Using data from the Women's Health Survey of Accra, Wave 2 (WHSA-2), the authors a) examine the consistency of the objective measures of health status (anthropometry and blood pressures) with self-report measures, including the Short Form 36 indices for 8 separate domains of health; and b) describe the main socio-economic differentials in morbidity. Cross-sectional household survey with field measurements. 2814 women aged 18 and over were interviewed and measured in their homes in late 2008 and early 2009. The physical measurements included height, weight, waist and hip measurement and 3 or more measures of resting blood pressure. Using the 8 domains of self-reported health captured by the Short Form 36 instrument, we find that physical health worsens more sharply with age than mental health. Social class differentials are narrow in t...

Mental disorders represent 9% of disease burden in Ghana. Women are more affected by common mental disorders, and are underrepresented in treatment settings. This study examines physical and social correlates of mental illness in adult women in Accra, Ghana, so as to inform general clinical practice and health policy. The SF-36 and K6 forms and 4 psychosis questions were administered in three languages to 2,814 adult women living in Accra, as part of a larger cross-sectional population-based survey of women's health. The validity of these tools was assessed through correlations within and between measures. Risk factors for mental distress were analysed using multivariate regression. Health service use was also described using statistical frequencies. Both the SF36 and K6 appear valid in a female Ghanaian population. Low levels of education, poverty and unemp...

The sample (n=2,814) consisted of Ghanaian women aged 18 years and older, living in the Accra Metropolitan Area. Data were collected between October 2008 and June 2009 in the Women's Health Study of Accra, Wave II household survey. Height (m), weight (kg), waist (cm) and hip (cm) measurements were used to calculate body mass index (BMI, measured in kg/m(2)), waist circumference, and waist-to-hip ratio. In addition to reported socio-demographic characteristics, medical history, and exercise and diet history, the standardized Stunkard Figure Rating Scale (FRS) was used to identify silhouettes of varying body sizes that most closely resemble a woman's current body image (CBI), the body image she sees as the ideal for Ghanaian women (IBI), and the body image she sees as healthiest (HBI). The validity of using the standardized Stunkard FRS to assess body image in a populatio...

Intimate partner violence was measured using the World Health Organization's domestic violence questionnaire which was modified by the results of focus group discussions conducted in Amman. The percentages of women experiencing at least 1 form of control or violence since marriage were: control, 97.2%; psychological violence, 73.4%; physical violence, 31.2%; and sexual violence, 18.8%. Modifications of the WHO questionnaire were needed to measure control and psychological violence in Jordan. Similar modifications might be required when conducting research in the Region.

Irrigated urban agriculture (UA), which has helped alleviate poverty and increase food security i... more Irrigated urban agriculture (UA), which has helped alleviate poverty and increase food security in rapidly urbanizing sub-Saharan Africa, may inadvertently support malaria vectors. Previous studies have not identified a variable distance effect on malaria prevalence from UA. This study examines the relationships between self-reported malaria information for 3,164 women surveyed in Accra, Ghana, in 2003, and both household characteristics and proximity to sites of UA. Malaria self-reports are associated with age, education, overall health, socioeconomic status, and solid waste disposal method. The odds of self-reported malaria are significantly higher for women living within 1 km of UA compared with all women living near an irrigation source, the association disappearing beyond this critical distance. Malaria prevalence is often elevated in communities within 1 km of UA despite more favorable socio-economic characteristics than communities beyond 1 km. Neighborhoods within 1 km of UA...
We report the initial findings of a research programme on the fertility and reproductive health o... more We report the initial findings of a research programme on the fertility and reproductive health of both men and women in rural Gambia. The reproductive experiences of men and women in the population studied were very different. During the period 1993-97, the total fertility rates were 12.0 for men and 6.8 for women. For men fertility began later, reached higher levels and continued into older ages than for women. Through serial and polygynous marriages, men were able to extend their reproduction beyond what would be possible with one woman. Of the married men interviewed, 40% were married polygynously. Men's fertility preferences indicated that they recognized their reproductive potentials to be greater than those of their individual wives. On average, married men desired 15.2 children for themselves and 7.3 for each wife. In this polygynous population the means available for attaining reproductive goals were different for the two sexes, depending on the separate lives and diffe...

This study assessed the prevalence rate of HIV and risk factors associated with HIV infection in 1,328 women age 18 years and older. The weighted overall HIV prevalence rate for women residing in Accra is 3.1%. The highest prevalence rate of HIV infections was identified in women age 25 to 29 years at 8.3%, OR (95%CI) 3.8 (1.68-8.33), P = 001. In addition to young age, other significant risk factors included sexually transmitted infection (STI) symptoms (OR 1.81 [1.14-2.87], P = 0.012) and mean number of lifetime sexual partners (P < 0.001). All HIV-positive women were sexually active. Other findings significantly associated with HIV-positive status included chills, oral lesions, tuberculosis, bloody sputum production, and intestinal parasite infections. There was a significant association with HIV-po...

Data were collected by trained interviewers through questionnaire interviews of 3183 women. Data analysis was restricted to 1329 women with complete data. Only 19% of our study group had STI symptoms. Only 35% of the women with STI symptoms received care. Having high wealth index, being older and having no history of condom use were protective factors for experiencing STI symptoms. Seeking care was associated with increased by high wealth index and the presence of an offensive vaginal odour. Income level on its own did not affect health seeking behavior. Wealth index is the most significant determinant of a woman having STI symptoms and seeking care in Accra.
