Aerial firefighters operating single-engine air tankers (SEATs), carry up to 800 gallons of waters and are used for quick responses and multiple passes to pop-up fires (USDA Forest Service & USDI Buerau of Land Management, 2002).

RESUMO: Esta pesquisa documental tem finalidade exploratória e foca o ocorrido em falhas do motor em voo, no âmbito dos Serviços Regionais de Investigação e Prevenção de Acidentes Aeronáuticos (SERIPA). Foram utilizados dados originários dos “Relatórios Finais” dos processos de investigação conduzidos pelos SERIPA e disponíveis no website do Centro de Investigação e Prevenção de Acidentes Aeronáuticos (CENIPA). A investigação de acidentes e incidentes aeronáuticos aponta a “Falha do Motor em Voo” como a principal tipificação de tais ocorrências, responsável por 21,83% de acidentes no decênio “2004-2013”, segundo o “Panorama Estatístico da Aviação Civil Brasileira”. Tal documento, emitido pelo CENIPA em 29 de julho de 2014, permite uma ampla visão das ocorrências aeronáuticas em termos de: tipos de aviação envolvida, fatores que contribuíram para os eventos e regionalização dos mesmos, dentre outros. Destes percentuais, verifica-se que a maioria dos casos de falha do motor situa-se na aviação privada e de instrução, principalmente. Da mesma forma, as ocorrências concentram-se em aeronaves equipadas com motores a pistão ou convencionais. Os resultados apontam, ainda, que o aspecto manutenção, em tais motores, foi preponderante para que a falha do grupo motopropulsor se constituísse como primeiro evento na cadeia de acontecimentos que levou ao acidente ou incidente grave. Foram realizados estudos qualitativos, tendo como fulcro os processos de investigação conduzidos pelos SERIPA, cujo produto documental encontra-se disponível no repositório “Relatórios Finais”, no website do CENIPA. Destaque é dado para as investigações de acidentes concentradas em 2013, nos Estados de São Paulo e Mato Grosso do Sul. Nesse prisma, práticas inseguras de manutenção são reveladas, promovendo-se então uma visão do que ocorreu com tais aeronaves, quanto à manutenção desses motores. Dessa maneira, o panorama dessas recentes ocorrências pode propiciar ao ente regulador subsídios para o implemento de políticas de fiscalização e de orientação, em atendimento a uma filosofia de Gerenciamento de Riscos, e em acréscimo às Recomendações de Segurança emitidas pelo CENIPA, enquanto órgão executivo do Sistema de Investigação e Prevenção de Acidentes Aeronáuticos (SIPAER).
ABSTRACT: This documental research has an exploratory purpose, steering to in in-flight engine failure events, carried on by Regional Services of Investigation and Prevention of Aeronautical Accidents (SERIPA). It is used data from "Final Reports" of the investigation procedures, conducted by SERIPA, and available on the Center of Investigation and Prevention of Aeronautical Accidents (CENIPA) website. The investigation of aircraft accidents and incidents points to "In-flight Engine Failure" as the main classification of such occurrences, accounting for 21.83% and 13.93% of the total, respectively, for accidents and serious incidents in the "2004-2013 Statistical Overview of the Brazilian Civil Aviation". Such document, issued by CENIPA on July 29th, 2014, displays a wide range of aviation occurrences in terms of: types of aviation involved, factors that contributed to the events and regionalization, among others. Over these figures, we found that that most part of engine failure cases is located in private and instruction aviation. Likewise, events are concentrated in aircraft equipped with reciprocating engines. Results also show that the maintenance aspect, in such engines, has been predominant for the failure, being this failure constituted as the first event in the chain of events that led to the accident or serious incident. Qualitative studies were made, aiming the research conducted onto SERIPA, whose documentary product is available in the "Final Reports" repository, on CENIPA website. Accident investigations emphasized are concentrated in 2013, in the States of São Paulo and Mato Grosso do Sul. In this perspective, unsafe maintenance practices are revealed, promoting a vision of what occurred with such aircraft, and the maintenance of these engines, as well. Thus, the outlook of these recent events may grant the regulator board means to implement enforcement and guidance policies, in addition to the Safety Recommendations issued by CENIPA as the executive body of the System of Investigation and Prevention of Aeronautical Accidents (SIPAER).
