Since 1980 we have seen the introduction of several classifications of mental health disorders in... more Since 1980 we have seen the introduction of several classifications of mental health disorders intended for use in primary health care. This list began with the Diagnostic and Statistical Manual of Mental Disorders (DSM-III) and has continued through DSM-IV and DSM-IV-PC (Primary Care), the 10th edition of the International Classification of Diseases for Primary Health Care (ICD-10-PHC), and the International Classification of Primary Care (ICPC-2). Development of the ‘next generation’ of primary care mental health classifications is now under way, with work on ICD11-PC, DSM-V and ICPC-3. Despite considerable efforts at harmonisation, there are significant differences in how each of these tools covers the clinical domain of mental health problems. These differences reveal our lack of understanding about the process of emotional suffering and its relationship to the development of mental disorders, and reflect a fundamental difference between primary care and specialty mental health professionals in the perceived importance of social and cultural factors in understanding and formally diagnosing mental disorders. We seem to be able to agree on a set of criteria to diagnose hypertension, or diabetes, and these biomedical classification standards have been relatively stable over time. But we cannot agree on a stable set of criteria for ‘depressive disorder’: The DSM, ICD, and ICPC definitions will each capture a different group of patients suffering from depressive symptoms, and we cannot even agree whether anxiety and depression represents one disorder or two! RESUMO Objetivos: Estimar a prevalência da síndrome de burnout (SB) e fatores associados entre mé... more RESUMO Objetivos: Estimar a prevalência da síndrome de burnout (SB) e fatores associados entre médicos plantonistas de unidades de terapia intensiva (UTI) de São Luís-MA. Métodos: Participaram 60 médicos plantonistas de seis UTIs de São Luís-MA, investigados em setembro a dezembro de 2012, por meio de um questionário sociodemográfico abrangendo características condições de trabalho, fatores estressantes e do Maslach Burnout Inventory (MBI). Foram realizadas estatística descritiva e análise dos fatores associados à SB por meio do teste de Qui-quadrado e Razão de Prevalência. A integração desses cuidados com a Atenção Primária à Saúde (APS) é a principal estratégia segundo a OMS. Nesta direção, no Brasil, a criação do Núcleo de Apoio à Saúde da Família (NASF) foi um avanço, composto por uma equipe multidisciplinar que tem como principal ferramenta de trabalho o apoio matricial. Trata-se de um estudo transversal com 624 pacientes em 2012/2013 e aplicação dos instrumentos: “General Health Questionnaire”, “Hospital Anxiety and Depression Scale”, “Screening for Somatoform Symptoms”, “Alcohol Use Disorder Identification Test” e “World Health Organization Quality of Life Instrument (bref version)”. Realizou-se análise bivariada (Teste-t) e regressões lineares múltiplas para cada domínio de QV. Objective: To identify the associations among quality of life (QoL), social determinants and psyc... more Objective: To identify the associations among quality of life (QoL), social determinants and psychological distress in primary care in two cities in Brazil. Methods: A cross-sectional study with 1,466 patients from 2009 to 2010. The statistical analysis used the t-test to compare the variables of interest to the study. Results: The prevalence of Common Mental Disorders (CMD3), severe forms of Common Mental Disorders (CMD5), anxiety and depression were 20.5%, 32%, 37% and 25.1% respectively. Thes presence of psychological distress is associated with worse QoL among the patients studied, especially those older than 40 years of age. In cases of CMD3, those with higher income and educational levels presented higher QoL in the psychical and psychological domains. OBJETIVOS: investigar a existência de história de violência na vida de mulheres com queixa de dor... more OBJETIVOS: investigar a existência de história de violência na vida de mulheres com queixa de dor crônica, e que solicitam atendimento em um serviço especializado no atendimento à dor da Universidade Estadual do Rio de Janeiro, Brasil. A somatização foi um dos temas destacados para se pensar a ligação entre violência e dor crônica. MÉTODOS: estudo de casos por meio da história de vida de 11 pacientes, enfatizando suas histórias de violência e vivência da dor. RESULTADOS: observou-se grande tendência das pacientes em buscar causas físicas para suas dores e nessa procura elas peregrinam pelos serviços de saúde passando de um médico ao outro. Todas as entrevistadas relataram nunca terem falado sobre suas histórias de violência com nenhum profissional de saúde e ao fazê-lo associam sua dor não somente a ocorrências físicas, como também a sofrimentos emocionais. The main objectives of this study were to detect whether there is any special group of patients within the Family Health Strategy that should be considered to be in greater risk for common mental disorders and to recommend alternative interventions to aid these patients. METHOD: In 2002, a cross-sectional study on common mental disorders seen at Family Health Strategy centers was conducted in Petrópolis, State of Rio de Janeiro. RESULTS: Common mental disorders were associated with women (OR = 2.90; 95% CI 1.82-4.32), younger than 45 years of age (OR = 1.43; 95% CI 1.02-2.01), with a monthly per capita family income of less than US$40.00 (OR = 1.68; 95% CI 1.20-2.39), and without a partner (OR = 1.71; 95% CI 1.22-2.39). A study conducted as part of the development of the Eleventh International Classification of Ment... more A study conducted as part of the development of the Eleventh International Classification of Mental Disorders for Primary Health Care (ICD-11 PHC) provided an opportunity to test the relationships among depressive, anxious and somatic symptoms in PHC. Primary care