Introduction: Nowadays, several indicators are used to evaluate scientific research outputs. The h index is one of the most common indices taken into account by many authorities to assess individuals and institutions, worldwide. In this study, the attitudes of medical sciences researchers towards significance of factors affecting the h index and citations were investigated.
Methods: At the beginning of 2008, h-indices of all academic members of Iranian medical universities were screened based on the Scopus database. The attitudes of the faculty members who had an h index of one or more were investigated through a self-structured questionnaire in which increasing factors of the h index and paper citations was assessed. The data were analyzed using statistical methods.
Results: The r esponse rate was about 37%. The respondents’ attitudes toward the items were not different between the the group whose h-index was more than 6 to the other group whose h-index was less than 6. They respondents believed that publishing in high Impact Factor journals, well-designed researches considering the methodology, and to be the 1st study of its subject in the country and the world, are the most important factors in achieving citations. They did not believe that publishing review articles, contributing internationally, and having frequent paper topics were important in gaining citations.
Conclusion: It seems that our researchers were not aware of the importance of publishing review articles and also in Open Access journals to increase citation rates. Proper interventions should be conducted to correct this attitude in order to increase the country’s and researchers’ citations.

Information is the symbol of the present age due to the significant development in accessing, processing, storage and transferring information. Information societies have been formed by the widespread utilization of information and communication technologies in human social life and generally focus on the computer systems and information networks. Nowadays, various technologies of medical informatics comprise an important component of the management infrastructures of health care systems. Medical informatics is the development and assessment of specific methods and systems for acquisition, processing and analyzing patients' data with the help of knowledge and information from scientific researches. Moreover, it intends to increase access, improve quality and decrease the costs of care through decreasing chronological and geographical limitations. On the other hand, ethics have been always considered as a basic component of these systems. The increasing development of digital technologies and their application in health information management provides numerous benefits; however, it encounters health care managers with new challenges in the information societies. These challenges may has been mainly caused by the conflicts among ethical principles by themselves or disregarding them in the field of medical informatics. Therefore, careful consideration of info ethics as well as beneficence, autonomy, fidelity and justice is essential to overcome those conflicts in the information societies.

Introduction: Evidence based practice is an approach to clinical practice which emphasizes the use of best clinical researches as a component of clinical decision making. The aim of this study was to determine the percentage of current clinical decision making and disease management which is based and supported by high-level evidence in an educational hospital located in Isfahan, Iran. Methods: A cross-sectional study was carried on 103 patients who were admitted during one month to seven Internal medicine wards in Alzahra hospital. For each patient, the primary diagnosis and primary treatment were recorded. Evidence based resources were then searched for randomized controlled trials (RCT's), systematic reviews and clinical practice guidelines to find the best research evidence that the treatments were effective. The main outcome measure was the level of evidence which supports the primary intervention for the primary diagnosis of each patient. Finding: Level I evidence (at least one randomized trial) supported the primary intervention used in 68.9% of internal admissions and level II evidence (convincing non-experimental evidence) supported the primary intervention in 31.1% of admissions. None of patients received intervention with level III supporting evidence. Conclusion: Most patients had received high level supported interventions. It seems that we should repeat this study in other wards. Also it may be recommended to design some educational evidence based practice for non academic physicians.

Electronic Health Record (EHR) is a repository of patient health information shared among multiple authorized users. As a modern method of storing and processing health information, it is a solution for improving quality, safety and efficiency of patient care and health system. However, establishment of EHR requires a significant investment of time and money. While many of healthcare providers have very limited capital, application of open source software would be considered as a solution in developing national electronic health record especially in countries with low income. The evidence showed that financial limitation is one of the obstacles to implement electronic health records in developing countries. Therefore, establishment of an open source EHR system capable of modifications according to the national requirements seems to be inevitable in Iran. The present study identifies the impact of application of open source software in developing national electronic health record in Iran.
