There is growing evidence that technological mechanisms which support eHealth enhance patient-pro... more There is growing evidence that technological mechanisms which support eHealth enhance patient-provider engagement to improve health, care coordination, and healthcare outcomes while developing healthcare into a more cost-effective ecosystem. Web-based patient-provider portals are pivotal technological advancements driven by clinical health demands of the twenty-first century and promote structured experiences in interactive virtual environments that offer self-management tools. This paper identifies key issues that have adversely impacted the patient-provider engagement and have compromised self-management of health as well as explicates key functional requirements that would effectively address these issues. Through the implementation of a HIPAA-compliant, web-based patient-provider portal, self-management of health would be centralized to promote patient-centered care and care coordination in/outside primary care settings. This ultimately empowers patients to manage their health while enabling providers to effectively engage via health information technology platforms. Additionally, this paper describes how web portals serve as a practical solution to empowering high-risk patients with chronic conditions in managing their health.
Millions of individuals experience incapacitating events and symptoms that derive from chronic co... more Millions of individuals experience incapacitating events and symptoms that derive from chronic conditions and better understanding these events at the molecular level will ultimately result in individualized, pharmacological approaches that alleviate pain, symptomatic precursors, and death in these individuals. Based on genomics and symptoms science and the integration of human genomes and pharmacogenomics with electronic health records, we can determine strategies to mitigate risks associated with chronic disease while altering genetic and epigenetic events thus influencing improved health outcomes.
