    Introduction: We aimed in the present study to investigate the effects of pulmonary rehabilitation on the respiratory parameters and effort capacity, quality of life (QoL), anxiety, depression, and sleepiness in the patients with Parkinson's disease (PD). Methods: The study included 34 mobilized patients with PD. Patients underwent a 12-week pulmonary physiotherapy (PPT). Pulmonary function tests (PFT), six-minute walk test (6mWT), Hoehn and Yahr Scale (HYS), Unified Parkinson's Disease Rating Scale (UPDRS), QoL, Parkinson's Disease Questionnaire (PDQ), Beck Anxiety Inventory (BAI), Beck Depression Inventory (BDI), and Epworth Sleepiness Scale (ESS) scores were evaluated before and after PPT. Results: PPT improved parameters of PFT significantly, and caused an increase of about 35 m in the 6mWT (p<0.001).     Objective: The aim of this study was to investigate the efficacy of favipiravir (FVP) in severe COVID-19. Materials and methods: This is a retrospective study of 142 COVID-19 patients with severe pneumonia signs, who received inpatient treatment between March 15 and May 20, 2020. The patients were divided into two groups according to the use of FVP treatment; group 1 (n = 99) included patients who treated with FVP and group 2 (n = 43) who didn’t receive FVP. Results: Mean age was 66.47 ± 11.89 in group 1, and 68.58 ± 14.78 in group 2. Forty patients (40.4%) in group 1 and 22 (51.2%) in group 2 were treated in the intensive care unit (P > 0.05). The proportion of eosinophil, tendency of increasing thrombocyte counts and eosinophil/neutrophil ratio in FVP group was significantly higher than non-FVP group (p 0.05).     Ectopic adrenocorticotropic hormone (ACTH) syndrome is a rare cause of the Cushing's syndrome. The occurrence of the ectopic ACTH syndrome presenting with severe hypokalaemia, metabolic alkalosis, and hypertension has been highlighted in case reports. However, presentation with lower gastrointestinal perforation is not known. We report the case of a 70-year-old male patient with severe hypokalaemia, metabolic alkalosis, hypertension, and colonic perforation as manifestations of an ACTH-secreting small cell lung carcinoma. Ectopic ACTH syndrome should be kept in mind as a cause of hypokalaemia, hypertension, and intestinal perforation in patients with lung carcinoma.

    Conventional methods for the aetiological diagnosis of community-acquired pneumonia (CAP) are often insufficient owing to low sensitivity and the long wait for the results of culture and particularly serology, and it often these methods establish a diagnosis in only half of cases. To evaluate the most common bacterial and viral agents in CAP using a fast responsive PCR method and investigate the relationship between clinical/laboratory features and aetiology, thereby contributing to empirical antibiotic selection and reduction of treatment failure. In children aged 4-15 years consecutively admitted with a diagnosis of CAP, the 10 most commonly detected bacterial and 12 most commonly detected viral agents were investigated by induced sputum using bacterial culture and multiplex PCR methods. Clinical and laboratory features were compared between bacterial and viral pneumonia. In 78 patients, at least one virus was detected in 38 (48.7%) and at least one bacterium in 32 (41%). In addition, both bacteria and viruses were detected in 16 (20.5%) patients. Overall, the agent detection rate was 69.2%. The most common viruses were respiratory syncytial virus and influenza and the most frequently detected bacteria were S. pneumoniae and H. influenzae. PCR was superior to culture for bacterial isolation (41% vs 13%, respectively). Fever, wheezing and radiological features were not helpful in differentiating between bacterial and viral CAP. White blood cell count, CRP and ESR values were significantly higher in the bacterial/mixed aetiology group than in the viral aetiology group. In CAP, multiplex PCR is highly reliable, superior in detecting multiple pathogens and rapidly identifies aetiological agents. Clinical features are poor for differentiation between bacterial and viral infections. The use of PCR methods allow physicians to provide more appropriate antimicrobial therapy, resulting in a better response to treatment, and it may be possible for use as a routine service if costs can be reduced.

    Tracheobronchomegaly or Mounier-Kuhn syndrome is a rare disorder characterized by marked dilatation of the trachea and main bronchi, bronchiectasis, and recurrent respiratory tract infections. The etiology of this disorder is uncertain. It is thought to be due to defective development of elastic and muscle tissues in the upper airways. Patients usually present with symptoms related to infection and bronchiectasis. Symptoms can range from chronic cough to severe respiratory failure requiring mechanical ventilation. However, some patients may be asymptomatic. The diagnosis is usually made on the basis of the characteristic computerized thomography scan findings. Treatment was mainly supportive with chest physiotherapy and antibiotics during infectious exacerbations. 19 years old and 36 kg weigh male patient was admitted with shortness of breath and recurrent lung infections. Chest X-ray showed marked tracheal widening, bilateral pneumothorax, and bronchiectasis. Trachea diameter was measured of 27 mm in the thoracic tomography. Because of the rarity, we report the case of a patient who was diagnosed with Mounier-Kuhn syndrome with bilateral pneumothorax (a previously non reported association) and developing respiratory failure even with simple exercise.

