Resumo Objetivo avaliar o impacto econômico domiciliar do adoecimento pela tuberculose no Brasil. Método pesquisa transversal multicêntrica de 2016 a 2018, em cinco capitais do Brasil, em pacientes diagnosticados com tuberculose. Os custos totais foram a soma dos custos diretos e indiretos incorridos antes e durante o diagnóstico e tratamento. A regressão logística foi utilizada para estudar determinantes de custos catastróficos. A pobreza foi medida como renda familiar per capita diária < U$ 5,5. Resultados trezentos e sessenta e um pacientes foram inscritos no estudo. O custo extrapolação foi de R$3.664,47 (DP: R$ 2.667,67) e o custo total de extrapolação foi de R$22.291,82 (DP: R$ 16.259,50). No geral, 29% dos participantes do estudo foram caracterizados como pobres antes da tuberculose, e 39% depois. Em média, a renda diminuiu em 11% dos participantes e 41% tiveram custos catastróficos. Objective: To estimate the prevalence of tuberculosis in professional nursing and analyze the profile of these professionals in the period 2000 to 2008 in the Espirito Santo State-Brazil. Methods: We conducted a descriptive study used secondary data from databases of morbidity due to tuberculosis and registration nursing professional of Espirito Santo State (COREN). We included all records of patients with tuberculosis were reported in the Health Information System for Diseases Reported (SINAN-TB) for the years 2000 to 2008 and all registered nursing staff in the COREN from May 1975 to July 2008. Multidrug-resistant tuberculosis (MDR-TB) is a threat for the global TB epidemic control. Despite existing evidence that individualized treatment of MDR-TB is superior to standardized regimens, the latter are recommended in Brazil, mainly because drug-susceptibility tests (DST) are often restricted to first-line drugs in public laboratories. We compared treatment outcomes of MDR-TB patients using standardized versus individualized regimens in Brazil, a high TB-burden, low resistance setting. The 2007-2013 cohort of the national electronic database (SITE-TB), which records all special treatments including drug-resistance, was analysed. OBJECTIVE To evaluate the impact of catastrophic costs on unfavorable tuberculosis treatment outcomes. METHODS This was a prospective cohort study, conducted in five Brazilian state capitals (Manaus, Recife, Vitória, Campo Grande and Porto Alegre) from June 2016 to July 2018. Logistic regression was used to calculate the odds ratio (OR) and 95% confidence intervals (95%CI). RESULTS Of the 350 participants, 310 were included, of whom 30 presented unfavorable outcomes. Catastrophic cost (OR=2.53 -95% CI 1.13;5.67) and divorce (OR=5.29 -95% CI 1.39;20.05) increased the chances of unfavorable outcomes. CONCLUSION Financial difficulties during tuberculosis treatment may impair its outcome. Catastrophic cost and divorce were determining factors for treatment outcomes.

Household contacts of pulmonary tuberculosis (TB) patients are at increased risk of TB infection and disease. However, their risk in relation to the intensity of exposure remains unknown.We studied smear-positive TB cases and their household contacts in Vitória, Brazil. We collected clinical, demographic and radiographic information from TB cases, and obtained tuberculin skin test (TST) and QuantiFERON-TB Gold (QFT) results from household contacts. We measured intensity of exposure using a proximity score and sleep location in relation to the TB index case and defined infection by TST ≥10 mm or QFT ≥0.35 UI·mL-1 We ascertained secondary TB cases by reviewing local and nationwide case registries.We included 160 TB index cases and 894 household contacts. 464 (65%) had TB infection and 23 (2.6%) developed TB disease. Este artigo tem por objetivo determinar a prevalência da infecção pelo Mycobacterium tuberculosis em acadêmicos de enfermagem e medicina da Universidade Federal do Espírito Santo. Estudo de corte transversal no qual os dados foram coletados através de questionário, composto de questões abertas e fechadas sobre características pessoais; informações a respeito da tuberculose; utilização de medidas preventivas, etc. Aplicou-se teste tuberculínico, com leitura após 72h por enfermeiros treinados, considerando como ponte de corte positivo 10 mm de enduração. Participaram 225 voluntários, sendo 98 estudantes da Medicina e 127 da Enfermagem. A positividade ao teste tuberculínico fora encontrada em 54 estudantes de ambos os cursos, resultando uma prevalência de 24%. Entre os Estudantes de Enfermagem o resultado do