Tracheostomy is one of the most frequent procedures performed in intensive care unit (ICU) patients. Of the many purported advantages of tracheostomy, only patient comfort, early movement from the ICU, and shorter ICU and hospital stay have significant supporting data. Even the belief of increased safety with tracheostomy may not be correct. Various techniques for tracheostomy have been developed; however, use of percutaneous dilation techniques with bronchoscopic control continue to expand in popularity throughout the world. Tracheostomy should occur as soon as the need for prolonged intubation (longer than 14 d) is identified. Accurate prediction of this duration by day 3 remains elusive. Mortality is not worse with tracheotomy and may be improved with earlier provision, especially in head-injured and critically ill medical patients. BACKGROUND: Tracheostomy is electively performed in critically ill patients requiring prolonged respiratory support. The risk of transporting, the increasing associated cost and operative room schedule are some of the obstacles for wider acceptance of this procedure. The use of rigid selection criteria exclude many patients who would benefit of this approach. OBJECTIVE: To determine the safety of open bedside tracheostomy (OBT) as a routine intensive care units (ICU) procedure without any selection criteria, considering its peri and postoperative complications. METHOD: Retrospective medical chart review of all patients that underwent elective tracheostomy between April 1999 and December 2005 at ICU of three private hospitals. RESULTS: The study group comprised 552 patients with a mean age of 69.6 ± 15.8 years. The incidence of significant complications (until 30 days after the procedure) was 4.34% (24 cases): 9 minor bleeding, 9 major bleeding, 2 subcutaneous emphysema, 4 stomal infections. Late complications were: laryngotracheal stenosis in 2 and tracheoinomminate fistula in 1 patient. CONCLUSIONS: OBT seems to be a safe and simple procedure, when performed by a team of experienced physicians under controlled circumstances, and should be considered as an option for ICU patients.

Tracheostomy is a life saving procedure of placement of a surgical airway. It is imperative for every medical personnel to be conversant with it as it helps secure airway, the first step in resuscitation when necessary. It is not only thorough knowledge of the anatomy of the neck and procedural technique but also the awareness of the unusual challenging situations likely during this procedure that can help avoid complications and enable one to be better prepared for any eventuality. This chapter aims to draw the attention to the likely challenges during tracheostomy including pediatric tracheostomy and percutaneous dilatational tracheostomy. An encounter with pseudoneurysm of the internal carotid artery helps understand the gravity of the likely challenges that a surgeon must be prepared to manage.

La ville antique de Narbonne était reliée à une vaste lagune par le cours de l'Aude, nécessitant pour le commerce une navigation fluvio-lagunaire et des opérations de transferts de marchandises des bateaux de haute mer vers des embarcations fluviales. Autour des étangs, plusieurs sites sont considérés par l'historiographie comme des avant-ports. Or, excepté le site de Mandirac identifié comme l'embouchure du fleuve, il s'agit d'établissements littoraux dont le rôle le dans le système portuaire restait à préciser. Deux sites ont fait l'objet de recherches récentes dans cet objectif : Saint-Martin à Gruissan et Port-la-Nautique à Narbonne. Le premier est situé à l'entrée de la lagune narbonnaise, le second en fond d'étang et proche de l'ancienne embouchure de l'Aude. Une approche interdisciplinaire a permis de renouveler la documentation nécessaire pour préciser leurs fonctions.
