Objective: To evaluate the accessibility to a Montreal low-threshold methadone program (Relais- M... more Objective: To evaluate the accessibility to a Montreal low-threshold methadone program (Relais- Méthadone). This program is aimed at a marginalized population of heroin addicts who are injection drug users (IDU).Method: The data (n=141 clients) were collected during the first year of the program implementation with questionnaires administered by the programme workers.Results: Analysis of the characteristics of the clients revealed that the program does reach the target population. The program’s clientele is characterized by long-time and frequent heroin use, unstable lifestyles, and the presence of numerous behaviours that put them at high risk for HIV transmission. The retention of clients in the program is very high (88 %) within the first 30 days.Interpretation: These results demonstrate the importance and value of flexible intervention programs in reaching a marginalized clientele exposed to the HIV virus, who would not have access to regular programs characterized by restrictive selection criteria and limited availability.
Objective: To establish the prevalence of clinically significant psychological distress in women ... more Objective: To establish the prevalence of clinically significant psychological distress in women and men involved in first-trimester abortions and to identify related risk factors. Design: Prospective cohort study. Setting: A downtown Montreal public abortion clinic and the Montreal metropolitan area. Participants: We recruited 197 women and 113 men involved in first-trimester abortions and compared them with control groups composed of 728 women and 630 men 15 to 35 years old who had taken part in a previous public health survey (Enquête Santé Québec 1987). One hundred twenty-seven women and 69 men completed the follow-up questionnaire. Main outcome measures: Level of distress as measured by the Ilfeld Psychological Symptom Index. Results: Before the abortion, 56.9% of women and 39.6% of men were much more distressed than their respective controls. Three weeks after the abortion, 41.7% of women and 30.9% of men were still highly distressed. Predictors of distress for women were fear of negative effects on the relationship, unsatisfactory relationships, relationships of less than 1 year, ambivalence about the decision to abort, not having a previous child, and suicidal ideation (this association was weaker than in controls). Predictors for men were fear of negative effects on the relationship, relationships of less than 1 year, preoccupation with the abortion and anxiety about its accompanying pain, negative perceptions of their own health, suicidal gestures in the past, and suicidal ideation in the past year (only the association with suicidal gestures was marginally stronger than in controls). Conclusion: Being involved in a first-trimester abortion can be highly distressing for both women and men.

Objectif: L'etude vise a determiner l'efficacite d'un programme de methadone a exigen... more Objectif: L'etude vise a determiner l'efficacite d'un programme de methadone a exigences peu elevees (Relais-Methadone) a rejoindre une clientele de consommateurs d'heroine marginalisee de la region de Montreal, de meme qu'a evaluer son accessibilite en terme de delai de prise en charge et sa capacite de retention en traitement. Methode: Les donnees ont ete recueillies aupres de 141 clients admis lors de la premiere annee d'implantation du programme par le biais de deux instruments a questions ouvertes et a choix multiples. Resultats: L'analyse des caracteristiques de la clientele revele que le programme rejoint efficacement le groupe cible. En effet, celle-ci se caracterise par une frequence et une duree de consommation d'heroine elevees, des conditions de vie precaires et de nombreux comportements a risque pour la transmission du VIH. La periode d'attente en debut de traitement, quant a elle, se chiffre en moyenne a 11,2 jours. Enfin, la retention de la clientele dans le programme est elevee (88 %) pour les premiers 30 jours. Discussion: Ces resultats demontrent la valeur de programmes d'interventions flexibles pour rejoindre une clientele marginalisee fortement exposee au virus du VIH qui n'aurait pas acces a un programme regulier etant donne les criteres de selection eleves et le manque de places disponibles.