TST positivo foi obtido em 24% e nos Estudantes de Medicina em 23%, sendo esta diferença não significativa estatisticamente. Este estudo analisa o conhecimento dos agentes comunitários de saúde no controle da tuberculose e a autopercepção do seu nível de conhecimento e de sua importância no enfrentamento da doença, no Município de Vitória, Espírito Santo, Brasil. Tratou-se de um estudo de corte transversal. Um questionário semi-estruturado, auto-aplicável e pré-testado foi preenchido por 105 agentes comunitários de saúde randomicamente selecionados. A comparação das proporções entre os grupos formados pela estratificação por tempo de serviço foi realizada usando-se o teste qui-quadrado com nível de significância de 5%. A idade média foi de 34,5 (± 9,7) anos. Atuavam há três anos ou menos 66 agentes comunitários de saúde (62,9%). Observou-se que um maior tempo de atividade está relacionado com um aumento do nível de compreensão em torno da doença, bem como das atividades efetivamente realizadas no controle da tuberculose. OBJETIVO: Descrever os fatores associados ao abandono de quimioprofilaxia de TB em maiores de 15 anos atendidos em programas de referência de controle da doença. MÉTODOS: Realizou-se um estudo de coorte histórica com análise de prontuários preenchidos entre 2002 e 2007 nos programas de referência de controle da doença no município de Vitória (ES). Os casos de infecção por Mycobacterium tuberculosis foram estratificados em dois grupos- profissionais de saúde (grupo PS) e indivíduos não profissionais de saúde (grupo NPS). RESULTADOS: Um total de 395 indivíduos foi incluído no estudo: 35 no grupo PS e 360 no grupo NPS. A média de idade nos grupos PS e NPS foi de 34,8 e 32,4 anos, respectivamente (p = 0,36). A maioria de pacientes no grupo PS eram mulheres (29; 82,9%), enquanto 180 pacientes no grupo NPS eram mulheres (50,0%). OBJETIVO: Traçar o perfil epidemiológico dos casos notificados de tuberculose entre os profissionais de saúde no Programa de Controle da Tuberculose do Hospital Universitário Cassiano Antônio de Moraes, em Vitória (ES) Brasil. MÉTODOS: Realizou-se um estudo descritivo e retrospectivo de dados secundários no período de 2002 a 2006. RESULTADOS: Foram notificados 25 casos de profissionais de saúde com tuberculose: 8 técnicos de enfermagem (32%); 4 médicos (16%); 3 enfermeiros (12%); 2 técnicos em radiologia (8%) e 8 profissionais pertencentes a outras categorias (32%); 14 do sexo masculino (56%) e 11 do sexo feminino (44%). A faixa etária com maior incidência foi a de 35-39 anos. A forma clínica predominante foi a extrapulmonar com 12 casos (48%), seguido da pulmonar com 11 casos (44%), e 2 casos (8%) apresentaram ambas as manifestações. An interferon-γ release assay, QuantiFERON-TB (QFT) test, has been introduced an alternative test for the diagnosis of latent Mycobacterium tuberculosis infection (LTBI). Here, we compared the performance of QFT with tuberculin skin test (TST) measured at two different cut-off points among primary health care work (HCW) in Brazil. A cross-sectional study was carried out among HCWs in four Brazilian cities with a known history of high incidence of TB. Results of the QFT were compared to TST results based on both ≥5 mm and ≥10 mm as cut-off points. We enrolled 632 HCWs. When the cut-off value of ≥10 mm was used, agreement between QFT and TST was 69% (k = 0.31), and when the cut-off of ≥5 mm was chosen, the agreement was 57% (k = 0.22). We investigated possible factors of discordance of TST vs QFT. Compared to the TST-/QFT- group, risk factors for discordance in the TST+/QFT- group with TST cut-off of ≥5 mm included age between 41-45 years [OR = 2.70; CI 95%: 1.32-5.51] and 46-64 years [OR = 2.04; CI 95%: 1.05-3.93], BCG scar [OR = 2.72; CI 95%: 1.40-5.25], and having worked only in primary health care [OR = 2.30; CI 95%: 1.09-4.86]. On the other hand, for the cut-off of ≥10 mm, BCG scar [OR = 2.26; CI 95%: 1.03-4.91], being a household contact of a TB patient [OR = 1.72; CI 95%: 1.01-2.92] and having had a previous TST [OR = 1.66; CI 95%: 1.05-2.62], were significantly associated with the TST+/QFT- group. No statistically significant associations were found among the TST-/QFT+ discordant group with either TST cut-off value. Although we identified BCG vaccination to contribute to the discordance at both TST cut-off measures, the current Brazilian recommendation for the initiation of LTBI treatment, based on information gathered from medical history, TST, chest radiograph and physical examination, should not be changed.

