AFFIDAVIT OF QUITCLAIM

WITH INDEMNITY UNDERTAKING

We, JUANITO ABELLAR, of legal age, Filipino, FRANCINE ABELLAR, of


legal age, Filipino, FRANCESCA ABELLAR, minor, Filipino, and BASILLO
ABELLAR, minor, Filipino, all residents of Jaro Iloilo City subscribing under oath,
depose and state:

1. That I (JUANITO ABELLAR) am the legally married spouse of IMELDA


DOMINGO ABELLAR, who died intestate on 14 February 2018 in Cebu City, per
Certificate of Death No. 20131749, a copy of which is hereto attached as integral part
hereof.

2. That we: (FRANCINE ABELLAR, FRANCESCA ABELLAR, and BASILLO


ABELLAR) are the legitimate children of Juanito Abellar and the deceased Imelda
Domingo Abellar.

3. That at the time of his death, Imelda Domingo Abellar left certain personal
properties consisting of bank deposits at UNIONBANK CEBU-MANDAUE BRANCH
under Savings Account No. 100570118625.

4. That as a matter of law, the said bank deposit is Absolute Community Property
of which one-half (1/2) thereof belongs to me (JUANITO ABELLAR) and the other
one-half (1/2) belongs to my husband Imelda Domingo Abellar.

5. That being the surviving heirs of Imelda Domingo Abellar, WE are the only
persons legally entitled to inherit the one-half (1/2) share of Imelda Domingo Abellar
in Savings Account No. 100570118625 at UnionBank Cebu-Mandaue Branch.

6. That to the best of our knowledge, no other person is rightfully entitled to the
money under the said savings account of Imelda Domingo Abellar except US, being
the surviving spouse and legitimate children of the decedent.

7. That upon receipt of the proceeds of said savings account, WE undertake to


render the Bank free and harmless against any and all claims by third parties
whatsoever.

8. Wherefore, for and in consideration of the foregoing premises, and upon OUR
actual receipt of the proceeds of Savings Account Number 100570118625, WE hereby
freely and voluntarily RENOUNCE, WAIVE, QUIT CLAIM, and FOREVER
RELEASE any and all claims and/or causes of action which WE may have or hereafter
have, against the Bank, its officers and agents, over Savings Account
Number100570118625, in a manner that is absolute and irrevocable.

IN WITNESS WHEREOF, WE have hereunto affixed my signature on this
