    In acne management, oral isotretinoin (IST) is associated with various untoward systemic effects, while its topical formulation has irritation side effects.Delonix(DLX) is a natural galactomannan derived fromDelonix regiaseed that can fabricate nanoparticles for topical skin delivery. This study aims to develop IST-DLX nanoparticles and assess their prospects for acne treatment. Fluorescent-DLX nanoparticles (made with a lipophilic BODIPY dye) or IST-DLX nanoparticles were prepared and characterized. BODIPY-DLX nanoparticles' skin distribution and IST-DLX nanoparticles'in-vitrotargeting were assessed in pig ear skin, inflammatory modulation was assessed in AMJ-2 macrophage cells, while skin penetration and irritation were assessed in Wistar rats. IST-DLX nanoparticles had ≈230 nm, negative zeta potential, and ≈30% encapsulation efficiency. Confocal showed BODIPY in DLX nanoparticles accumulated in hair follicles as compared to BODIPY solution. IST-DLX nanoparticles released ≈37% IST over 48 h and increased IST 3-fold in hair follicles compared to IST solution. IST-DLX nanoparticles suppressed IL-6 expression in cells and reduced photo-irritation in Wistar rats compared to IST solution. In conclusion, IST-DLX nanoparticles may target and deliver adequate IST to skin layers associated with acne, avoid systemic penetration, modulate inflammatory pathogenic acne stage and prevent IST topical photo-irritation.

    Mohs micrographic surgery (MMS) promotes high cure rates, but accessibility to MMS is limited in distinctive realities and countries. We sought to verify applicable criteria for MMS indication and prioritization regarding basal cell carcinoma (BCC) in the face of various limitations. We analyzed MMS-excised BBC, without patient exclusion, through a retrospective cohort study at a single university center. Mohs micrographic surgery was performed in 101 BCCs, average size = 5.44 ± 11.91 cm, 56.44% ≥ 20 mm. Most BCCs were in the H-zone (87.13%) and on the nose (52.47%). Histology showed high-risk pattern in most of tumors (69.31%), primary (64.71%), and recurrent (74.0%) BCC. Nasal (p = .01) and recurrent BCC (p = .03) had increased risk for two or more MMS stages.     PURPOSE To describe the occurrence of multiple trigeminal nerves (TGNs) enlargement in patients with orbital IgG4-related disease. METHODS Retrospective review of MRI findings and medical records of 6 patients (10 orbits) with orbital IgG4-related disease and enlargement of more than 1 TGN. Orbital biopsies were performed in all cases revealing the typical lymphoplasmacytic infiltrate with significant plasma cell positivity for IgG4 (IgG4+/IgG ratio ≥ 40%). Three experienced neuroradiologists reviewed the MRI sequences using a digital imaging viewer system (Horos, https://horosproject.org/). RESULTS Bilateral involvement of at least 2 TGNs divisions was detected in all 6 patients. Enlargement of both V1 and V2 nerves was diagnosed in 5 patients, and in 3 cases, all TGN divisions were involved. V2 nerves were the most affected. In this division, all 12 infraorbital nerves were enlarged, followed by lesser palatines (10/83.3%), superior alveolar (10/83.3%), and zygomatic (6/50%). V1 and V3 nerves were less affected albeit 9 (75%) frontal branches (V1), and 50% of the inferior alveolar (V3) nerves were also enlarged. CONCLUSIONS Widespread involvement of the TGN is an important feature of IgG4-related disease.

    We report a case of a juvenile xanthogranuloma in a 7-year-old girl on the corneoscleral limbus. It presented as a massive, unsightly, nodular lesion that prevented the patient from completely closing her left eye. We were able to excise the lesion completely, leading to a successful outcome. Histopathology examination with immunohistochemical staining established the diagnosis. As far as we know, this is the most extensive xanthogranulomatous limbal lesion reported in the literature.     Congenital optic nerve cystic-like malformations associated with normally developed globes are extremely rare. We describe 3 children who presented since birth with proptosis, and eye motility limitation. MRI showed in all cases that the intraorbital segment of the optic nerves was malformed with large cystic-like lesions in the intraconal segment of the orbit. In all cases, biopsies of the wall of the lesions were positive for glial fibrillary acidic protein. Since this protein is a neurobiomarker that exists only in astrocytes in the central nervous system, nonmyelinating Schwann cells of peripheral nerves, and enteric glial cells, we believe that these lesions represent true opticmeningoceles.

    The authors developed a retinoblastoma model using fresh harvested cells from an enucleated eye that were transplanted in chick embryos (chorioallantoic membrane model). The transplanted embryos were treated with escalating doses of Melphalan. This exploratory model was developed with the goal of testing drug sensitivity. Our findings suggest this tumor model could be employed to personalize treatment for patients with retinoblastoma, especially those with bilateral and more refractory disease.

    The association of C-antineutrophil cytoplasmic antibody (ANCA) vasculitis and IgG4 positivity is a new condition not well described in clinical terms. The authors examined a 28-year-old man with a previous diagnosis of eosinophilic granulomatosis with polyangiitis, formerly known as Churg-Strauss disease, who presented with bilateral orbital inflammation. Magnetic resonance imaging revealed diffuse orbital infiltration and enlargement of the major divisions of the trigeminal nerve. Biopsy of the orbital contents showed necrotizing granulomatous vasculitis and a high number of IgG4-positive plasma cells (IgG4/IgG = 60%).

