Idiopathic granulomatous mastitis, four decades since its first description still remains a disea... more Idiopathic granulomatous mastitis, four decades since its first description still remains a disease of dilemma. Correct diagnosis with exclusion of various causes and appropriate treatment with surgery, antibiotics and steroids are the keys to overcome the persisting challenge that lies in management of this dilemmatic disease. We report 4 cases of idiopathic granulomatous mastitis that were treated in Kannur medical college hospital. Idiopathic granulomatous mastitis is uncommon benign breast disease that can be confused with two of the very common breast conditions, breast carcinoma and breast abscess. The management of IGM needs a holistic approach with the surgeon, pathologist and radiologist working in unison to unravel the difficulties that lie in treating this disease and bringing back the smile on patients face.

By altering the biomechanics of the movements at great toe, the interphalangeal sesamoid is prone to various painful pathologies like trauma, infection, degeneration, osteonecrosis and inflammation. Imaging plays an important role in narrowing down the differentials and guiding for appropriate therapy. Herein, we present a neglected case of hallucal interphalangeal sesamoiditis presenting as a case of chronic great toe pain and discuss the role of Magnetic resonance imaging (MRI) and Computed tomography (CT) scan in its diagnosis.
Mature cystic teratomas, also known as dermoid cysts, are composed of a mixture of tissues derive... more Mature cystic teratomas, also known as dermoid cysts, are composed of a mixture of tissues derived from all three germ layers and constitute about 20% of all ovarian neoplasms. However, malignant transformation in a mature cystic teratoma is an uncommon event occurring only in about 1.8% cases. A variety of tumours can arise within a mature cystic teratoma (most common being squamous cell carcinoma), with malignant melanoma being extremely rare among such tumors. While most authors believe that primary ovarian melanoma almost always arises within a mature cystic teratoma, the primary nature of an ovarian malignant melanoma can be ascertained only once metastasis from a cutaneous melanoma is ruled out. The first case of malignant melanoma arising in an ovarian mature cystic teratoma was reported in 1901 and only about 45 additional cases have been reported till date. We present another case of a primary malignant melanoma arising within a mature cystic teratoma.
ABSTRACT There has been a tremendous progress in most of the computational domains in recent time... more ABSTRACT There has been a tremendous progress in most of the computational domains in recent times. One of the key areas is analyzing the complex interaction of blood flow through elastic artery. Such study will aid in predicting the progression and behaviour of diseases like atherosclerosis and hypertension. In the present investigation, a simple straight common carotid artery having an eccentric blockage is analyzed for normal as well as high blood pressure condition. The clinical data has been obtained from ultrasound measurements on a volunteer patient and a model is generated, ANSYS, a commercially available Finite Element Analysis software. The ANSYS Fluid-Structure Interaction (FSI) solver is used to perform the coupled field transient analysis for three pulse cycles and the obtained results demonstrate the pulsatile flow. The simulation shows that the influence of hypertension has high deformation than normal case, severely altering the flow dynamics in downstream of the stenosis. The results obtained agree very well with available clinical observation of normal blood pressure condition.

Gravity associated with postural changes has a strong bearing on haemodynamics of blood flow in a... more Gravity associated with postural changes has a strong bearing on haemodynamics of blood flow in arteries. Its effect on stenosed cases has not been widely investigated. In the present study, variation observed in blood flow during postural changes is investigated for different conditions like standing, sleeping and head-down position. A fluid structure interaction study is carried out for idealized normal and 75% eccentric and concentric stenosed common carotid normal artery. The results clearly indicate the effects of altered gravity on flow conditions. It was found to be very significant during head-down position and demonstrated very high arterial blood pressure in stenosed common carotid when compared with normal carotid.

The present study deals with a patient specific case of having an aneurysm on left side internal carotid bifurcation (ICA) bifurcation and right side is normal. The three dimensional(3D) fluid models of normal and aneurysm ICA bifurcation is generated using MIMICS13.0. After volume rendering in CATIA V5R19.0, the meshing is carried out in GAMBIT and transient analysis is simulated in ANSYS CFX. The simulation results provide a comparison of flow behaviour between normal and aneurysm models. It demonstrates the significance of flow analysis in understanding the haemodynamics at crucial sites and agrees well with the available literature and clinical observation.
