Dr. Hatim A. Omar is Professor of Pediatrics and Obstetrics and Gynecology and Chief of Adolescent Medicine & Young Parent Program at the University of Kentucky in Lexington. Dr. Omar obtained his medical training at the Medical Academy, Sofia, Bulgaria, completed a residency in Obstetrics and Gynecology from Second United City Hospital, Sofia, Bulgaria and Pediatrics from West Virginia University, Morgantown, West Virginia and received postgraduate training in Adolescent Medicine at West Virginia University and Physiology at New York Medical College in Valhalla, New York. Dr. Omar is board certified in the areas of Pediatrics and Adolescent Medicine. He is the founder & Chairman of the Stop Youth Suicide Campaign. He served as a member of Executive committee of the Section on Adolescent Health of the American Academy of Pediatrics and as Board of directors’ member of the North American Society for Pediatric and Adolescent Gynecology. Dr. Omar has published extensively in all areas of care for adolescents in various peer-reviewed scientific journals, books and many media outlets. He is an associate editor or on the editorial Board of the Journal of Pediatric and Adolescent Gynecology, International Journal of Adolescent Medicine & Health, Frontiers in Child Health & Human Development and 4 other peer reviewed journals. Dr Omar has served as an ambassador for wellbeing of adolescents world wide and is highly regarded for his tireless volunteer work on behalf of adolescents. Subjects were early and middle adolescent males and females recruited from a psychiatric clinic (n = 77) and two general pediatric clinics (n = 131). SS was measured by using the Sensation Seeking Scale for Children. Pubertal development was measured with a modified Pubertal Development Scale that was completed by the adolescent and his/her parent about the adolescent. Adolescent self-reports of nicotine, alcohol, and marijuana use were also obtained using questionnaires. SS was higher in males and females who reported nicotine and alcohol use and in males who reported marijuana use. SS was positively associated with pubertal development in males and females, even when controlling for age. Furthermore, SS mediated the relationship of pubertal development and drug use in males and females. The observation that SS mediates the relationship between pubertal development and drug use in males and females may contribute to understanding changes in drug use that are seen during adolescence. In addition, SS is associated with drug use and is easily measured in a variety of clinical settings.

This study examined the association of Sensation Seeking and symptoms of Disruptive Disorders and... more This study examined the association of Sensation Seeking and symptoms of Disruptive Disorders and investigated the associations of each with the risk of nicotine, alcohol, and marijuana use in 11- to 14-yr.-old boys and girls from psychiatric and pediatric clinics (n = 206). Sensation Seeking and symptoms of Disruptive Disorder were significantly related, and both were associated with drug use. In particular, Sensation Seeking was strongly associated with drug use in early and mid-adolescents seen in a clinical setting. Measurement of Sensation Seeking and symptoms of Disruptive Disorder should help identify and characterize youth who are at increased risk for drug use during early and mid-adolescence-a time when onset of use may be a harbinger of long-term habit.

PURPOSE: to assess knowledge of pediatric residents attending adolescents, including ethical issu... more PURPOSE: to assess knowledge of pediatric residents attending adolescents, including ethical issues on contraceptive advice.METHODS: A prospective study was conducted involving 75 residents in order to assess their knowledge, along with ethical issues involved, in clinical attendance of adolescents. The SPSS 13 software package was used for data analysis.RESULTS: 100% of interviewees stated that adolescents have the right to privacy during medical appointments. Yet, when asked from what age this privacy should be granted, 62% reported at any age. When questioned on whether an adolescent could be examined alone, without another person with them in the room, 50% of physicians answered yes and 50% no. 91.7% of interviewees felt that adolescents had the right to confidentiality and medical secrecy, but only 84.8% knew the contents of the Code of Medical Ethics regarding medical secrecy. A further 79.2% felt prepared to give advice on contraception to adolescents.CONCLUSIONS: The attitudes held by pediatric residents on privacy, confidentiality and medical secrecy are highly ambiguous. The doctors interviewed were ill-prepared to deal with the issues of contraception in adolescence on both practical and legal levels.

In the presence of increased extracellular Ca2+, both of these responses were enhanced in endothelium-intact arteries. Nitro-L-arginine, a blocker of the biosynthesis of endothelium-derived relaxing factor (EDRF), abolished the initial endothelium-dependent relaxation and Ca(2+)-induced enhancement of hypoxic contraction in endothelium-intact arteries but did not alter responses in endothelium-denuded vessels. Inhibition of prostaglandin formation with indomethacin had no effect on the hypoxia-elicited responses. Preincubation with LY 83583, an inhibitor of guanylate cyclase activation, abolished the initial hypoxia-elicited relaxation and subsequent contraction. M & B 22948, a guanosine 3',5'-cyclic monophosphate (cGMP) phosphodiesterase inhibitor, decreased tone under O2 but not under N2, causing an apparent enhancement of the contraction to hypoxia. Thus the modulation of hypoxic responses by the endothelium is dependent on changes in EDRF production, and a decrease in smooth muscle cGMP not involving an EDRF mechanism appears to mediate the endothelium-independent hypoxic contraction observed in the isolated rat pulmonary artery.
