Need for Service

Pullman is known as one of the poorest


communities in Allegan County.

There is only one medical office that is closed


to new patients however, a walk-in clinic is
available however no chronic management
for diabetes.

69.2% of Allegan County residents are obese


or overweight per BRFSS data

Above state averages for Heart Attacks, Diabetes and Stroke per BRFSS

17.3% no health care coverage among those ages 18-64 years

Unfortunately, the number of children living in poverty has continued


to trend upwards in Allegan

Ridge Diabetes Care is to
provide quality diabetes
management education in
order to empower the
community.

We strive to help our clients


with finding resources to
help with medical, financial,
and psychological aspects of
living with diabetes.

Philosophy

The philosophy of Pullman Ridge Diabetes Care is to


provide quality diabetes management education to
the community.

This includes but is not limited to individuals and


families with low income, limited English
proficiency, the uninsured, migrant and seasonal
farmworkers, homeless, and those living in public
housing.

We believe that education empowers and


strengthens the confidence of individuals living with
diabetes.

Goals and Objectives

Improve the quality of life for all persons in the community


of Pullman who have, or are at risk for diabetes mellitus.

Objective: To reduce health disparities in populations burdened by diabetes.

Objective: To enhance the program by bringing in additional volunteers.

Objective: To collaborate with resources in the community.

Objective: To detect and implement evidence-based, culturally appropriate


tools that support behavior modification.

Develop and enhance support systems, in the community of


Pullman, for those diagnosed with diabetes mellitus.

Objective: To identify and evaluate existing local support groups in or near


Pullman.

Objective: To develop five key strategies that encourages diagnosed


individuals to participate in basic education classes, workshops and/or
individual sponsorships.

Objective: To develop a Pullman diabetes web site.
