These patients usually require alternate routes of feeding during treatment. However, these methods of feeding are not without associated complications. This study was carried out to assess the complications associated with percutaneous endoscopic gastrostomy tube insertion, the commonest means of providing nutrition in this patient group.
Self-Expanding Metallic Stents (SEMS) are a surgery-sparing option for malignant colonic obstruct... more Self-Expanding Metallic Stents (SEMS) are a surgery-sparing option for malignant colonic obstruction. They can be inserted as a "bridge to surgery" (BTS) for potentially curable disease, or as a palliative measure, thereby avoiding the higher morbidity and mortality associated with surgery. The objective of this study was to report our local experience of left-sided colonic stents. Data on 49 patients was collected retrospectively from Oct 2008 to Nov 2014 at our cancer centre. This included demographics, baseline clinical characteristics, indications for SEMS placement (bridge to surgery/palliative), technical and clinical success, complications, and the mean patency of duration. Survival in both groups was also plotted on a Kaplan-Meier chart. A total of 49 patients with colorectal carcinoma (CRC) of the left side were enrolled. The mean age was 50 years (range 18-86). Ninety percent of patients had disease involving the rectum, sigmoid colon, or both. Forty-seven percent (n = 23) of patients had stent insertion as a BTS, whereas 53 % (n = 26) had the procedure with palliative intent. Technical and clinical success rates were 96 and 88 %, respectively. The clinical success rate of the palliative arm was lower than that of the BTS arm (p = 0.024). 87.5 % (n = 42) had no procedure-related complications. Technical failure, perforation, and stent migration/expulsion, were each observed in 4 % of cases. Mean stent patency duration was 83.9 days. SEMS insertion for left-sided malignant colonic obstruction is a safe and effective procedure when used either as a bridge to surgery or with palliative intent.
Tumour-induced osteomalacia (TIO) is a rare paraneoplastic syndrome usually presenting with bone ... more Tumour-induced osteomalacia (TIO) is a rare paraneoplastic syndrome usually presenting with bone pain, fracture of bones and muscle weakness. It is caused by high serum levels of fibroblast growth factor 23 (FGF- 23), which is a hormone-regulating phosphate, and vitamin D. FGF-23 is secreted by several tumours, especially benign mesenchymal tumours which are very small and difficult to locate. There is a significant delay from onset of symptoms to the diagnosis of this entity dueto occult nature of this disease. We present a case of young male who presented with long history of progressively worsening muscular pain and weakness, rendering the patient confined to bed. Our aim of presenting this patient as a case report is to make physicians realise that any patient with unexplained muscular weakness and pain must undergo workup for TIO, including serum phosphate measurement, as this is a rare but potentially curable disease.

Out of the total cropped area of 20 million hectares about 5 million hectares do not have any irrigation facility and completely depend on rains. In Punjab, barani areas contribute about 20 percent of cropped area. While in the Pothowar region, cropped area is over 90 percent. Livestock is an important part of the farming system. Feed resources, however, are scare and livestock has to survive on crop residues and on degraded range areas. Due to the low and unreliable rainfall of the region, yields of main crops are low and farm income mostly remains insufficient to maintain the family. Since, prospects for increasing cultivated area are limited due to irrigation water constraints, the strategy for sustainable food security and livelihood calls for significant increases in crop and livestock productivity through substantial enhancement of output per unit of land, animal and labour. To describe the etiology, functional status and short term outcome of patients with pituitary les... more To describe the etiology, functional status and short term outcome of patients with pituitary lesions (PL). Brain or pituitary MRI reports of 3753 patients were analyzed for PL over the period of 2000 to 2007, done at the Aga Khan University Hospital (AKUH), Karachi, Pakistan. MRIs with reported PL and all those ordered by Endocrinologists with or without pituitary abnormalities, were included in the analysis. This made a total of 338 (9%) MRI reports. PL were defined as pituitary tumors, cysts, haemorrhage, hypoplasia; and empty sella. Patients with these PL, were analyzed for symptoms, hormonal profile and short term outcome in the hospital retrospectively. In the analysis of 338 MRI reports, 23% had normal pituitary gland on MRI examination. Hypogonadotrophic hypogonadism was the commonest (38.5%) endocrine abnormality seen with normal pituitary MRI, followed by hypopituitarism (5.1%).
