I Determinanti Sociali della Salute Mentale rappresentano le condizioni attraverso le quali le persone hanno maggior o minor probabilità di godere di una buona salute mentale in virtù della propria posizione sociale.
October 10, 2015 a study day took place in Turin organized by the DUPP Working Group (Diritti Uma... more October 10, 2015 a study day took place in Turin organized by the DUPP Working Group (Diritti Umani Psicologi Piemonte-Human Rights Psychologists Piedmont) of the Order of the Psychologists of Piedmont on the issue The Right to Health. One thing appeared particularly significant: the very low number of psychologists called to intervene when compared to the large number of psychiatrists and psychotherapists of medical training. Starting from this simple, painful, indisputable consideration the objective we have set is threefold: the first is to propose a reflection on the positioning of italian psychology today, in particular clinical psychology, with respect to the theme of the right to health and of the inequalities in health, characterized, on a superficial observation, by a scarcity of psychologists in this field. The second objective is to briefly illustrate, from what emerged during the seminar (despite everything promoted by a Working Group of the Order of Psychologists), those processes and forces that shape the conditions in which people are born, grow up, grow old, are more or less likely to get sick and die. Finally, in the light of these considerations, the third objective, the main one, will be to try and outline the role that psychologists and psychology could play in protecting the right to health. What information could we obtain, starting from the difficulty in finding psychologists able to articulate with expertise, starting from their own work, a debate about the relationship between the right to health and psychology? Benasayag, psychoanalist and philosopher, can help us with the answer by asking if the current decline of psychoanalysis is not due to a difficulty of the discipline in recognizing the challenges posed by what he calls "modern man's suffering". He speaks of a state of being out of phase with respect to the postmodern period of psychoanalysis, arguing that the result of this being out of phase has been the failure of a historic goal for psychoanalysis caused on the one hand by the fall of the discipline into the individualistic trap, and on the other hand by the construction of a universal model, rendered substantial and reified by the psychic apparatus. Can such considerations come to our aid in replying to our question? Freud can help us further towards a possible answer. In Civilization and Its Discontents (1971) he observes how the clinician tends to turn his back on the great movements of History (in his case the reference is to the Great War and the Great Depression) by severing the threads that bind the individual to his context. These observations, though formulated in very different historical periods, appear to be more relevant than ever. If on one hand, even following the studies on social determinants of health (WHO, 2008), the epidemiological evidence grows exponentially regarding the role that inequalities play in shaping the lives of the population and individuals, on the other psychology does not seem to be prepared yet, lacking the epistemological, theoretical, methodological and operational tools, in order to act with authority in this scenario, so in fact it is often ineffective in protecting the right to health. Even in the face of compelling evidence on the role of mental health in determining a more general state of health (the title of an article published in the Lancet, 2007 is emblematic: Prince, Martin, and others "No health without mental health), the apparent disinterest of psychology concerning inequalities might find a mitigating factor when referring to non-psychic manifestations of suffering, with respect to which it is given for granted the impact of the "distal" determinants on different exposure to risk factors, on the vulnerability to unfavorable effects of such factors on health, on the unjust vulnerability in respect of the social consequences of the disease. (Costa and others 2014). This apparent indifference results, however, quite frankly bizarre in the light of the evidence related to the comorbidity of psychiatric disorders with pathologies of other kinds and the increasing amount of data that underscores the crucial role of inequalities in the etiopathogenesis of mental diseases. (Compton and others 2015; WHO, 2014). Such detachment from the reality of the epidemiological scene would lead some fanatic of classification to give a diagnosis of a frankly psychotic context. But while this epidemiological evidence begins to proliferate and consolidate within a coherent and worrying international picture, in the psychological field, where the importance of a bio-psycho-social approach is reaffirmed in a loud voice, we are witnessing the emergence of a multiplicity of theories and practices that continue to rotate around a strictly biomedical paradigm and where suffering is represented as a purely individual phenomenon. All this production of knowledge and practices aimed at the aspired overcoming of a rigid cartesian dualism that separated body and mind, however continues to have the individual as a unit of analysis and intervention. The contribution of psychoneuroendocrinoimmunology was very important to knowledge relative to the psycho-biological replies to stress, of EMDR in the treatment of trauma as well as sensorimotor psychotherapy. Come può lo psicologo lavorare con consapevolezza rispetto ai determinanti sociali della salute in linea con quanto prevede l'OMS e con le più recenti evidenze scientifiche provenienti dall'epidemiologia sociale sulle disuguaglianze sociali e di salute? I quaderni sul Diritto alla Salute e Diritto alla Cura, ad opera del Gruppo di Lavoro Diritti Umani Psicologi Piemonte, dell'Ordine degli Psicologi del Piemonte, hanno come obiettivo di fornire un quadro teorico-operativo che possa aiutare a posizionarsi rispetto a questi interrogativi.

