    NEAR MISS DESCRIPTION:
    -On TUESDAY, January 02th, 2017 at 09.24 Am in STEAM TURBINE after starting the work of main
    template assembly (on ground( mentioned in PTW( LW2017- 710(/(LW2017-711 ( ,Collision head
    crane with steel structure . because absence of signal men during lifting equipment .

    However, the J/V HSE saw the process and alerted the worker who was passing Next to equipment
    .HSE immediately to interfere and save the situation in order to stop and to avoid any human or
    material loses.
    ESTIMATE OF LOSS POTENTEL:
    - NO INJURY NO DAMAGE

    ANALISIS:
    - Absence hse supervisor of sinoconst during of lifting operation.
    - absence of signature and revalidation of hse supervisor in PTW.
    - absence signature of workers concerned with this operation in TBM SHEET .
    - absence of signal men during lifting equipment .
    - absence the soft barricade around area of lifting operation.
    - Do not exploit tagline during lifting operation .

    REQUIRED ACTION TO PREVENT RECURRENCE AND TO ELIMINATE RISK FACTORS:


    1- Compliance with HSE instructions
    2- Must follow and respect the PTW procedures and precautions.
    3- Ensure supervision of hse during lifting operation
    4- must installed soft barricade around area of lifting operation
    5- must secure loose objects.
