This webpage contains financial, academic and regulatory forms and documents for faculty and staff. Students who wish to submit forms may find instructions on The HUB Forms page. 

Academic Forms & Help Guides

Academic forms should filled out and submitted as PDFs. Please fill out forms with all required fields and signatures, providing any notes and comments directly on your form in the designated "comments" section. Before sending the form, be sure to print to PDF. Send all forms as attachments to registrar-forms@andrew.cmu.edu, unless otherwise noted.

If you wish to type your information into any of the PDF fillable forms below, you must first save the form and then reopen it in order to complete and submit the form. If you choose to complete your form by hand, please print clearly. At this time, we do not accept electronic signatures - so if a signature is required, please print and sign the form. Submit your completed form with the appropriate signatures electronically (via scan or photo) to thehub@andrew.cmu.edu, unless otherwise noted.