Tuttavia, nonostante gli indubbi benefici per individui, società e sistemi sanitari della partecipazione come principio democratico alcune sue forme pratiche inevitabilmente comprendono rischi, criticità e contraddizioni, spesso misconosciuti. Questo contributo ha due obiettivi: da una parte, evidenziare l’associazione tra affermazione del paradigma neoliberista e partecipazione, iscrivendo contingenti forme partecipative nel contesto storico-culturale in cui si sono sviluppate e che hanno contribuito a determinare, e, dall’altra, descrivere lo stato dell’arte della patient advocacy in Italia.

Ciò che lega le pratiche mutualistiche alla salute mentale di individui e comunità sembra risiedere nel campo dei rapporti di reciproca cittadinanza. Se “salute mentale”, a livello individuale, indica anche la possibilità di ciascun cittadino di contribuire alla vita sociale (il ché può anche essere l’opposto di un suo mero adattamento) le pratiche mutualistiche sembrano costituirne un ingrediente essenziale. Promuovono il benessere nella comunità per chi è vettore e destinatario degli interventi facilitando partecipazione, capitale sociale e supporto, tutti ingredienti, a livello sia individuale che ecologico, fondamentali per la salute fisica e mentale. Inoltre, di per sé rappresenta un elemento per la valutazione dello stato di salute delle comunità e la sua capacità di questa di produrre pratiche che promuovano e distribuiscano salute e benessere. La malattia mentale nell’individuo passa sullo sfondo attraverso il mutuo riconoscimento del valore preventivo e curativo dei rapporti di cittadinanza per la salute mentale di tutti e tutte, di cui tutte e tutti siamo, almeno in minima parte, ogni giorno, in ogni ambito, responsabili.

Costituisce la naturale conseguenza della gestazione di teorie e pratiche che risalgono alla fine degli anni ’60 in Lapponia (“il trattamento adattato al bisogno” di Alanen). Il gruppo di ricerca finlandese diretto da Alanen iniziò a perseguire, sin dal 1968, l’obiettivo di sviluppare un modello di trattamento psichiatrico pubblico destinato a pazienti schizofrenici e alle loro famiglie caratterizzato da estreme adattabilità e flessibilità (di metodi e strumenti).

Introduzione Partiamo dallo stato dell'arte, i cosiddetti dati. Mettiamo tra parentesi, per il mo... more Introduzione Partiamo dallo stato dell'arte, i cosiddetti dati. Mettiamo tra parentesi, per il momento, che ogni dato sia il risultato di una rilevazione effettuata con strumenti e metodologie prodotte all'interno di un contesto sociale, che ogni "dato" sia in realtà "preso" da un osservatore portatore di un sapere collettivamente costruito, che sia catturato e diffuso con determinate intenzioni, per qualche interesse, specificatamente umano, per indirizzare in qualche modo la società, e abbia perciò un valore profondamente politico. Dopo aver messo tutto ciò tra parentesi rivolgiamoci all'oggetto che tenteremo di catturare, la salute mentale, ricordando quanto questo bene preziosissimo, inevitabilmente, sfugga a qualsiasi tentativo di oggettificazione. Tuttavia solo attraverso una riduzione a dato misurabile, quantificabile e comparabile, possiamo analizzare il modo in cui salute e malattia si distribuiscono all'interno del corpo sociale, cambiando nel tempo, nello spazio, distribuendosi in maniera peculiare nei diversi gruppi sociali. Tali caratteristiche, per quanto non riconducibili al livello individuale, non possono essere trascurate nel tentativo di rispondere in maniera appropriata alla domanda posta da ogni bisogno di salute di ciascun individuo. Lo scenario internazionale. Partiamo quindi dai dati. Cosa possiamo dire riguardo le condizioni di salute mentale della popolazione italiana? Per inquadrare il fenomeno occorre collocarlo all'interno del contesto dello scenario internazionale che ne costituisce lo sfondo. Il "Global Burden of Disease"(GBD), il più ampio studio epidemiologico osservazionale su scala globale afferma che il peso dei disturbi psichiatrici e psicologici, nel mondo, rappresenta il 13% di tutti i problemi di salute e il 32,4% della disabilità legata alla salute (Vigo 2016). Per la comparazione delle diverse malattie e dei diversi fattori di rischio, l'unità di misura utilizzata dal Global Burden of Disease, commissionato dalla Banca Mondiale, comprende due parametri. Gli anni di vita vissuta con disabilità conseguente ad una malattia e quelli perduti a causa di morte prematura per via di questa. Il grande peso delle malattie mentali, nel suo insieme, va attribuito più agli anni di vita vissuti con disabilità che a quelli di vita perduta (rappresentano il 30% di tutti gli anni vissuti con disabilità nel mondo ma solo l'1,4% di tutte le morti premature) per quanto, per specifiche fasce di età come ad esempio quella dei giovani adulti, sembra aumentare il peso legato alle morti premature in relazione all'aumentato rischio di suicidalità. Di fronte a questa preoccupante situazione le testate giornalistiche riportano con allarmata e allarmante regolarità l'aumento del peso della depressione sulle nostre società riprendendo, ad esempio, il grido dell'OMS che stima che entro il 2030 questa rappresenterà la malattia che contribuirà maggiormente al carico di disabilità in tutto il mondo superando tutte le malattie cardiovascolari.