Estudo retrospectivo de perfil dos casos de TBMR do Espírito Santo, entre 2000 e 2004. Identificou-se 61 pacientes com TBMR, sendo a amostra composta por 57 casos, que tiveram seus prontuários revisados para obtenção das variáveis estudadas. Estimou-se como prevalência para a TBMR combinada 0,87% (0,66 - 1,13; IC95%). O teste anti-HIV foi feito em 78,9% dos casos, sendo 11,1% positivos. Investigando co-morbidades, destacaram-se etilismo e tabagismo. Encontrou-se 11 casos de resistência primária (RI) e 46 de resistência adquirida (RA), com média de 2,3 ± 1,3 tratamentos anteriores. Em 35,1% dos casos houve relato de contato prévio conhecido com doente de tuberculose, enquanto em 67,9% não houve ou nega-se ter havido. Dez pacientes (17,5%) foram tratados com o esquema de 1ª linha, 18 (31,6%) com o de 2ª linha e 27 (47,4%) com o de 3ª linha. RESUMO O objetivo deste estudo foi verificar se conhecimento insuficiente sobre a tuberculose é um fator determinante do abandono do tratamento. Um estudo caso-controle foi realizado com pacientes em tratamento para tuberculose, sendo casos definidos como pacientes em re-tratamento para tuberculose devido a abandono de terapêutica instalada anteriormente, e controles os pacientes em curso do primeiro tratamento. A amostra foi composta por 68 pacientes, sendo 17 casos e 51 controles. Sexo masculino esteve significantemente asssociado com o re-tratamento (77% dos casos e 50% dos controles eram do sexo masculino). Não houve diferença significativa entre casos e controles com relação a escolaridade, tendo sido identificada um nível baixo de escolaridade na maioria dos entrevistados. Com relação ao conhecimento sobre transmissão e tratamento da doença não houve diferença significativa entre casos e controles. OBJECTIVE: To compare tuberculosis cure rates among patients supervised by household members or health care workers. METHODS: Prospective cohort study of 171 patients treated by the program in Vitoria, Southeastern Brazil, from 2004 to 2007. Each patient was followed-up for six months until the end of the treatment. Of the patients studied, a household member supervised 59 patients and healthcare workers supervised 112 patients. Patients' sociodemographic and clinic data were analyzed. Differences between groups were assessed using chi-square test or Student's t-test. Significance level was set at 5%. RESULTS: Most patients had smear positive, culture confirmed pulmonary tuberculosis. Two patients were HIV-positive. There were more illiterate patients in the healthcare-supervised group, in comparison to those supervised by their families (p=0.01). OBJECTIVE To analyze the prevalence and factors associated with multidrug-resistant tuberculosis in Espírito Santo, Brazil. METHODS This is a cross-sectional study of cases of tuberculosis tested for first-line drugs (isoniazid, rifampicin, pyrazinamide, ethambutol, and streptomycin) in Espírito Santo between 2002 and 2012. We have used laboratory data and registration of cases of tuberculosis – from the Sistema Nacional de Agravos de Notificação and Sistema para Tratamentos Especiais de Tuberculose. Individuals have been classified as resistant and non-resistant and compared in relation to the sociodemographic, clinical, and epidemiological variables. AIDS among older adults is a public health problem emerging. This study compared the demographic and epidemiological profile and the evolution and trend of the epidemic among men and women aged 50 years and older and 20 to 39 years affected by AIDS in the State of Espirito Santo, Brazil. We conducted a time serie study with secondary data from SINAN/AIDS for the period January 1991 to December 2006. In this period were 3,382 reported cases of AIDS in individuals aged 20 to 39 years and 551 cases among individuals with 50 years or older. In both age groups most affected are men. There are differences related to race or color, where the majority of the older is white (45.3% - p-value = 0.044) and the young brown (44.7%, p = 0.003). Illiteracy prevails among the older (17.7% - p-value = 0.001). More than half (80%) of the notifications occurred in cities of medium to large. The degree to which tuberculosis (TB) is transmitted between persons is variable. Identifying the factors that contribute to transmission could provide new opportunities for TB control. Transmission is influenced