In Österreich konnte eine Krise innerhalb des medizinischen Versorgungssystems bisher verhindert werden. Die Behandlung von Patienten mit ambulant erworbener Pneumonie („community acquired pneumonia“, CAP), inklusive durch SARS-CoV-2-Infektionen, sollte sich auch während der Pandemie weiterhin an evidenzbasierten CAP-Leitlinien orientieren. COVID-19-spezifische Anpassungen sind jedoch sinnvoll. Die Behandlung von Patienten mit chronischen Lungenerkrankungen muss während der Pandemie angepasst werden, ist aber weiterhin zu gewährleisten.
A new molecular assay, based on a rapid DNA extraction protocol, PCR, and hybridization to a spec... more A new molecular assay, based on a rapid DNA extraction protocol, PCR, and hybridization to a specific probe in a nonradioactive microwell plate format was used to detect Mycoplasma pneumoniae in bronchoalveolar fluid specimens. The sensitivity of the assay was determined to be 10 to 100 organisms with M. pneumoniae reference strains. Specificity testing with different bacteria capable of producing pneumonia showed no cross-reactivity. In a prospective study, bronchoalveolar lavage fluids obtained from patients with pneumonia were investigated with the PCR assay and compared to culture. Twelve positive samples were detected with the PCR assay. Seven of them were subsequently confirmed by culture. All patients with positive PCR results seroconverted. Application of the PCR assay described may lead to safe and early diagnosis of M. pneumoniae in patients with pneumonia.

ABSTRACT In der Behandlung von Patienten mit zystischer Fibrose („cystic fibrosis“, CF) werden in... more ABSTRACT In der Behandlung von Patienten mit zystischer Fibrose („cystic fibrosis“, CF) werden inhalative Antibiotika als Teil von evidenzbasierten Behandlungskonzepten zur Eradikation bei einer Erstinfektion mit Pseudomonas aeruginosa (PA) sowie zur Suppressionstherapie bei chronisch mit Pseudomonas kolonisierten Patienten seit Jahren erfolgreich eingesetzt. Von einer chronischen PA-Kolonisation sind auch viele Patienten mit Non-CF-Bronchiektasen (NCFB) betroffen. Hierbei handelt es sich jedoch um eine heterogene Patientengruppe mit unterschiedlichen Grunderkrankungen und die Evidenz zur Wirksamkeit inhalativer Antibiotika bei CF kann nicht unkritisch auf NCFB übertragen werden. Publizierte Daten belegen mittlerweile den klinischen Nutzen inhalativer Antibiotika bei ausgewählten NCFB-Patienten. Wird die Anwendung inhalativer Antibiotika im NCFB-Bereich erwogen, ist auf strenge Indikationsstellung, strukturierte Therapiepläne und die konsequente Umsetzung multimodaler Behandlungskonzepte zu achten. Ein universeller Test existiert nicht; erst die Kombination mehrerer Methoden erlaubt eine umfassende Beurteilung des Funktionszustands des Respirationstrakts. Die g€angigsten Methoden erfordern eine aktive Mitarbeit des Probanden oder Patienten und sind deshalb in der Routine erst ab dem sp€ateren Kleinkindesalter bzw. Schulalter einsetzbar. Die Ruhespirometrie ermoglicht eine Messung der einbzw. ausgeatmeten Volumina. Gasdilutionsmethoden und Ganzkorperplethysmografie erg€anzen diese Messung um das nicht ausatembare Residualvolumen und machen so die Erfassung der statischen Lungenvolumina komplett. To evaluate the reliability and accuracy of a rapidly performed enzyme immunochromatography metho... more To evaluate the reliability and accuracy of a rapidly performed enzyme immunochromatography method for measuring the serum concentration of theophylline, we compared it with a conventional technique (fluorescence-polarisation immunoassay). We investigated 103 venous blood samples from children and adolescents with bronchial asthma, medicated with sustained-release theophylline preparations. There was a highly significant correlation of the measured values; with a few exceptions, the individual values scattered only narrowly around the regression line. Based on this close correlation, the investigated enzyme immunochromatography test can be recommended for rapid theophylline serum level determinations in clinical practice.
ABSTRACT Congenital abnormalities of the lung are individually infrequent but as a group are impo... more ABSTRACT Congenital abnormalities of the lung are individually infrequent but as a group are important disorders of lower respiratory tract development. They comprise a wide range of abnormalities of the bronchial, arterial, venous and lymphatic trees. In addition, abnormalities of the chest wall, including the diaphragm, obviously may have a huge impact on lung development. These abnormalities today are usually diagnosed and managed antenatally, in the newborn period, in infancy, or in childhood, and some of them may cause severe respiratory distress immediately after birth. In a small number of patients, congenital lung abnormalities may go unrecognised in infancy and childhood; in these cases, late complications such as recurrent localised pneumonitis, abscess formation and haemoptysis, or the coincidental discovery on a chest radiograph, may lead to the diagnosis in adolescence or adulthood [1, 2]. For many congenital lung abnormalities, endoscopic and radiographic techniques (such as plain radiography, computed chest tomography and magnetic resonance imaging) are essential diagnostic tools. In addition, a host of lung function tests has been applied in these patients for various purposes; however, the clinical relevance of lung function testing has not yet become entirely clear in this context. In this chapter, the most prominent congenital lung abnormalities, i.e. tracheo-and bronchomalacia, tracheal and bronchial stenosis, tracheo-oesophageal fistula (TOF), the so-called cystic malformations of the lung, and congenital diaphragmatic hernia (CDH), are discussed. The individual subsections are uniformly organised and contain 1) short descriptions of the respective abnormalities with some information on clinical aspects, followed by 2) a review of relevant studies on lung function testing performed in patients with the respective disorders including the main findings, and finally 3) a short concluding statement on the tests used so far and their clinical relevance in patients with the respective congenital lung abnormalities, and future prospects. Tracheo-and bronchomalacia, tracheal and bronchial stenosis Tracheo-and bronchomalacia are characterised by absence, deficiency, malformation or softness of airway wall cartilages; as a consequence of intrathoracic airway malacia, dynamic airway compression on expiration and during coughing occurs, resulting in retention of secretions from the lower airways, in turn possibly leading to recurrent or chronic bronchopulmonary infections. Congenital forms of airway malacia may be distinguished from acquired, localised from generalised, and primary from secondary ones. While primary airway malacia is characterised by an intrinsic abnormality of the airway wall, in secondary forms there is extrinsic compression, e.g. by a vascular ring (such as double aortic arch, right aortic arch with left patent ductus arteriosus or ligamentum arteriosum, and aberrant left pulmonary artery/pulmonary artery sling).