    INTRODUCTION AND AIM: Community-acquired pneumonia (CAP), which is often seen in daily practice, is a lower respiratory tract and pulmonary parenchyma infection which develops in society and daily life with community-acquired pathogens in individuals with no known immune failure. Delay in the treatment of pneumonia is known to increase morbidity and mortality. Various scoring systems are currently used in the identification of treatment groups in pneumonia. With the aim of evaluating the approach to CAP cases, the infection. MATERIALS AND METHODS: Working Group of the Turkish Respiratory Research Association (TUSAD) prepared a 22-item questionnaire. RESULTS: The survey was published on the TUSAD official website between July 2013 and June 2016. A total of 78 individuals responded to the questionnaire on the website. CONCLUSION: The responses to the questionnaire could indicate the way forward for new guidelines for physicians in respect of the approach to CAP.

    Aim: The current study aimed to evaluate the factors affecting survival in non-small cell lung cancer invading the chest wall. Method: A total of 45 cases operated on for Non-Small Cell Lung Cancer invading the chest wall (NSCLC) were followed-up for five years. The effects of factors such as depth of tumor invasion of the chest wall (parietal pleura, extra pleural fatty tissue, intercostal muscles, and rib involvement), perinodal involvement, “N” involvement, surgical margin of the resection, and adjuvant chemotherapy on prognosis and survival were evaluated. Results and Discussion: The number of males and females among the cases was 38 (84%) and seven (16%), respectively, with a mean age of 55 ± 8 years (42-74).     Swyer-James Syndrome is a rare pulmonary disease, characterized by one-sided hyperlucency, smaller hilum shadow and lack of vascular structures in affected side. Although usually asymptomatic, can also be symptoms such as recurrent lung infection, exertional dyspnea and hemoptysis. 23-year-old male patient presented with exercise-induced dyspnea. On physical examination, decreased breath sounds on the left hemithoraxes. Chest x-ray showed right middle and lower lobe more prominent hyperlucency, mediastinal shift to the left and small hilar shadow. Ventilation-perfusion scintigraphy were determined consistent defect areas. Swyer-James Syndrome diagnosed by chest X-ray is possible, but it is unthinkable because of the rarity. Swyer-James syndrome diagnosed patient is presented with literature data.

    BACKGROUND/AIM Oxidative stress plays an important role in the pathogenesis of chronic obstructive pulmonary disease (COPD). Smoking is the leading source of oxidants in lungs. However, it is currently unknown why some individuals are more resistant to the detrimental effects of smoking and do not develop COPD. The aim in this study is to measure and compare the oxidant/antioxidant balance between in non-COPD individuals who smoke and COPD patients who smoke. MATERIALS AND METHODS Included in the study were 137 patients with COPD and 102 healthy individuals. Participants were divided into groups as COPD patients (former and current smokers), non-COPD individuals who smoke and non-smokers healthy persons. In the following stage, the total antioxidant status (TAS), total oxidant status (TOS) and oxidative stress index (OSI) levels were measured in serum for all participants. RESULTS In the current-smoker COPD group, the level of oxidant status were significantly higher than the former-smoker COPD group (p < 0.001). Similarly, oxidant levels were significantly high in current-smoker healthy group than never smoker healthy group. According to these results TOS was associated with especially smoking status rather than COPD. Antioxidant status were similar between former-smoker COPD group and current-smoker COPD group. The antioxidant levels were found significantly low in current-smoker COPD patients, compared to the current-smoker non-COPD individuals (p = 0.007). Nevertheless, no significant difference was found in OSI levels between two groups. Briefly, high TOS and OSI values were correlated with only smoking, independently from COPD. CONCLUSION It was concluded that there are complex pathogenetic mechanisms, including genetic and individual variations other than oxidant/antioxidant balance, involved in the development of smoking-related COPD. TOS and OSI values are not predictive parameters for the development of COPD, but high level of TAS in non-COPD smokers is promising for future studies.

