We determined the efficacy of atropine sulfate combined with papaverine hydrochloride, prostaglan... more We determined the efficacy of atropine sulfate combined with papaverine hydrochloride, prostaglandin E1 and phentolamine mesylate in the pharmacological erection test. A total of 230 consecutive patients with erectile dysfunction was randomized for pharmacological erection test and received an intracorporeal combination of 50 mg. papaverine hydrochloride, 10 micrograms. prostaglandin E1, 0.2 mg. phentolamine mesylate and 0.075 mg. of atropine sulfate (group 1), or the same combination without atropine sulfate (group 2). Erectile response was evaluated subjectively and by intracorporeal pressure measurement. In group 1, 40 patients (35.1%) showed only tumescence, and 22 (19.3%) had poor erection. In group 2, 45 patients (39.5%) had tumescence and 17 (14.9%) poor erection. In both groups 52 patients (45.6%) had rigid erection. There was no statistically significant difference regarding erectile response and intracorporeal pressure. The addition of atropine sulfate did not improve results of the pharmacological erection test when combined with 50 mg. papverine hydrochloride, 10 micrograms, prostaglandin E1, and 0.2 mg. phentolamine mesylate.

An independent population-based study was carried out. A self-applicative questionnaire covering urinary symptoms, coping strategies, quality of life and treatment seeking behavior was developed. A total of 848 subjects between 15 and 55 years completed the questionnaire. The overall prevalence of OAB was 18.9%. Women were significantly more affected than men (p = 0.001). All age groups were equally affected (p = 0.152). Subjects with OAB reported significant impairment on household chores (p = 0.009), physical activities (p = 0.016), sleep (p < 0.001), work (p < 0.001), social life (p < 0.001) and sexual life (p < 0.001). In addition, OAB individuals present higher prevalence of depression (p = 0.036), anxiety (p < 0.001), shame (p < 0.001) and tiredness (p < 0.001) OAB was independently associated to sexual life impairment (OR = 3.36, 95% CI=1.20-9.39). To determine if dutasteride-treated men can be monitored safely and adequately for prostate cance... more To determine if dutasteride-treated men can be monitored safely and adequately for prostate cancer based on data from the Reduction by Dutasteride in Prostate Cancer Events (REDUCE) study. To analyse whether the use of treatment-specific criteria for repeat biopsy maintains the usefulness of prostate-specific antigen (PSA) level for detecting high grade cancers. The REDUCE study was a randomized, double-blind, placebo-controlled investigation of whether dutasteride (0.5 mg/day) reduced the risk of biopsy-detectable prostate cancer in men with a previous negative biopsy. The usefulness of PSA was evaluated using biopsy thresholds defined by National Comprehensive Cancer Network guidelines in the placebo group and any rise in PSA from nadir (the lowest PSA level achieved while in the study) in the dutasteride group. This chapter reviews the possible effectiveness of oral agents as an option for treatment of Peyr... more This chapter reviews the possible effectiveness of oral agents as an option for treatment of Peyronie’s disease (PD). We present the rationale for use of these drugs and base our recommendations on levels of published evidence and the report by the International Consultation on Urological Diseases. A definitive and ideal medical therapy for PD has yet to be established and will not be until we completely understand the trigger mechanism and the maintenance process of fibrosis in the tunica albuginea. Several oral substances have been proposed with promises of success; however, these agents appear to lack robust scientific support for their benefit. Until new evidence is available, the indications for oral treatment in PD are recent onset of disease, painful plaque, and unstable plaque. An early trial of inexpensive, safe, and well-tolerated oral therapy may be offered with limited enthusiasm for a positive outcome.

Both patients presented with bilateral cryptorchidism, 1 was seen for gynecomastia and 1 was seen for an abdominal mass that was found to be a testicular tumor.
