Perioperative Nursing

The author considers that whilst nurse tutors teach theory, nurse practitioners go on nursing in the same way that they always have done. There appears to be a chasm dividing theory and practice. This paper describes an attempt to leap that chasm. The paper identifies the problem, inherent in the nursing process that creates such difficulties in its implementation. The nursing process alone does not provide an adequate framework for nursing practice. The paper then describes how the translation of theory into practice took place on a ward in Brighton General Hospital, Brighton, England. The barrier between theory and practice is highlighted, along with a description of how the problems this presents to ward staff were overcome. The author considers that the attribution of cause is a key concept in the problem-solving approach, since it generates the question — Why?— and therefore that attribution of cause must be a concept in any model of nursing that is being used to implement the nursing process. The paper provides an example of how nursing models may be adapted, at ward level, to guide everyday practice, whilst retaining the conceptual framework ofthe model. The inference of the paper is that such a process could be applied to other models resulting in a greater use of theory in everyday nursing practice.

The Bentall procedure is a surgical repair of an ascending aortic or aortic root aneurysm in combination with aortic valve disease. Less commonly, it is used to repair aortic dissection affecting the aortic root and valve. During the procedure, a composite aortic valve graft is used to replace the proximal ascending aorta and aortic valve. The procedure is performed through a median sternotomy during cardiopulmonary bypass. In this modification of the original procedure, coronary artery circulation is maintained by removing a full-thickness "button" of aorta surrounding the coronary ostia, making it easier to implant the proximal end of the coronary arteries into openings made in the aortic vascular graft.

Analysis of surgical nurses' attitudes toward postoperative vital sign (VS) assessment found tradition and ritual rather than research continue to drive postoperative VS assessment practices. Recommendations for practice, education, and research are made based on these findings.

This literature review aims to explore the appropriateness of the objective structural clinical assessment (OSCA) as method of assessment for graduate perioperative nurses. The methodology used involved searches of the following databases: CINAHL and Science Direct. The terms used in the search were: OSCE; OSCA; competenc* AND nurs* assess*/measure*/evaluat*; graduate nurse assess*; Benner AND assess* and Bondy rat* scale from January 2001 to August 2011. Strict inclusion and exclusion criteria were applied. This resulted in the inclusion of 10 papers. These papers predominantly addressed the need to define clinical competence, the lack of appropriate measurement instruments used in OSCAs – specifically; the reliability, validity and establishment of more rigorous methods used to define success in OSCAs. The paper concludes by finding that the literature does not achieve consensus regarding the definition of competence nor does it identify appropriate measurement instruments. Gaps and contradictions in the literature are evident, leaving doubts over the reliability of using OSCAs as a means to determine the notion of “clinical competence”. Longitudinal studies are needed to define clinical competence and to track the use and relevance of OSCAs in the clinical setting.

This document contains the results of a literature review of transformational and shared leadership theories through the lens of a nursing leader in a perioperative setting, using theory context and application. Transformational leadership is the preferred form of leadership for nursing.  The author selects and defends transformational leadership with strong emphasis on shared leadership, as the best and most effective leadership style to use when enacting change in a hostile nursing environment where there is overt resistance to change.  Involving followers and converting them into leaders of change is an effective and powerful way to evoke follower support and sustainability of changes in nursing practice and adoption of best practices.

report in which the Gordon Functional Health Patterns were used for data collection, with identification of Nursing Diagnoses according
to the diagnostic terminology by NANDA - I, and subsequent validation with patients treated between 2008 and 2009. Conclusion: the
PNCS protocol proposed allowed the process of nursing training at undergraduate level to be approached beyond the theoretical point
of view. The report of extension students showed the effectiveness of the Perioperative Nursing Care System on the quality of care and
nursing work process.

Background: The notion of preoperative visiting has been in existence since the 1960s and its value and worth in relation to healthcare delivery has been debated consistently since the term preoperative-visit is often confusing. However, little is known about knowledge, attitude and practice of preoperative visits among perioperative nurses in Nigeria. Background The use of intrathecal baclofen pumps (ITBP) has become increasingly common worldwide for the treatment of severe spasticity. Increased prevalence of patients using ITBPs makes encounters ever more common with surgical teams, these surgical interventions may be related to ITBP manipulation or completely unrelated to these devices. The anesthetist must be familiar with the normal functioning of ITBPs and be aware of baclofen pharmacokinetics in the cerebrospinal fluid, the occurrence of baclofen overdosing or withdrawal, and the potential pharmacological interactions between baclofen and drugs commonly used intraoperatively. Case We present the case of a successful management of a patient with chronic use of an ITBP which had to be suspended before undergoing a surgical procedure to perform intraoperative neuromonitoring to guide the surgical intervention. Conclusions A good communication and organization of the multidisciplinary team improves the prognosis of the patient and reduces morbidity and mortality.