by host, bacterial and environmental factors. However, distinguishing their individual effects is problematic because measures of disease severity are tightly correlated, and assessing the virulence of Mycobacterium tuberculosis isolates is complicated by epidemiological and clinical confounders. To overcome these problems, we investigated factors potentially associated with TB transmission within households. We evaluated patients with smear-positive (≥2+), pulmonary TB and classified M. tuberculosis strains into single nucleotide polymorphism genetic cluster groups (SCG). We recorded index case, household contact, and environmental characteristics and tested contacts with tuberculin skin test (TST) and interferon-gamma release assay. Households were classified as high (≥70% of contacts with TST≥10 mm) and low (≤40%) transmission. We used logistic regression to determine independent predictors. From March 2008 to June 2012, we screened 293 TB patients to enroll 124 index cases and their 731 contacts. There were 23 low and 73 high transmission households. Index case factors associated with high transmission were severity of cough as measured by a visual analog cough scale (VACS) and the Leicester Cough Questionnaire (LCQ), and cavitation on chest radiograph. SCG 3b strains tended to be more prevalent in low (27.3%) than in high (12.5%) transmission households (p = 0.11). In adjusted models, only VACS (p<0.001) remained significant. SCG was associated with bilateral disease on chest radiograph (p = 0.002) and marginally associated with LCQ sores (p = 0.058), with group 3b patients having weaker cough. We found differential transmission among otherwise clinically similar patients with advanced TB disease. We propose that distinct strains may cause differing patterns of cough strength and cavitation in the host leading to diverging infectiousness. Larger studies are needed to verify this hypothesis.

INTRODUÇÃO: A AIDS entre adultos mais velhos é um problema de saúde pública emergente. Comparamos o perfil epidemiológico e sociodemográfico, bem como a evolução e a tendência da epidemia entre homens e mulheres nas faixas etárias de 50 anos e mais e 20 a 39 anos acometidos pela AIDS, no Estado do Espírito Santo, Brasil. MÉTODOS: Realizamos um estudo de série temporal, com dados secundários do SINAN/AIDS, no período de Janeiro de 1991 a dezembro 2006. RESULTADOS: Neste período, foram notificados 3.382 casos de AIDS em indivíduos com idade entre 20 e 39 anos e 551 casos entre indivíduos com 50 anos e mais. Em ambas as faixas etárias, os mais acometidos são os homens. Há diferenças referentes à raça/cor, em que a maioria dos mais velhos são brancos (45,3% - p-valor = 0,044) e os mais jovens pardos (44,7%, p-valor = 0,003). On the other hand, for the cut-off of ≥10 mm, BCG scar [OR = 2.26; CI 95%: 1.03-4.91], being a household contact of a TB patient [OR = 1.72; CI 95%: 1.01-2.92] and having had a previous TST [OR = 1.66; CI 95%: 1.05-2.62], were significantly associated with the TST+/QFT- group. No statistically significant associations were found among the TST-/QFT+ discordant group with either TST cut-off value. Although we identified BCG vaccination to contribute to the discordance at both TST cut-off measures, the current Brazilian recommendation for the initiation of LTBI treatment, based on information gathered from medical history, TST, chest radiograph and physical examination, should not be changed.

RESUMO O objetivo deste estudo foi verificar se conhecimento insuficiente sobre a tuberculose é um fator determinante do abandono do tratamento. Um estudo caso-controle foi realizado com pacientes em tratamento para tuberculose, sendo casos definidos como pacientes em re-tratamento para tuberculose devido a abandono de terapêutica instalada anteriormente, e controles os pacientes em curso do primeiro tratamento. A amostra foi composta por 68 pacientes, sendo 17 casos e 51 controles. Sexo masculino esteve significantemente asssociado com o re-tratamento (77% dos casos e 50% dos controles eram do sexo masculino). Não houve diferença significativa entre casos e controles com relação a escolaridade, tendo sido identificada um nível baixo de escolaridade na maioria dos entrevistados. Com relação ao conhecimento sobre transmissão e tratamento da doença não houve diferença significativa entre casos e controles.
