TERMS OF REFERENCE

State Consultant Integrated Management of SAM - 1 position


1.
BACKGROUND
According to NFHS1 3 (2005-06), in Jharkhand, about half (47.2%) of the children under 3
years are stunted, or too short for their age, which indicates that they have been
undernourished for some time. One-third (35.8%) children are wasted, or too thin for
their height, which may result from inadequate recent food intake or a recent illness.
About 54.6% are underweight, which takes into account both chronic and acute undernutrition. Children in rural areas are more likely to be undernourished, but even in urban
areas almost two out of five children suffer from chronic under-nutrition.
NFHS 3 also highlighted that the situation is worse among tribals, where 12% of the
children aged 0-59 months are severely wasted and around 30% are severely stunted.
This not only compromises the ability of the children to grow to their full potential but
also puts them at a higher risk of dying. Severely wasted children are more susceptible to
the risk of dying (9 times higher than a normal child).
To address the situation, Department of Health and Family Welfare runs 88 malnutrition
treatment centers (MTC). There are at least 2-3 MTCs in each district which provide
treatment to children with severe acute malnutrition (SAM) with medical complications.
At present, to treat children with SAM, >1000 beds are available across the state. UNICEF
provides techno-managerial, capacity building and technical support to the department
to improve the quality of care at the MTCs.
Both facility and community based care and treatment are essential for children with
SAM. It is important to establish and strengthen the continuum of care for such children,
therefore effective linkages and convergence between the Department of Health and
Family Welfare and Department of Women and Child Development needs to be
strengthened.
UNICEF is committed to support the state government to establish the continuum of care
for children with SAM through an integrated programme. Alongwith the treatment the
focus of an integrated programme should be on prevention by promotion of Infant and
Young Child Feeding practices at all levels.
This consultancy is proposed to provide technical support to the Department of Health
for scaling up of MTCs with quality, as well as establishing linkages with the community
based programmes.
2.
PROGRAMME AREA AND SPECIFIC PROJECT AREA

PCR 1: Infants, young children and mothers have equitable access to and utilize quality
services for child survival, growth and development
Intermediate Result 1.3: Governments and partners can scale up programmes to
provide essential nutrition care and support for children who are severely
undernourished, sick and/or affected by disasters, particularly in the most deprived

4.
OBJECTIVES
The aim is to provide technical and programmatic support to NRHM in the state
primarily towards management of Severe and Acute Malnutrition. Key objectives are:

Provide technical assistance in planning, implementing and monitoring of initiatives


for management of children with SAM and focus on prevention by improving IYCN
behaviors, with special focus to the most vulnerable and unreached population
groups.
Provide support for the development of technical modules for capacity building of
functionaries on F-SAM and C-SAM
Coordinate with the departments to conduct review meetings and ensure
convergence to strengthen the continuum of care for children with SAM

5. DUTY STATION
The incumbent(s) will be based out of Ranchi, with travelling to districts for monitoring
and supportive supervision
6. SUPERVISOR
Supervisor: Nutrition Specialist, UNICEF office for Jharkhand
7. MAJOR TASKS TO BE ACCOMPLISHED

Support NRHM to adhere to the National Guidelines of F-SAM and its


adaptation in the state
Support ICDS and NRHM to adhere to the CMAM guidelines (as agreed upon by
the state technical committee) and updation of training module
Support in improving the technical capacity of partners and government on
Integrated Management of Severe and Acute Malnutrition
Support the department of health towards the development of PIP on Facility
based Management of SAM and adequate allocation for strengthening of MTCs
Support the development of training PIP for staff of MTCs with adequate
resources from NRHM
Support ICDS for capacity building of functionaries on SAM and expansion of CMAM
Support towards the development and administration of tools and checklists
for MTCs (F-SAM) to assess the quality of service delivery at MTCs, as per
standard operating procedures
Support in development of innovative plans and strategies to improve the
quality of services at MTC.
Support in analysis of reports from MTCs; collect, analyse and prepare report
on the indicators and ensure the achievement of standards of care by
providing supportive supervision wherever needed (based on reports)
Ensure that linkages between IYCN and management of SAM are incorporated
into the PIP
Facilitate coordination with Jharkhand State AIDS control Society for capacity
building of ART staff and counsellors on management of SAM
Coordinate the work of consultants at the state and district level to improve
outcomes at MTCs (cure rate, defaulter rate)

Carry out field validation to assess, explore and improve the quality of service
delivery
Any other functions deemed necessary by UNICEF/ Department-WCD and
NRHM, as and when required

There could be overlap in some of the tasks for the above deliverables. If
deliverables are delayed due to extraneous circumstances (in extra ordinary
cases and satisfaction of the supervisor) monthly payments can be claimed
against submission of a Progress Report

10.

OFFICIAL TRAVEL INVOLVED

Travel is required within the state and in special cases, he/she may be required to travel
outside the State. If such travel would contribute to the achievement of the assignment,
as approved by the Supervisor. The Consultant will be on travel for approximately for 6
days in a month (approximately 66 -70 days for the entire duration of the contract). The
Consultant would be required to make own arrangement for travel and stay during travel,
and bear the cost for the same. Consultant will submit the prospective travel plan to the
Supervisor at the end of previous month.
11. QUALIFICATIONS AND COMPETENCIES:

Post graduate degree/diploma in Nutrition/ Public Health Nutrition/MPH/ MHA/

Work experience of 4-5 years or more in the field of maternal health, child
development and nutrition and emergency nutrition.
Experience of working with UN agencies, or international organizations will be
an added advantage
Understanding of social mobilization and behavior change communication
Good analytical, negotiation and communication (oral and written) skills
Computer literate and conversant with MS Office and its application, internet,
e-mail
Knowledge/familiarity with the national flagship programmes (NRHM, ICDS and
others) and experience of working with Government is an asset
Demonstrated ability to work in a multi-disciplinary environment and to
establish harmonious and effective working relationships with governmental,
non-governmental and civil society organizations
Knowledge of English essential, working knowledge of Hindi would be an
added advantage

The selection will be on the basis of technical evaluation & financial proposal in the ratio of
80:20. The technical criteria for evaluation is as follows:

Written Test : 30 marks
Interview: 30 marks
Total score - 100 Minimum qualifying marks (70 i.e. 70% of the total)
