A pilot study to test the feasibility of topical tissue expanders to close the RFFF donor site. The setting was a tertiary care teaching hospital. This pilot study consisted of a series of 21 cases that used topical tissue expanders to preoperatively expand the forearm skin. This allowed primary closure of the donor site and, in some cases, closure with a full-thickness skin graft. The main outcome measure was the ability to close the RFFF donor site without the use of a partial-thickness skin graft from the lateral thigh. Secondary outcome measures were the postoperative appearance of the donor site wound and function of the forearm. Twenty-one cases were treated with the preoperative tissue expanders. Aggressive fibromatosis (or desmoid tumor) refers to a histologically benign but locally aggressi... more Aggressive fibromatosis (or desmoid tumor) refers to a histologically benign but locally aggressive lesion arising from musculoaponeurotic structures in various anatomic sites. Extra-abdominal desmoids represent about one third of all desmoid tumors; of these only about 11 to 15% arise in the head and neck. Desmoid tumors arising in the infratemporal fossa are exceedingly rare; to our knowledge only one such tumor has been reported in the literature. We present a desmoid tumor arising in the infratemporal fossa with intracranial extension in a twenty-seven year old male and review the literature on this rare condition.

Another option is the use of intratympanic gentamicin to effect a 'gentamicin labyrinthectomy.' Nedzelski and coworkers have previously reported an 83% complete elimination and 17% substantial reduction in vertigo using this method. We retrospectively reviewed 21 patients treated at our centre with intratympanic gentamicin between 1991 and 1995, all of whom fulfilled the criteria for reporting results in Meniere's therapy described in the American Association of Otolaryngology Committee on Hearing and Equilibrium 1995 guidelines. Our patients are presented using these guidelines, specifically examining four criteria: frequency of vertigo, word recognition, functional level, and hearing threshold. Computed tomography (CT) is a valuable imaging tool in the examination of neonates and infants wi... more Computed tomography (CT) is a valuable imaging tool in the examination of neonates and infants with nasal obstruction. At present, however, it is difficult to quantitatively evaluate the nasal fossa with CT as normative data and the relative significance of individual nasal fossa dimensions have not been established. A standardized CT image was proposed, and performed on a prospective cohort of 56 infants up to 1 year of age. A parental questionnaire was used to identify infants with nasal obstruction. Normative data for four nasal fossa dimensions are presented and analyzed. The statistical validity of these dimensions in the diagnosis of nasal obstruction was examined; only the maximal posterior bony diameter showed a significant difference between normal and nasally obstructed infants (t-test, P = 0.05). Examples of CT findings in the above-mentioned cases as well as past cases of "choanal stenosis' are demonstrated.

Previously, using the American National Health Interview Survey (NHIS) and a logistic regression ... more Previously, using the American National Health Interview Survey (NHIS) and a logistic regression analysis, we found that upper aerodigestive tract (UADT) cancer is correlated with low socioeconomic status (SES). The objective of this study was to determine if this correlation between low SES and cancer prevalence exists for other cancers. We again used the NHIS and employed education level as our main measure of SES. We controlled for potentially confounding factors, including smoking status and alcohol consumption. We found that only two cancer subsites shared the pattern of increased prevalence with low education level and decreased prevalence with high education level: UADT cancer and cervical cancer. UADT cancer and cervical cancer were the only two cancers identified that had a link between prevalence and lower education level. This raises the possibility that an associated risk factor for the two cancers is causing the relationship between lower education level and prevalence.

However, it is not known if the same is evident under a universal healthcare system such as that in Canada. We used data from the Canadian Thyroid Cancer Consortium, a large thyroid cancer registry that collects data from two major thyroid cancer referral centers (London, Ontario, and Halifax, Nova Scotia). We included patients who presented with thyroid cancer between 1998 and 2011. We determined age at presentation, sex, and thyroid cancer status using the American Joint Committee on Cancer (AJCC) staging criteria. Individuals' postal codes were used to retrieve data from the Canadian census for the years 1996, 2001, and 2006 to approximate household income. Ordered logistic regression was used to determine odds ratios of presenting with more advanced stage thyroid cancer as they relate to income, age, and sex.
