The clinical picture of dengue is characterized by a maximum duration of 14 days despite frequent complaints of longer symptoms. This study evaluated the occurrence of persistent symptoms (> 14 days) and its impact on daily life. A hundred eighteen patients were interviewed, and the main symptoms at diagnosis were mialgia (98.3%), fever (97.5%) and weakness (95.8%). The presence of at least a persistent symptom was related by 77 (65.2%) individuals of wich 10 (8.5%) described it as intense and lasting for 30 days or more. The most persistent symptoms mentioned were weakness (58 cases), hiporexia (49) and sleepiness (40), occurring mostly in women, with odds ratio: 5.4 (IC95%: 2.3-12.3). A significant association between the persistence of the symptoms and the history of extra expenses (p = 0,02) was found, as well as a delay to return to normal activities (p < 0.001). Thus, it was verified that dengue presented a relevant impact on every day life, even after 14 days, a fact wich was associated with the presence of persistent symptoms of the illness.

Response to treatment with antimonial drugs varies considerably depending on the parasite strain involved, immune status of the patient and clinical form of the disease. Therapeutic regimens with this first line drug have been frequently modified both, in dose and duration of therapy. A regimen of 20 mg/kg/day of pentavalent antimony (Sb5+) during four weeks without an upper limit on the daily dose is currently recommended for mucosal disease (&amp;amp;amp;amp;amp;amp;amp;quot;espundia&amp;amp;amp;amp;amp;amp;amp;quot;). Side-effects with this dose are more marked in elderly patients, more commonly affected by this form of leishmaniasis. According to our experience, leishmaniasis in Rio de Janeiro responds well to antimony and, in cutaneous disease, high cure rates are obtained with 5 mg/kg/day of Sb5+ during 30 to 45-days. In this study a high rate of cure (91.4%) employing this dose was achieved in 36 patients with mild disease in this same geographic region. Side-effects were reduced and no antimony refractoriness was noted with subsequent use of larger dose in patients that failed to respond to initial schedule.

This research aimed to investigate whether mofebutazone causes gastrointestinal and renal side effects in healthy ponies as well as to verify the capacity of omeprazole to inhibit the genesis of gastric ulcers. The experiment was carried out in two phases. In the first, six ponies were used, with three of them being treated daily with different doses (3, 4.5 and 6mg kg-1) of intravenous (IV) mofebutazone for 12 days. The other ponies were given 3mg kg-1 of omeprazole in addition to the anti-inflammatory drug. In the second phase, four ponies were included, with two of them being treated daily with 4.5mg kg-1 of mofebutazone for 12 days and the two remainders with 5mL of IV NaCl at 0.9%. All ponies underwent gastroscopy before and after each experimental phase. Additionally, in the first phase, urine, hematological (hematocrit, and total plasma protein) and biochemical (creatinine, albumin, Ca+2 and P+3) analysis were determined. In the first phase, only the two ponies treated with 6mg kg-1 of mofebutazone presented ulcers in the aglandular region along the margo plicatus. In the second phase, two animals also presented gastric ulcers, with one having received only NaCl at 0.9% solution. Ulcers occurrence was neither influenced (P>0.05) by the administration and dose of mofebutazone, nor by the presence of omeprazole. Mofebutazone effect on the hematological and biochemical variables was unremarkable (P+3) or absent (hematocrit, total plasma protein, creatinine, albumin, Ca+2) (P>0.05). Based on these results the following conclusions could be drawn: the occurrence of gastric ulcers in the aglandular region of healthy ponies was not influenced by application and dose of mofebutazone when administered for 12 days; grade four ulcers in the aglandular region of ponies may not be accompanied by clinical signs; healthy ponies tolerate application of up to 6mg kg-1 of IV mofebutazone for 12 days without the occurrence of renal damage; hematological and biochemical variables are not or minimally influenced by mofebutazone and a relation between omeprazole and gastric ulcers could not be confirmed in mofebutazone-treated ponies.

This paper investigates the transmission channel of macroprudential instruments in a closed-economy DSGE model with a rich set of financial frictions. Banks&#39; decisions on risky retail loan concessions are based on borrowers&#39; capacity to settle their debt with labor income. We also introduce frictions in banks&#39; optimal choices of balance sheet composition to better reproduce banks&#39; strategic reactions to changes in funding costs, in risk perception and in the regulatory environment. The model is able to reproduce not only price effects from macroprudential policies, but also quantity effects. The model is estimated with Brazilian data using Bayesian techniques. Unanticipated changes in reserve requirements have important quantitative effects, especially on banks&#39; optimal asset allocation and on the choice of funding. We explore an alternative approach to bridge the gap between New Economic Geography (NEG) theory and practice. We depart from Haddad and Hewings (2005), which offers some preliminary steps in the marriage of some of the theoretical foundations of NEG with spatial computable general equilibrium (CGE) models. It is argued that such approach should not be neglected as a potential modeling strategy to be pursued in order to reinforce policy relevance of NEG-based models. We apply the proposed methodology to look at the ex ante potential spatial implications of reductions in transportation costs within the Colombian economy. The results are shown to be in line with NEG models, reproducing empirical regularities evidenced from econometric estimates.
