In a prospective study of 115 patients with systemic meningococcal disease, 61 control patients a... more In a prospective study of 115 patients with systemic meningococcal disease, 61 control patients and 293 population controls, environmental and other factors which preceded the illness and which might have influenced the acquisition and case fatality rate of the meningococcal disease were investigated. Passive smoking in children under 12 year of age, stressful events, and symptoms and signs of preceding illness within the last two weeks were significantly more frequent in meningococcal patients than among the population controls. In contrast, those patients who had been exposed to stressful events, or who had symptoms or signs of ill-health preceding the meningococcal disease, had significantly reduced case fatality rates as compared to those who had not had any such experiences. Passive smoking remains a factor of great interest for further studies and intervention.
Paired sera from 10 patients and a convalescence sample from one patient suffering from campyloba... more Paired sera from 10 patients and a convalescence sample from one patient suffering from campylobacteriosis were analysed for IgG, IgM and IgA antibodies against the infective organism (Campylobacter jejuni PEN 0:6,7) with a DIG-ELISA system. Either formalinized, ethanol-inactivated or heat-inactivated preparations of the infecting organism were used as antigens. Cross reactivity was tested with human sera having agglutinating antibodies against Yersinia enterocolitica (N = 6) or Salmonella typhi or S. parathyphi b (N = 7). All patients displayed IgA and IgM levels in the convalescence sample above that found in healthy blood donors (N = 55). Using the ethanol-inactivated or formalinized preparations more than 90% of the convalenscence sera showed IgG levels above that found in blood donors whereas the heat-inactivated preparation detected 73% IgG positives in the same group of sera. During the ten-year period 1975-84, 251 patients were hospitalized because of self poisoning in N... more During the ten-year period 1975-84, 251 patients were hospitalized because of self poisoning in Nordmøre, the northern part of Møre and Romsdal county. During the five last years, there was a large increase in hospital admittances because of self poisoning, especially of persons below the age of thirty. Men dominated in the age group 20-30 years, and women in the age group 15-20 years. There was a significant correlation between the number of unemployed and the number of self poisonings through out the ten-year period.

Fundamental aspects of the lifestyle of Mycobacterium tuberculosis implicate DNA metabolism in ba... more Fundamental aspects of the lifestyle of Mycobacterium tuberculosis implicate DNA metabolism in bacillary survival and adaptive evolution. The environments encountered by M. tuberculosis during successive cycles of infection and transmission are genotoxic. Moreover, as an obligate pathogen, M. tuberculosis has the ability to persist for extended periods in a subclinical state, suggesting that active DNA repair is critical to maintain genome integrity and bacterial viability during prolonged infection. In this chapter, we provide an overview of the major DNA metabolic pathways identified in M. tuberculosis, and situate key recent findings within the context of mycobacterial pathogenesis. Unlike many other bacterial pathogens, M. tuberculosis is genetically secluded, and appears to rely solely on chromosomal mutagenesis to drive its microevolution within the human host. In turn, this implies that a balance between high versus relaxed fidelity mechanisms of DNA metabolism ensures the maintenance of genome integrity, while accommodating the evolutionary imperative to adapt to hostile and fluctuating environments. The inferred relationship between mycobacterial DNA repair and genome dynamics is considered in the light of emerging data from whole-genome sequencing studies of clinical M. tuberculosis isolates which have revealed the potential for considerable heterogeneity within and between different bacterial and host populations.

Eight phenotypically homogeneous Moraxella-like strains were isolated from the nasal flora of hea... more Eight phenotypically homogeneous Moraxella-like strains were isolated from the nasal flora of healthy goats. Total genomic DNA-DNA hybridization, DNA base composition determination, and genetic transformation studies were performed to determine the relationships of these bacteria to the classical moraxellae. The eight new isolates exhibited very high levels of genetic affinity to Moraxella bovis, as shown by quantitative and qualitative genetic transformation data, and exhibited high DNA-DNA relative binding ratios to each other (63% or more) but lower levels of DNA homology with all of the other species investigated, including the closely related classical moraxellae. Our results, combined with the general morphologic and phenotypic profiles of these organisms, indicate that they should be classified with the classical moraxellae, and we propose the name Moraxella caprae for them. Strain 8897 (= CCUG 33296 [corrected] = NCTC 12877) is the type strain of M. caprae.

During the one-year period of study, 37 patients were admitted to the medical department, Kristia... more During the one-year period of study, 37 patients were admitted to the medical department, Kristiansund hospital, because of self poisoning. Mixed intoxications dominated, especially combinations of benzodiazepines and alcohol. One patient died. Interpersonal conflicts were stated as the main reason for the self poisoning. Five patients were considered suicidal. 27 patients were followed up six months later. A majority considered both their social and mental situation to be better than at the time of the self poisoning episode, and had reduced consumption of alcohol and drugs.
We present a 50-year-old female who experienced generalized convulsion 3 months after a successfu... more We present a 50-year-old female who experienced generalized convulsion 3 months after a successful cadaveric renal transplantation. The first cerebral CT scan indicated cerebral frontal infarction. Repeat CT some days later revealed progressive lesions, and a highly malignant tumor or abscess was suspected. Antifungal and broad-spectrum antibacterial therapy was initiated. Cerebral MRI could not differentiate between these conditions, but a neutrophil granulocyte scan strongly suggested an infectious process. A stereotactic puncture of the frontal lobe was followed by temporary improvement. A severe progressive left-sided hemiparalysis gave indication for a craniotomy with evacuation of the abscess 9 days later. Culture of aspirated pus yielded growth of a gram-positive, rod-shaped bacterium, later identified as Nocardia otitidiscaviarum by sequencing the 16S rRNA. The patient was treated with meropenem plus rifampicin intravenously for 6 weeks followed by oral ciprofloxacin and rifampicin for 2 months. Due to pharmacokinetic interaction with rifampicin, the prednisolone dose was doubled, and the dose of tacrolimus had to be tripled for maintenance of adequate trough concentrations. Five months following cessation of antibiotic treatment, the patient has regained normal strength and function in her left-sided extremities and has a serum creatinine level of about 160 micromol/l (1.8 mg/dl).
