    The Preschool Students will be participating in a Study Trip
    When: Monday, November 17, 2014
    Where: Clowes Memorial Hall of Butler University
    4602 Sunset Ave. Indianapolis, IN 46208

    Cost per student: $11.50.


    A additional $3.10 needs to be sent in for half-day children
    Money is due by Thursday, November 13, 2014. If money and form
    are not in on 11/13, your child will not have a ticket for the show.

    understand that my child is responsible for proper behavior and following teacher or
    staff instructions on this trip and that my child may be returned to school if there is a
    problem. I/We hereby release Saints Francis and Clare School, the Archdiocese of
    Indianapolis, and any and all of its employees from any and all liability for any and all
    harm brought upon my son/daughter as a result of this trip. I/We, the undersigned,
    have read this release and understand all its terms and execute it voluntarily and with
    full knowledge of its significance.

    SSFC reserves the right to prohibit any student who presents a discipline problem from participating in
    this study trip.
    All parents/guardians must have a current drivers license, proof of insurance, Safe & Sacred trained,
    a background check, and code of conduct if attending field trip.

    Information for children who are half-day and Tuesday/Thursday


    Half-day
    If your child is half-day they will be eating school lunch at 10:15.
    Please remember that you need to send in an additional 3.10$
    for their lunch. If your child is lunch bunch, there is no need to
    send in money because it will be taken out from their account.
    Tuesday/Thursday
    The trip is on a Monday, which means that they are switching a
    day to attend. Their two days for the week will be Monday,
    November 17th and Tuesday, November 18th