The American Food and Drug Administration (FDA) published a relevant alert in 2005. In the currently valid Austrian consensus guidelines for drug therapy of bronchial asthma in children and adolescents, LABAs are only recommended as add-on therapy in those patients whose asthma is not sufficiently controlled by inhaled corticosteroids (ICS) alone. LABAs have no established role in earlier steps of the therapeutic algorithm; consequently, the prescription of ICS-LABA combinations for initial treatment of paediatric asthma is not supported by these consensus treatment guidelines.

One rare cause of arthralgia is hypertrophic osteoarthropathy associated with digital clubbing. We present a child where hypertrophic osteoarthropathy led to the rare diagnosis of an inflammatory pseudotumour of the lung. In a 12-year-old girl with arthralgia and digital clubbing, a chest radiograph disclosed a large round mass in the right upper lobe, and the following chest computed tomography scan showed a large solid homogenous, round, well marginated lesion with little contrast enhancement. A lobectomy of the right upper lobe was performed, and histological examination showed an inflammatory pseudotumour. The postoperative course was without problems: arthralgia and digital clubbing disappeared.

ABSTRACT Congenital abnormalities of the lung are individually infrequent but as a group are impo... more ABSTRACT Congenital abnormalities of the lung are individually infrequent but as a group are important disorders of lower respiratory tract development. They comprise a wide range of abnormalities of the bronchial, arterial, venous and lymphatic trees. In addition, abnormalities of the chest wall, including the diaphragm, obviously may have a huge impact on lung development. These abnormalities today are usually diagnosed and managed antenatally, in the newborn period, in infancy, or in childhood, and some of them may cause severe respiratory distress immediately after birth. In a small number of patients, congenital lung abnormalities may go unrecognised in infancy and childhood; in these cases, late complications such as recurrent localised pneumonitis, abscess formation and haemoptysis, or the coincidental discovery on a chest radiograph, may lead to the diagnosis in adolescence or adulthood [1, 2]. For many congenital lung abnormalities, endoscopic and radiographic techniques (such as plain radiography, computed chest tomography and magnetic resonance imaging) are essential diagnostic tools. In addition, a host of lung function tests has been applied in these patients for various purposes; however, the clinical relevance of lung function testing has not yet become entirely clear in this context. In this chapter, the most prominent congenital lung abnormalities, i.e. tracheo-and bronchomalacia, tracheal and bronchial stenosis, tracheo-oesophageal fistula (TOF), the so-called cystic malformations of the lung, and congenital diaphragmatic hernia (CDH), are discussed. The individual subsections are uniformly organised and contain 1) short descriptions of the respective abnormalities with some information on clinical aspects, followed by 2) a review of relevant studies on lung function testing performed in patients with the respective disorders including the main findings, and finally 3) a short concluding statement on the tests used so far and their clinical relevance in patients with the respective congenital lung abnormalities, and future prospects. Tracheo-and bronchomalacia, tracheal and bronchial stenosis Tracheo-and bronchomalacia are characterised by absence, deficiency, malformation or softness of airway wall cartilages; as a consequence of intrathoracic airway malacia, dynamic airway compression on expiration and during coughing occurs, resulting in retention of secretions from the lower airways, in turn possibly leading to recurrent or chronic bronchopulmonary infections. Congenital forms of airway malacia may be distinguished from acquired, localised from generalised, and primary from secondary ones. While primary airway malacia is characterised by an intrinsic abnormality of the airway wall, in secondary forms there is extrinsic compression, e.g. by a vascular ring (such as double aortic arch, right aortic arch with left patent ductus arteriosus or ligamentum arteriosum, and aberrant left pulmonary artery/pulmonary artery sling).

The American Food and Drug Administration (FDA) published a relevant alert in 2005. In the currently valid Austrian consensus guidelines for drug therapy of bronchial asthma in children and adolescents, LABAs are only recommended as add-on therapy in those patients whose asthma is not sufficiently controlled by inhaled corticosteroids (ICS) alone. LABAs have no established role in earlier steps of the therapeutic algorithm; consequently, the prescription of ICS-LABA combinations for initial treatment of paediatric asthma is not supported by these consensus treatment guidelines.

One rare cause of arthralgia is hypertrophic osteoarthropathy associated with digital clubbing. We present a child where hypertrophic osteoarthropathy led to the rare diagnosis of an inflammatory pseudotumour of the lung. In a 12-year-old girl with arthralgia and digital clubbing, a chest radiograph disclosed a large round mass in the right upper lobe, and the following chest computed tomography scan showed a large solid homogenous, round, well marginated lesion with little contrast enhancement. A lobectomy of the right upper lobe was performed, and histological examination showed an inflammatory pseudotumour. The postoperative course was without problems: arthralgia and digital clubbing disappeared.