L'istituzione psichiatrica continua a rappresentare un determinante sociale di salute potenzialmente iatrogeno. Solo ri-contestualizzando Basaglia in uno spazio più ampio di quello tecnico dei servizi di salute mentale dentro cui è stato " internato " , alienato da sé, è possibile rinnovarne il potenziale trasformativo. Basaglia è chiaro: si tratta di una partita che va giocata all'esterno dell'istituzione psichiatrica, pena un affievolimento delle forze necessarie al cambiamento.

Mettiamo tra parentesi, per il momento, che ogni dato sia il risultato di una rilevazione effettuata con strumenti e metodologie prodotte all'interno di un contesto sociale, che ogni "dato" sia in realtà "preso" da un osservatore portatore di un sapere collettivamente costruito, che sia catturato e diffuso con determinate intenzioni, per qualche interesse, specificatamente umano, per indirizzare in qualche modo la società, e abbia perciò un valore profondamente politico. Dopo aver messo tutto ciò tra parentesi rivolgiamoci all'oggetto che tenteremo di catturare, la salute mentale, ricordando quanto questo bene preziosissimo, inevitabilmente, sfugga a qualsiasi tentativo di oggettificazione. Tuttavia solo attraverso una riduzione a dato misurabile, quantificabile e comparabile, possiamo analizzare il modo in cui salute e malattia si distribuiscono all'interno del corpo sociale, cambiando nel tempo, nello spazio, distribuendosi in maniera peculiare nei diversi gruppi sociali. Tali caratteristiche, per quanto non riconducibili al livello individuale, non possono essere trascurate nel tentativo di rispondere in maniera appropriata alla domanda posta da ogni bisogno di salute di ciascun individuo. Lo scenario internazionale. Partiamo quindi dai dati. Cosa possiamo dire riguardo le condizioni di salute mentale della popolazione italiana? Per inquadrare il fenomeno occorre collocarlo all'interno del contesto dello scenario internazionale che ne costituisce lo sfondo. Il "Global Burden of Disease"(GBD), il più ampio studio epidemiologico osservazionale su scala globale afferma che il peso dei disturbi psichiatrici e psicologici, nel mondo, rappresenta il 13% di tutti i problemi di salute e il 32,4% della disabilità legata alla salute (Vigo 2016). Per la comparazione delle diverse malattie e dei diversi fattori di rischio, l'unità di misura utilizzata dal Global Burden of Disease, commissionato dalla Banca Mondiale, comprende due parametri. Gli anni di vita vissuta con disabilità conseguente ad una malattia e quelli perduti a causa di morte prematura per via di questa. Il grande peso delle malattie mentali, nel suo insieme, va attribuito più agli anni di vita vissuti con disabilità che a quelli di vita perduta (rappresentano il 30% di tutti gli anni vissuti con disabilità nel mondo ma solo l'1,4% di tutte le morti premature) per quanto, per specifiche fasce di età come ad esempio quella dei giovani adulti, sembra aumentare il peso legato alle morti premature in relazione all'aumentato rischio di suicidalità. Di fronte a questa preoccupante situazione le testate giornalistiche riportano con allarmata e allarmante regolarità l'aumento del peso della depressione sulle nostre società riprendendo, ad esempio, il grido dell'OMS che stima che entro il 2030 questa rappresenterà la malattia che contribuirà maggiormente al carico di disabilità in tutto il mondo superando tutte le malattie cardiovascolari.

L'istituzione psichiatrica continua a rappresentare un determinante sociale di salute potenzialmente iatrogeno. Solo ri-contestualizzando Basaglia in uno spazio più ampio di quello tecnico dei servizi di salute mentale dentro cui è stato " internato " , alienato da sé, è possibile rinnovarne il potenziale trasformativo. Basaglia è chiaro: si tratta di una partita che va giocata all'esterno dell'istituzione psichiatrica, pena un affievolimento delle forze necessarie al cambiamento.

La malattia mentale nell’individuo passa sullo sfondo attraverso il mutuo riconoscimento del valore preventivo e curativo dei rapporti di cittadinanza per la salute mentale di tutti e tutte, di cui tutte e tutti siamo, almeno in minima parte, ogni giorno, in ogni ambito, responsabili.
La salute fisica e mentale è un bene troppo prezioso per essere affidato a tecnici o al mercato. La partecipazione attiva di ciascuno e di tutti alla costruzione di una società più sana costituisce un elemento fondamentale per la tutela e la promozione della salute.
