Dr. John R. Calamia is a Full Professor, and a Director of D-4 Honors Aesthetics for the Department of Cariology and Comprehensive Care, New York University College of Dentistry, New York City, New York. He has taught Restorative Dentistry and dental material science at NYU Dental Center for 41 years and is recognized both nationally and internationally as co-developer of Etched Porcelain Laminate Veneer Technology. His research, articles, and presentations on porcelain etching and Silane coupling agents led to his placement of the earliest recorded Etched Bonded Porcelain Veneers in October of 1982 and these restorations are still successful 39 years later. He is very active in research, teaching, and private practice and he describes this combination as the perfect marriage within the profession.

To evaluate in vivo the feasibility of utilizing Amalgambond adhesive agent alone for the treatme... more To evaluate in vivo the feasibility of utilizing Amalgambond adhesive agent alone for the treatment of cervical sensitivity on teeth that did not require a restoration. Nineteen subjects were selected from a group of patients reporting sensitivity at the cervical region of anterior or posterior teeth. The degree of sensitivity was determined by tactile sense (scraping an explorer over the surface area) or "fluid-shift" sensitivity determined by an air-blast from a syringe for 5 seconds. Patient reactions to stimuli were qualified as : (1) none; (2) slight; (3) moderate; and (4) severe. Only patients with at least two teeth experiencing moderate or severe sensitivity were used. Each of the patients had at least one sensitive tooth treated with Amalgambond and at least one untreated sensitive tooth to act as control. Results were recorded at 24 hours, 7 days, 1 and 6 months. At 6 months, 18 of 19 treated teeth recalled showed decreased sensitivity with 15 showing no sensitiv...
Even if a clinician possesses basic knowledge in esthetic dentistry and clinical skills, many cas... more Even if a clinician possesses basic knowledge in esthetic dentistry and clinical skills, many cases presenting in modern dental practices simply cannot be restored to both the clinician's and the patient's expectations without incorporating the perspectives and assistance of several dental disciplines. Besides listening carefully to chief complaints, clinicians must also be able to evaluate the patient's physical, biologic, and esthetic needs. This article demonstrates the use of a smile evaluation form designed at New York University that assists in developing esthetic treatment plans that might incorporate any and all dental specialties in a simple and organized fashion.

This study evaluated 115 etched porcelain laminate veneers placed by a single operator at New Yor... more This study evaluated 115 etched porcelain laminate veneers placed by a single operator at New York University Dental Center between 1982 and 1985. The restorations were divided into two groups. Group I consisted of 43 restorations placed without any tooth preparation; Group II consisted of 72 restorations placed using a chamfer preparation. The U.S.P.H.S. criteria was used to record cavosurface marginal discoloration, marginal integrity, and presence of caries. A clinical examination also revealed the number of debonds and/or the number of fractures of porcelain. A periodontal evaluation included the use of baseline and recall stone replicas of restored teeth to measure the height of the free gingival margin taken mid-facial to the incisal edge of the restoration. Any changes in gingival contour were also noted. Plaque and gingival indices were also recorded. The low fracture and debond rate found in the study combined with no incidence of caries seem to indicate that these restorat...

Twenty-one patients required Class V restorations because of erosion, abrasion, or decay at the cervical area. Each patient exhibited moderate or severe sensitivity and required replacement of tooth structure. The degree of sensitivity was determined by either tactile scraping of the tip of an explorer over the surface of the tooth or by an air blast from a syringe for 5 seconds. A total of 52 restorations were performed. According to phone conversations and questionnaires, 1 patient experienced continued sensitivity on 1 treated tooth for a duration of 1 week, but this sensitivity was gone at 1-month, 3-month, 6-month, and 1-year recalls. None of the other patients exhibited sensitivity at any phone checks or during recall visits.

PURPOSE To evaluate the microleakage of CAD-CAM porcelain inlay restorations cemented with four d... more PURPOSE To evaluate the microleakage of CAD-CAM porcelain inlay restorations cemented with four different dual cure resin cements. MATERIALS AND METHODS Thirty human extracted caries-free molar teeth were prepared with a Class II MOD cavity design. One proximal box ended on enamel and the other on root surface dentin. The teeth were divided into six groups of five samples each. Two groups were restored with a direct placement composite resin to serve as a control. The other four groups were restored with CAD/CAM-generated ceramic inlays that were cemented with four dual cure resin cements. All specimens were thermocycled. Microleakage of the restorations was assessed by dye penetration. RESULTS Compared to direct placement resin composite which demonstrated an excellent enamel marginal seal, CAD/CAM-generated porcelain inlays produced excellent marginal seals at both the enamel and dentin interfaces.

Only now are we beginning to see the fruits of this work with the commercial availability of some systems; the potential for this technology seems boundless. This article reviews the recent literature with emphasis on the period from June 1992 to May 1993. This review should familiarize the reader with some of the latest developments in this technology, including a brief description of some systems currently available and the clinical and economical rationale for their acceptance into the dental mainstream.
