The article is intended to complement the exhibition catalogue Raphael: From Urbino to Rome. General features of Raphael's paintings include the use of panel supports, gesso grounds bound in animal glue, off-white colored imprimatura layers that are pigmented with lead white and lead-tin yellow. Raphael often transferred drawings from paper to panel by pouncing. Raphael also used the method of squaring to transfer drawings to panel. Evidence of Raphael's use of rulers and compasses is presented. The binding media of the paint layers has been identified by gas chromatography-mass spectrometry (GC-MS) as drying oil. The story of sterilisation is traced over the century. An attempt is made to understand the evolution of the legal aspects of sterilisation, from a rather paternalistic/eugenic standpoint in the early 1900's to a more humanistic one in the present day, as evidenced in contemporary court decisions. We present a prospective study of TTP cases from the United Kingdom Thrombotic Thrombocytopenic Purpura (UK TTP) Registry with clinical and laboratory data from the largest cohort of pregnancy-associated TTP and describe management through pregnancy, averting fetal loss and maternal complications. Thirty-five women presented with a first TTP episode during pregnancy: 23/47 with their first congenital TTP (cTTP) episode and 12/47 with acute acquired TTP in pregnancy. TTP presented primarily in the third trimester/postpartum, but fetal loss was highest in the second trimester. Fetal loss occurred in 16/38 pregnancies before cTTP was diagnosed, but in none of the 15 subsequent managed pregnancies. Seventeen of 23 congenital cases had a missense mutation, C3178T, within exon 24 (R1060W). There were 8 novel mutations. Commissioning mechanisms such as Payment by Results (PbR) have been established in acute health services. An outcome focussed version of PbR will be implemented nationally from 2012 in an incremental manner in mental health services for adults and older people. Though intellectual disability services are currently excluded, it is proposed that needs led approaches would improve the quality and efficiency of specialist intellectual disability services. This paper aims to suggest that this approach could be the key to commissioning and designing personalised pathways of care. Design/methodology/approach – Health needs are scoped and care pathways are defined as primarily consisting of Needs, Interventions and Outcomes. The mandated cluster groups to be used for PbR in adult mental health and older people's services are extended to cover the non overlapping needs of people with intellectual disability to provide an integrated framework of health needs usually met by specialist services. A framework of interventions is suggested and components of “assessment” and “therapeutic” activities are outlined. An outcome framework is described. A case example illustrates the application of these components to design a care pathway to provide a personalised, needs led service. Findings – It is possible to use the principles underlying PbR to commission personalised services of high quality, improved efficiency and thus greater value. Originality/value – The principles underlying PbR can be used to commission personalised pathways of care in intellectual disability services at a time when this approach is being extended to mental health services nationally.
