    AFFIDAVIT OF DISCREPANCY
    I, ZENAIDA VARGAS ALIPANTE, of legal age, Filipino citizen, widow and
    a resident of No. 80 Block 5 Antique Street, Bago Bantay, Quezon City, in
    accordance with law hereby depose and state that:
    1.
    I am married to ROGERIO ALABAT ALIPANTE, solemnized
    last March 3, 1960 and registered with LCR Registry No. 39 at the Office
    of the Local Civil Registrar of Labo, Camarines Norte;
    2.
    My husbands legitimate parents were Catalino Alipante and
    Mercedes Alabat born on November 7, 1941;
    3.

    He died due to illness last February 2, 2008 at Naga City;

    3.
    He was a member of the Social Security System (SSS) with
    SS No. 05-0158887-2;
    4.
    Upon securing the marriage contract records of my husband
    from the National Statistics Office (NSO) it appeared that the surname of
    my husband was erroneously written as ALFANTE instead of
    ALIPANTE;
    5.
    The true and correct
    ALIPANTE and not ALFANTE;

    This is executed to attest to the veracity and truthfulness of
    the foregoing.
