So I can confirm the feature is working onwiki per this API result:
https://he.wikipedia.org/w/api.php?action=query&list=recentchanges&rctype=categorize

Looking at Special:RecentChanges these also appear to be showing up:

pasted_file (142×1 px, 52 KB)

This means they should also be appearing in peoples watch lists.

My preferences notifications boxes are marked all.

I'm not sure exactly what you mean here!
The feature is disabled by default, you have to go into your preferences and UNTICK the hide options for category changes.
Your preferences should look like this:

pasted_file (358×523 px, 32 KB)

(I believe I have highlighted the correct rows here)

(It might be possible that the translations for the user options are incorrect?

My preferences look exactly as you shown.
And I can see the catwatch changes in watchlist. But I'm not getting any notifications (echo message or alert).

There are no such notifications! / echo messages!!