physicians participating in the ICD-11 PHC field studies in five countries selected patients who presented with somatic symptoms not explained by known physical pathology by applying a 29-item screening on somatic complaints that were under study for bodily stress disorder. Patients were interviewed using the Clinical Interview Schedule-Revised and assessed using two five-item scales that measure depressive and anxious symptoms. Major changes from the ICD-10 primary care version have been proposed for: (i) mood and anxiety disorders; and (ii) presentations of multiple somatic symptoms (bodily stress syndrome). This three-part field study explored the implementation of the revised classification by primary care physicians (PCPs) in five countries. Participating PCPs in Brazil, China, Mexico, Pakistan and Spain were asked to use the revised classification, first in patients that they suspected might be psychologically distressed (Part 1), and second in patients with multiple somatic symptoms causing distress or disability not wholly attributable to a known physical pathology, or with high levels of health anxiety (Part 2). In this field study of WHO's revised classification of mental disorders for primary care sett... more In this field study of WHO's revised classification of mental disorders for primary care settings, the ICD-11 PHC, we tested the usefulness of two five-item screening scales for anxiety and depression to be administered in primary care settings. The study was conducted in primary care settings in four large middle-income countries. Primary care physicians (PCPs) referred individuals who they suspected might be psychologically distressed to the study. Screening scales as well as a structured diagnostic interview, the revised Clinical Interview Schedule (CIS-R), adapted for proposed decision rules in ICD-11 PHC, were administered to 1488 participants. A score of 3 or more on one or both screening scale predicted 89.6% of above-threshold mood or anxiety disorder diagnoses on the CIS-R. O objetivo deste artigo é identificar a associação entre variáveis demográficas, socioeconômicas, transtorno mental comum, sintomas sugestivos de ansiedade e depressão, EVPE com QV na Atenção Primária (AP). Estudo transversal realizado com 1.466 pacientes atendido na AP, nos munícipios de São Paulo e Rio de Janeiro, em 2009 e 2010. Realizou-se análise bivariada por meio do Teste-t e regressão linear múltipla para cada domínio de QV. Os escores de QV para os domínios físico, psicológico, relações sociais e meio ambiente foram, respectivamente de, 64,7, 64,2, 68,5 e 49,1. Objective: To identify the associations among quality of life (QoL), social determinants and psyc... more Objective: To identify the associations among quality of life (QoL), social determinants and psychological distress in primary care in two cities in Brazil. Methods: A cross-sectional study with 1,466 patients from 2009 to 2010. The statistical analysis used the t-test to compare the variables of interest to the study. Results: The prevalence of Common Mental Disorders (CMD3), severe forms of Common Mental Disorders (CMD5), anxiety and depression were 20.5%, 32%, 37% and 25.1% respectively. Thes presence of psychological distress is associated with worse QoL among the patients studied, especially those older than 40 years of age. In cases of CMD3, those with higher income and educational levels presented higher QoL in the psychical and psychological domains. For the cases of probable anxiety, those with higher educational levels presented lower scores on the physical and social relationship scores. OBJETIVOS: investigar a existência de história de violência na vida de mulheres com queixa de dor... more OBJETIVOS: investigar a existência de história de violência na vida de mulheres com queixa de dor crônica, e que solicitam atendimento em um serviço especializado no atendimento à dor da Universidade Estadual do Rio de Janeiro, Brasil. A somatização foi um dos temas destacados para se pensar a ligação entre violência e dor crônica. MÉTODOS: estudo de casos por meio da história de vida de 11 pacientes, enfatizando suas histórias de violência e vivência da dor. RESULTADOS: observou-se grande tendência das pacientes em buscar causas físicas para suas dores e nessa procura elas peregrinam pelos serviços de saúde passando de um médico ao outro. Todas as entrevistadas relataram nunca terem falado sobre suas histórias de violência com nenhum profissional de saúde e ao fazê-lo associam sua dor não somente a ocorrências físicas, como também a sofrimentos emocionais. Fez-se uma revisão bibliográfica crítica de cunho narrativo, cujos resultados abordam: a delimitação do paradoxo; as dificuldades pelas quais passariam os profissionais de saúde e seus pacientes para, respectivamente, disponibilizarem e solicitarem intervenções sobre padrões considerados problemáticos de consumo de álcool; as aplicações e limitações das técnicas motivacionais, como as intervenções breves, na APS, e, por fim, discutem-se algumas questões relativas à formação profissional em saúde. OBJECTIVE: Common mental disorders are present in more than 50% of patients attending primary car... more OBJECTIVE: Common mental disorders are present in more than 50% of patients attending primary care clinics. The main objectives of this study were to detect whether there is any special group of patients within the Family Health Strategy that should be considered to be in greater risk for common mental disorders and to recommend alternative interventions to aid these patients. METHOD: In 2002, a cross-sectional study on common mental disorders seen at Family Health Strategy centers was conducted in Petrópolis, State of Rio de Janeiro. RESULTS: Common mental disorders were associated with women (OR = 2.90; 95% CI 1.82-4.32), younger than 45 years of age (OR = 1.43; 95% CI 1.02-2.01), with a monthly per capita family income of less than US$40.00 (OR = 1.68; 95% CI 1.20-2.39), and without a partner (OR = 1.71; 95% CI 1.22-2.39).