    Objectives: To assess the diagnostic accuracy of signal loss on in-phase (IP) gradient-echo (GRE) images for differentiation between renal cell carcinomas (RCCs) and lipid-poor angiomyolipomas (lpAMLs). Methods: We retrospectively searched our institutional database for histologically proven small RCCs (<5.0 cm) and AMLs without visible macroscopic fat (lpAMLs). Two experienced radiologists assessed MRIs qualitatively, to depict signal loss foci on IP GRE images. A third radiologist drew regions of interest (ROIs) on the same lesions, on IP and out-of-phase (OP) images to calculate the ratio of signal loss. Diagnostic accuracy parameters were calculated for both techniques and the inter-reader agreement for the qualitative analysis was evaluated using the κ test.     Lung cancers may extend along or grow through the pulmonary veins to invade or lie within the left atrium (LA). A 62-year-old man, previously healthy, presented with 1-month ventilatory-independent right hemithorax back pain, dry cough and large effort dyspnea. He also referred weight loss of 12 kg in 10 months and denied hemoptysis. As antecedents, he smoked for 40 years and moderate daily alcoholism. On physical examination, the patient was in good general condition, hydrated and regular respiration at rest [blood pressure (BP) =120/80 mmHg; heart rate (HR) =90 bpm; respiratory rate (RR) =16 rpm]. Cardiac auscultation revealed two standard rhythmic sounds without murmurs.     Copaiba oil is widely used in medicine, but there are no reports regarding its application in ophthalmology. Therefore, the objective of this study was to evaluate the clinical, histopathological and toxicogenetic effects of eye drops containing 0.1 and 0.5% of Copaifera multijuga Hayne oil on superficial corneal ulcers induced with alkali in the left eye of rats. For histological analysis, the percent reduction in ulcers and thickness of the corneal epithelium and stroma were evaluated 48 and 72 h after ulcer induction. Additionally, neovascularization and polymorphonuclear infiltration were classified in the stroma. The bone marrow micronucleus test was used for toxicogenetic assessment.     dren treated at our institution for bilateral Wilms tumor without antecedent diagnostic biopsy. All received vincristine, actinomycin D, and doxorubicin for a minimum of 6 weeks prior to surgery. RESULTS: Age at clinical diagnosis ranged from 8 to 63 months. All 5 children had a palpable abdominal mass. Ultrasound failed to detect bilateral disease in 3. All diagnoses were made with cross-sectional imaging. Studies to rule out neuroblastoma were obtained in 4. Time from diagnosis to chemotherapy ranged from 2 to 8 days. After 6 weeks of chemotherapy, the average decrease in size of the largest tumor and the largest contralateral tumor was 41% (median 49%) and 47% (median 34%), respectively. In 2 cases where additional chemotherapy was given to further reduce tumor size preoperatively, the interval decrease in size of the largest tumor was 14-16% with no change in contralateral tumors; in one, all other ipsilateral tumors increased. Radical nephrectomy was required in 3 children on one side only; one whose multifocal tumors increased in size after additional chemotherapy, one who had the only mass to increase in size after initial chemotherapy (11.0 to 11.2 cm), and one who underwent conversion to radical nephrectomy due to an inability to spare the renal vein and collecting system. Renal function was preserved in all 3. In 2 kidneys, the largest tumors were reduced from 16.0 to 8.2 cm and from 6.7 to 3.0 cm; partial nephrectomies were achieved. Among all other partial nephrectomies, the largest tumor was 2.8 cm at diagnosis. Pathology confirmed Wilms tumor in all children. Only 2 who had pulmonary metastases at diagnosis had individual kidney staging greater than Stage I. After an average 5 months postoperative followup, monitoring of pulmonary lesions is ongoing in 3 children and 2 have no evidence of disease. CONCLUSIONS: In our series, biopsy-free management of bilateral Wilms tumors did not lead to misdiagnosis. While bilateral partial nephrectomies were achieved, neoadjuvant chemotherapy did not preclude radical nephrectomy in 3 children.

    Plasmablastic lymphoma (PBL) is a rare B-cell lymphoma occurring mainly in HIV patients. The tumor frequently involves extranodal sites such as the oral cavity, nasal cavity, gastrointestinal tract, skin, and lungs. The neoplastic cells are characterized by a plasmablastic appearance and typical immunophenotype that indicates plasma cell differentiation. Herein, we report a case of intraocular involvement by plasmablastic lymphoma in a HIV patient with a long history of cytomegalovirus retinitis and loss of vision. After an evisceration performed to control the blind, painful eye a diagnosis of PBL was made by examining the eye contents. Two months later, a biopsy of cervical lymph node revealed nodal lymphoma of the same type. To the best of our knowledge, this is the second case of intraocular PBL reported in English literature.

    Orbital biopsy of nonspecific orbital inflammation, commonly referred to as "orbital pseudotumor," typically shows a combination of polyclonal lymphocytes, plasmocytes, leukocytes, macrophages, and variable degrees of collagen deposition. Herein, we report a patient with a positive history of mucocutaneous leishmaniasis who presented with an orbital mass with a histological profile of idiopathic orbital inflammation. Immunohistochemical and molecular analysis of the orbital specimens demonstrated that the orbital inflammation was associated with the presence of antigens of Leishmania braziliensis and DNA from the parasite.

    Posterior scleral tuberculoma formation is an extremely rare condition. The few reports on scleral involvement in tuberculosis refer to cases of anterior scleritis. In the present manuscript we describe a patient who had rheumatoid arthritis and developed a large posterior scleral tuberculoma. The lesion provoked retinal detachment and visual loss and was diagnosed only after enucleation due to a misdiagnosis of choroidal melanoma.
