Rosiglitazone Avandia Action: Improves insulin resistance by hepatic glucose metabolism, insulin receptor kinase activity, insulin receptor

phosporylation. Therapeutic Effect Reduces hyperglycemia and hyperlipidemia, thus improving hyperinsulinemia without stimulating pancreatic insulin secretion Uses Adjunct to diet in the treatment of type 2 diabetes. May also be used in combination with Metformin. Indication: Indicated as monotherapy as an adjunct to diet and exercise to improve glycemic control in patients with type II Diabetes Mellitus (DM). Also indicated for use in combination with Metformin or with Sulfonylurea when diet, exercise and rosiglitazone alone or diet, exercise and Metformin or Sulfonylurea alone does not result in adequate glycemic control in patients with type II Diabetes. Contraindication: Diabetic ketoacidosis or precoma; renal failure; type I Diabetes Precaution: Patients with type 1 Diabetes or for the treatment of Diabetic ketoacidosis Adverse Reaction: Urinary infection (UTI), headache, backpain, hyperglycemia, fatigue, sinusitis, diarrhea, viral infection, arthralgia, weight gain, lactic acidosis Nursing Considerations Monitor laboratory results closely Monitor hemoglobin Alc, serum glucose; signs and symptoms of fluid retention or heart failure; liver enzymes (prior to initiation of therapy, then periodically thereafter) Patients with an elevation in ALT >3 times the upper limit of normal should be rechecked as soon as possible. If the ALT levels remain >3 times the upper limit of normal, therapy with rosiglitazone should be discontinued. Monitor weight Advise women using oral contraceptives about need for alternative method of contraception

Assess for hypoglycemic reactions ( sweating, weakness, dizziness, anxiety, tremors, hunger), hyperglycemic reactions soon after meals Assess CBC (baseline every 3 months) during treatment; check liver function tests periodically; AST, LDH,FBS, ALT (if ALT >2.5XULN, do not use). HbA2c, fasting plasma insulin, plasma lipids, lipoproteins, B/P, body weight during treatment.
