Evidence of one of your parents' claim to British nationality. Provide one of the following:
the parent's full birth certificate or
the parent's certificate of registration or naturalisation and
if this is your father, his marriage certificate to your mother and evidence of termination of any previous
marriage if applicable.
Evidence of one parents immigration status at the time of your birth
the parents passport at the time of your birth and
if this is your father, his marriage certificate to your mother and evidence of termination of any previous
marriage if applicable.
Change of name evidence. If the name on the documentation you provide is different to the name you want to
have shown on your passport, provide proof of every change of name. Provide at least one of the following for
each change:
Marriage certificate
Civil Partnership certificate
Gender Recognition certificate
Enrolled deed poll
Change of name deed
Certificate of naturalisation or registration
Statutory Declaration or Affidavit
Birth Certificate (upon re-registration)
Certificate from the Court of the Lord Lyon of Scotland
Form PD2 for those about to marry or form a civil partnership
If you are reverting back to your maiden or unmarried name, all of the following are required:
Your birth certificate and
A decree absolute showing both names and
A marriage certificate showing both names and
A signed statement saying that you now use your maiden name for all purposes
If changing the spelling of your name slightly (e.g. Bryan to Brian), changing the order in which your forenames
appear in your passport or dropping a forename, one of the following is also accepted:
Letter from a council or government department
Driving Licence
Bank Statement
Baptismal/Confirmation Certificate.
If the change of name relates to a child, you must provide evidence that everyone who has parental
responsibility for the child has given permission to change the childs name.
