. The incidence of falls increases with increasing infant age. A high percentage of severe injuries was detected, most of them concussions (14.3%) and fractures (9.4%). Approximately 10% of infants with fall-related injuries required hospitalization. More than 36% of fall injuries involved nursery equipment. Infant walker use was associated with a higher incidence of falls (about 9 per 1000 infant-years), and these falls occasionally involved stairs and caused serious injuries. Infant bouncers, strollers, and changing tables were all associated with a similar incidence of falls (about 4 per 1000 infant-years). Falls are a common cause of serious infant injuries, and nursery equipment is frequently involved in the injury-causing event.
