IntroductionIn patients with unresectable Differentiated thyroid cancer (DTC), the use of external beam radiation therapy (EBRT), leads mostly to palliation. Our aim is to define the role of upfront EBRT, followed or not by salvage surgery, on Progression-free survival (PFS) or Overall survival (OS) in patients with DTC.MethodsThis is a cohort study of patients with initially unresectable DTC who received EBRT. Cohort A received EBRT followed by rescue surgery and cohort B, EBRT only. The Kaplan-Meier method and Cox model were employed for survival analysis.ResultsThirty-three patients were included; 69.6% females and 30.3% males. Mean age was 60.6 and mean tumor diameter was 10.4 cm; 17 and 16 patients were included in cohorts A and B, respectively. Belonging to cohort A (Hazard ratio [HR] 0.177, 95% CI 0.05–0.7) and use of intensity modulated radiotherapy (HR 0.177, 95% CI 0.03–1.08) were associated to better PFS, while high-risk histopathology (HR 6.6, 95% CI 0.9–50) and EBRT dos...

Two ferrite-martensite stainless steels(FMSSs)were developed by Baosteel based on the T4003 composition, through optimizing the manganese and nickel contents, reducing silicon, carbon and nitrogen contents, controlling remnant niobium and molybdenum, adding sufficient titanium and controlling the processing. In this study, the physical metallurgy of such FMSSs was investigated with the emphasis on the alloying effect on the phase balance during processing and the transformation behavior during welding for different microstructures. In addition, the mechanical behavior and the weldability were investigated. The results indicate that such steels have a good combination of strength and toughness and better weldability compared with the traditional 1.4003 steel.Such high strength steels are highly suitable for railway cargo transportation where the wall thickness of the wagons can be reduced, resulting in weight savings.

We describe an 18-year-old woman with a mass in the region of the right ovary. The mass, which had a central cavity lined by endometrium and surrounded by smooth muscle, extraordinarily resembled a uterus. Review of the literature disclosed only one such case in which this change was considered to arise from endomyometriosis of the ovary, a phenomenon characterized by smooth-muscle metaplasia in a focus of endometriosis. Although this possibility is interesting and plausible, we believe that a congenital malformation, possibly caused by a defect in fusion of müllerian ducts, should strongly be considered in explaining the pathogenesis of this kind of lesion.

Angiocentric lymphomas (AL) involving the mid-face are infrequent lesions. Their incidence seems to be higher in certain Asian and Latin American countries, where most of the reported series reveal a predominance of T-cell lineage and a high frequency of EBV-positive cells. The clinical and pathological features of 12 AL that affected the palate are presented. Immunophenotypic studies were performed in nine of these cases. Seven (77%) demonstrated T-cell lineage, one was of B-cell origin and one could not be classified. The high incidence of T-cell lesions in this and other studies supports the existence of geographical and possibly of etiological differences in these neoplasms.

Although various sources of vascularized bone/composite tissue have been used for mandibular reconstruction, the vascularized fibular flap has been reported to have many advantages over the others and to be the most suitable for bridging a long-span mandibular defect. In this paper, positive experience of 14 consecutive cases in which a free vascularized fibular graft with or without a skin paddle has been used to reconstruct a long mandibular defect is reported. Half of the cases were primarily reconstructed, the rest secondarily. The average length of the fibula graft harvested was 16.7 cm, with a range of 12 to 22 cm. The number of wedge osteotomies ranged from 0 to 4, with an average of 1.67. In ten flaps, a peroneal skin paddle was included. The donor wound was typically closed directly, except in three cases, where a skin graft was used. Vein grafts were performed in five cases to lengthen the pedicle. In two cases, the skin partially necrosed. All scintigrams except one showed good bone viability. But there was no total flap failure. Postoperative facial appearances were fair to excellent, with mouth opening ranging from 21 to 50 mm. Although unusual serious donor wound rupture was experienced in one case, in general the donor site complications were minimal.

Current research indicates that prostate-specific membrane antigen (PSMA) is related to angiogenesis of many solid tumors including breast cancer (BC), our objective is evaluating PSMA expression in primary tumor and metastatic BC by Positron emission tomography/computed tomography (PET/CT). In this retrospective study twenty-one patients with BC included all molecular subtypes, was evaluated with 18F-FDG-PET/CT imaging as stratification and 68Ga-PSMA-PET/CT. Primary sites of BC was identifying in all patients with 18F-FDG-PET/CT. We identified lymph node metastases in 17 patients (81%) and metastatic disease in 15 patients (71%). A total 127 lesions were detected by 18F-FDG-PET/CT, 30 of which were in the breast, 31 axillary lymph-node metastases, 25 mediastinal lymph-node metastases, 15 distant non-bone metastases and 26 bone metastases. 68Ga-PSMA-11-PET/CT showed lower detection-rate (DRs) than did 18F-FDG-PET/CT in all patients with LUM-A and LUM-B HER2. All 18F-FDG PET/CT posit...

One of the greatest achievements of medicine is the understanding of the infection from human papillomavirus (HPV) and its causal relation to some types of cancer. Most people with HIV infections have no symptoms, so it may go unnoticed and the infection is limited by the host. It is important to note that less than 10% of patients have a persistent infection, which can lead to the development of malignancy. HPV requires a small tissue damage placed in a transitional area of epithelia, such as the existent in the cervix, anus or tonsils, to infect cells. The KEYWORDS

In the last decade, minimal invasion surgery (MIS) has been used in a great variety of diseases and clinical situations. In gastric carcinoma (GC), specific indications in various circumstances have been described. MIS is used to make a diagnosis, for clinical stage classification, and for respectability evaluation, allowing to design a suitable therapy for each patient, because effective neoadjuvant treatments (chemotherapy and radiochemotherapy) are available. MIS has also been used for surgical resection of gastric neoplasia, including endoscopic mucosal resection, wedge resection of gastric wall, subtotal gastrectomy, total gastrectomy or even lymphadenectomy. MIS is used for perform palliative gastrojejunostomy in patients with distal GC, as well as for gastrostomy or jejunostomy for enteral nutrition in specific situations. MIS is still in experimental phase in GC, except in the case of preoperative staging laparoscopy.