    Introduction: One day point prevalence study to evaluate patient profile in ICUs was planned by Turkish Thoracic Society Respiratory Failure and Intensive Care Group. Methods: The study was cross-sectional and data were obtained by a questionnaire in participating ICUs on October 26th 2016 for 24 hours. A questionnaire evaluating characteristics of patients in ICU was prepared. A total of 78 units declared to participate in the survey. Results: Ten centers were excluded from the study because they couldn’t participate on study day. A total of 922 patients were enrolled from 68 units. The mean age was 64.8 ± 17.9 (17-99), 58.6% were male. The mean APACHE II, QSOFA and GCS scores on the study day were 18.5 ± 8.8, 1.4 ±1.4 and 10.2 ± 4.3, respectively, and mean length of stay (LOS) was 20 ± 70 days. The patients were admited from emergency room(44%[n=407]), clinics(39%[n=359]) and other ICUs(17%[n=156]).38.5% of patients had Type 1 respiratory failure(RF),35.6% had Type 2 RF. Comorbidity was seen in 80.6% and malignancy rate was 18.8%. On the study day it was found that 52.1% of the patients underwent invasive mechanical ventilation(IMV),12.3% of the patients had non-invasive ventilation(NIV) 29.1% of them had oxygen therapy. Tracheostomy was performed in 19.7% of patients. 70% of the patients had enteral, 20.7% had parenteral nutrition. The discharge rate was 7% while mortality was 1.1% on that day. Discussion: Most of the patients in ICUs are respiratory failure cases admited from emergency department and clinics. Advanced age, presence of comorbidities and cancer are thought to prolong LOS in ICU. For effective use of ICU beds further regulations for patient admission criteria is necessary.

    Oxidative stress is one of the major causes of methotrexate induced lung injury (MILI). Alpha-lipoic acid (ALA), which occurs naturally in human food, has antioxidative and anti-inflammatory activities. The aim of this study was to research the potential protective role of ALA on MILI in rats. Twenty one rats were randomly subdivided into three groups: control (group I), methotrexate (MTX) treated (group II), and MTX+ALA treated (group III). Lung injury was performed with a single dose of MTX (20 mg/kg) to groups 2 and 3. On the sixth day, animals in all groups were sacrificed by decapitation and lung tissue and blood samples were removed for histological examination and also measurement the levels of interleukin-1-beta (IL-1β), malondialdehyde (MDA), glutathione (GSH), tumour necrosis factor-alpha (TNF-α), myeloperoxidase (MPO), and sodium potassium-adenosine triphosphatase (Na+/K+ATPase). In MTX group tissue GSH, Na+/K+ATPase activities were lower, tissue MDA, MPO and plasma IL-1?, TNF-? were significantly higher than the other groups. Histopathological examination showed that lung injury was less severe in group 2 according to group 3. Oxidative damage of MTX in rat lung is partially reduced when combined with ALA.

    Objectives. Exposure to ambient metals and air pollutants in urban environments has been associated with impaired lung health and inflammation in the lungs. Fractional exhaled nitric oxide (FeNO) is a reliable marker of airway inflammation. In this study, we aimed to compare the FeNO levels of three schools that have different distances from iron and steel industry zone for assessing the effects of heavy metals and air pollution on their respiratory health. Methods. Pulmonary function test and FeNO measurements were evaluated in 387 adolescents in three schools which have different distance from plant. Results. FeNO levels were significantly higher in School I (n=142; 18.89±12.3 ppb) and School II (n=131; 17.68±7.7 ppb) than School III (n=114; 4.28±3.9 ppb). Increased FeNO concentration was related to the distance of iron and steel industry zone in young adults. Conclusion.     Tracheal tumors are very rare, and commonly malignant. Benign tracheal tumors, constitute only 1.9% of all lung tumors. Diagnosis of tracheal tumors is difficult. Clinical symptoms of asthma show similar characteristics due to easily mimic. In routine radiologic examinations is often inadequate the diagnosis. Those with airway obstruction caused is often incorrectly diagnosed with asthma and are treated in this direction. Our case 5 years from shortness of breath on exertion, complaints of wheezing and asthma medications are used. But had not seen for treatment despite receiving appropriate treatment and regular on the lack of improvement and tracheal obstruction was thought to evaluating the differential diagnosis. Bronchoscopic examination showed tracheal tumor. Tumor was removed endoscopically and complete clinical recovery was achieved. This article is not all wheezing respiratory asthma and not provided with appropriate treatment of patients with clinical improvement should be considered in the differential diagnosis of tracheal tumors and we present a case showing.

    Swyer-James Syndrome is a rare pulmonary disease, characterized by one-sided hyperlucency, smaller hilum shadow and lack of vascular structures in affected side. Although usually asymptomatic, can also be symptoms such as recurrent lung infection, exertional dyspnea and hemoptysis. 23-year-old male patient presented with exercise-induced dyspnea. On physical examination, decreased breath sounds on the left hemithoraxes. Chest x-ray showed right middle and lower lobe more prominent hyperlucency, mediastinal shift to the left and small hilar shadow. Ventilation-perfusion scintigraphy were determined consistent defect areas. Swyer-James Syndrome diagnosed by chest X-ray is possible, but it is unthinkable because of the rarity.
